ADDENDUM TO 52.212-1

The following additions/changes are made to FAR provision 52.212-1, which is incorporated by reference. Information found in this addendum supersede FAR 52.212-1. Non-conformance with the instructions provided may result in an unfavorable proposal evaluation and/or elimination from consideration for award.

1. Instructions to Offerors

a. The Government reserves the right to revise or amend the specifications, drawings or the solicitation prior to the Request for Proposal (RFP) closing time. Such revisions or amendments, will be communicated by amendments to this RFP. If such amendments require material changes in quantities or prices, the RFP closing date may be postponed by enough days to enable offerors to revise their proposals. In such cases, the amendment will include an announcement of the new RFP closing date and time. Amendments will be posted on the Internet with the solicitation documents without notice.

Offerors are responsible for accessing all amendments prior to proposal submission at www.fbo.gov.

All solicitation amendments must be acknowledged and turned in with offer.

2. General Information.

a. Solicitation number: FA481419RA007

b. Requests for clarification. Address any questions or concerns you have to the CO or CA. Written requests for clarification may be sent by e-mail to Felix.Castro_Espinosa.1@us.af.mil, melissa.biggar@us.af.mil, or jace.anders@us.af.mil or by mail to the address located in block 9 of the standard form 1449. No phone calls will be taken. All requests for clarification/questions are due no later than 13 Aug 2019 at 9:00 AM Eastern Time.

c. Receipt of offerors. Offers are due no later than 23 August 2019 at 9:00 AM Eastern Time to:

The sealed envelope or package used to submit your proposal must show the time and date specified for receipt, Solicitation Number and name and address of the offeror. Offerors are cautioned that MacDill AFB, FL has visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor pass prior to entrance. SOME DELAY SHOULD BE ANTICIPATED WHEN HAND CARRYING PROPOSALS. Offerors should allow sufficient time to obtain a visitor pass and to deliver the proposal PRIOR to the time specified for receipt. This includes delivery carriers.

The offeror is solely responsible to make sure their offers are received by the deadline. PROPOSALS MUST BE RECEIVED prior to deadline. Late proposals will be processed in accordance with FAR 52.212-1 (f) “Late submission, modifications, revisions and withdrawals of offers.”

d. “Notice to Offeror(s)/Supplier(s): The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs.” This Solicitation is being issued IAW FAR 52.232-19.

e. Points of Contact – The Contracting Officer (CO) and Contract Administrators (CA) are the sole points of contact for this acquisition. The CA for this solicitation is SSgt. Felix Castro Espinosa and alternate Mrs. Melissa Biggar they can be reached by email at Felix.castro_espinosa.1@us.af.mil & melissa.biggar@us.af.mil The CO for this solicitation is Mr. Jace Anders and he can be reached by email at jace.anders@us.af.mil.

f. Period for acceptance of offers. OFFERORS FOR ENTRANCE WHICH MAY PROHIBIT SUBMISSION OF HANDCARRIED

OFFERS; CONSEQUENTLY, OFFERORS ELECTING TO HANDCARRY THEIR PROPOSALS

FOR SUBMITTAL JUST PRIOR TO THE RFP CLOSING TIME DO SO AT THEIR OWN RISK.

3. Proposal Preparation Instructions.

a. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or sub factors. Failure to meet a requirement will result in an offer being ineligible for award. The offeror shall prepare submissions as set forth in the organizational table below. The titles and contents of the volumes are defined in the below table, all of which shall be within the required page limits and with the number of copies shown below.

b. Page Limitations – Page limitations shall be treated as maximums. If exceeded, the excess pages will not be read nor considered in the evaluation of the proposal and (for paper copies) will be returned to the offeror as soon as practicable. Each page shall be counted except the following: cover page, table of contents, tabs, and glossaries.

c. Glossary of Abbreviations and Acronyms – Each volume shall contain a glossary of all abbreviations and acronyms used, with an explanation for each. Glossaries do not count against the page limitations for their respective volumes

d. Page Size and Format – A page is defined as each face of a sheet of paper containing information.

When both sides of a sheet display printed material, it shall be counted as two pages. Page size shall be 8.5 X 11 inches, not including foldouts. Pages shall be single spaced and text size shall be no less than 12 point. Use at least 1 inch margins on top and bottom and ¾ inch side margins. Pages shall be numbered sequentially by volume. Legible tables, charts, graphs, and figures shall be used wherever practical to depict organizations, systems and layout, implementation schedules, plans, etc. These displays shall be uncomplicated, legible and shall not exceed 11 X 17 inches in size. Foldout pages shall fold entirely within the volume and count as a single page. Foldout pages shall not be used for text. These limitations shall apply to electronic submissions.

e. Labeling – Indicate on each CD the volume number and title. Use separate files to permit rapid location of all portions, including sub factors, exhibits, annexes, and attachments if any. Electronic copies shall be submitted in a format readable by Microsoft Office.

f. Electronic Offers – The content and page size of electronic copies must be identical to the hard copies.

For electronic copies, indicate on each CD the volume number and title. Use separate files to permit rapid location of all portions, including sub factors, exhibits, annexes, and attachments if any.

Electronic copies shall be submitted in a format readable by Microsoft Office. As part of the provided digital copy (compact disk), offerors must provide the completed Excel document (not a scanned or copied) for evaluation. It is the offerors responsibility to ensure the electronic offer can be accessed by the Government.

4. General Format and Content

The offeror shall submit the following volumes of material. A complete proposal consists of the following volumes:

a. Volume I – SF 1449, Representations and Certifications, and any parts of the request for proposal with fill-in blanks and solicitation attachments as applicable, i.e. excel spreadsheets at Attachment 2, Bid Schedule. Complete the Representations and Certifications IAW 52.212-3 and/or SAM website (https://www.sam.gov/portal/SAM/). Fill in blanks, when appropriate in any sections of the request for proposal.

b. Volume II Technical - The technical proposal shall be prepared in accordance with these instructions and shall be evaluated in accordance with the evaluation criteria and evaluation standards in 52.212-2, Evaluation Factors for Award. Technical proposals shall be used by the evaluators to determine offeror's ability to perform the required services in the PWS.

c. Volume III –Past Performance. The past performance evaluation results in an assessment of the offeror’s probability of meeting the solicitation requirements. The past performance evaluation considers each offeror’s demonstrated recent and relevant record of performance in supplying products and services that meet the contract’s requirements. In accordance with FAR 15.305(a) (2), the currency and relevance of the information, source of the information, context of the data, and general trends in contractor’s performance shall be considered. These are combined to establish one performance confidence assessment rating for each offeror IAW 52.212-2, Evaluation Factors for Award. Offeror is required to submit the following information for each Past Performance record:

-Company/Division name -Product/Service -Contracting Agency/Customer -Contract Number -Contract Dollar Value -Period of Performance

-Verified, up-to-date name, address, telephone number and email address of the contracting officer or any representative able to validate performance history -Summary of services provided and a description of how closely the provided services performed under those contracts relate to the Performance Work Statement

-Comments regarding compliance with contract terms and conditions -Comments regarding any known performance deemed unacceptable to the customer, or not in accordance with the contract terms and conditions.

Each offeror is requested to submit the information shown in Volume III for each relevant contract 5 days prior to the date set for receipt of proposals. Offerors shall complete Section 1, 2 and 3 of the attached Past Performance Questionnaire (Attachment 8) and e-mail it to all points of contacts (POCs) the offeror has listed as past performance references. The POCs listed in the proposal should represent the individuals most knowledgeable about the Offeror’s performance on the identified contract or program, such as the following: (1) Contracting Officer’s Technical Representative; (2) Program/Project Manager; (3) Contracting Officer. For commercial contracts, send the questionnaires to the commercial equivalents of these individuals. The POCs will complete the questionnaires and forward them by email, directly to the attention of Felix.Castro_Espinosa.1@us.af.mil, melissa.biggar@us.af.mil, and jace.anders@us.af.mil. RESPONDENTS TO THE

QUESTIONNAIRES SHALL NOT SEND THE COMPLETED INFORMATION SHEETS

The Government reserves the right to use both the information provided in the Offeror’s Past Performance submittals and information obtained from other sources available to the Government, to include, but not limited to, the Past Performance Information Retrieval System (PPIRS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), or other databases; interviews with Program Managers, Contracting Officers, and commercial sources and/or non-Government sources, such as any commercial business or customers referenced by the Offeror in any of the past performance information disclosed.
