PERFORMANCE WORK STATEMENT (PWS)
FOR
INSTALLATION OF CARPET AND TILE SERVICES
HOLLOMAN AIR FORCE BASE, NM

1.0. DESCRIPTION OF SERVICES. The contractor shall provide all personnel, equipment, tools, materials, supervision, transportation, and other items and services necessary to perform an installation of Government/Contractor-Furnished Carpet/Padding as defined in this Performance Work Statement (PWS).
1.1.
Carpet, cushion and floor tile installation.
1.1.1.
The government will ensure all facilities that require removal of existing carpet/cushion and floor tile under a specific work order will be free of asbestos hazards.
1.1.1.1.
Asbestos Containing Material Identification (ACM). The contractor shall not start any work that may disturb any materials in a facility unless and until all materials that will be disturbed through the performance of this contract have been identified as asbestos-free by the government. The contractor shall not commence work without the Quality Assurance Personnel’s (QAP) approval the performance area is asbestos-free.
1.1.2. Materials
1.1.2.1. CARPET SPECIFICATIONS: Carpet and color numbers/specifications shall be approved by 49 Civil Engineering (CES) QAP.
A. 100% Nylon
B. Density factor of 5,000
C. Face weight, 26 minimum
D. 1/10 gauge
E. Level loop surface texture
F. Woven carpet or synthetic with polypropylene backing
G. 25 or less fire spread rating
H. 450 or less smoke density
I. 3.5 KV. Or less static control
J. Dye method is solution type
K. Pattern must be random
1.1.3. The contractor shall perform an on-site inspection of the work area to determine requirements for carpet/padding installation and confirm dimensional requirements. If the contractor finds any discrepancy in the dimensions between the data provided by the government and the contractor’s survey; the contractor is required to notify the QAP within 24 hours or the next duty day.
1.1.4. Remove Carpet. The contractor shall remove all existing carpet/padding/cove base as specified per Delivery Order and dispose of it properly only at authorized off base locations.
1.1.5.
Prepare Floor. The contractor shall ensure concrete and tile floor surfaces are smooth and level to the eye to provide a proper surface for installation of new carpet/padding. The contractor shall fill all cracks, holes, and crevices with material compatible to the floor.
1.1.6.
Install Padding. The contractor shall install carpet cushion when required. Foamex style # 31-11 CX2580, ¼” thick X60” long, high density rubber, and 80 ounce “or equal”. If “or equal” product is used, contractor must get approval from QAP.
1.1.7.
Contractor shall not use padding on staircases. The contractor shall ensure carpet adhesives (glue) are approved by the QAP. The contractor will be informed in advance concerning which unit(s) will require padding versus which unit(s) will require adhesive (glue), which will be specified in each Delivery Order.
1.1.8. Fitting Carpet.
1.1.8.1. Installation Carpet and cushion shall be installed as per manufacturer’s specifications. contractor shall be responsible for preparation of floor surfaces as necessary to receive carpet and cushion. All surfaces to be free of voids and protrusions. Installed carpet shall be smooth, uniform and secure. Patterned goods shall be accurately matched. Contractor to remove and dispose of all existing floor coverings prior to installation of new floor coverings.
1.1.8.2. Door Clearances. The contractor shall ensure clearance between the bottom edges of doors and the level floor surface of newly installed carpet. All doors shall open freely or close without dragging the newly installed carpet. If the door is dragging, the contractor shall remove the door, trim it, and sand the trimmed doors smooth.
1.1.8.3. Unaffected Areas. Protect all unaffected areas with plastic sheeting, drop cloths, periodic vacuuming, or sweeping from work debris.
1.1.8.4. Remove Rubbish. The contractor shall remove rubbish, wrapping paper, salvages and scrap. The contractor shall not leave waste material inside the facility overnight or for a prolonged period of time, (more than 2 days) when work is not done. Remove and dispose of waste materials only at authorized off base locations daily.
1.1.8.5. Vacuum Carpets. The contractor shall vacuum new carpets to remove all debris and waste.
1.1.8.6. Final Cleaning. The contractor shall remove spots, smears, and other foreign matter with solvent. Use a solvent, which will not discolor or damage carpet surface to include cove bases.
1.1.8.7. Replace Furniture. The contractor shall return all furniture to its original location after final cleaning.
1.1.9. Install Cove Base and Edge Strip.
1.1.9.1.
Vinyl Cove base. The contractor shall furnish and install vinyl cove base as directed. Removal of existing materials shall be as required to support the work.
1.1.9.2.
Materials. 120 feet coils: 1/8 inch thick, homogeneous vinyl or rubber composition, tapered or bullnose edge, color to match flooring, or as selected from standard colors available; not less than 1 inch wide.
A. Height: 4 inch and 6 inch B. Thickness: 1/8 inch
C. Style: Standard top set cove
D. Finish: Matte
1.1.9.3.
Resilient Risers. Provide single piece riser for height and width of stair risers or equal sized units if riser width exceeds available manufactured lengths.
1.1.9.4.
Resilient Stringer Skirt. Cut to match riser and tread provide and to meet wall base height, of same material and color as base unless otherwise indicated.
1.1.9.5.
Adhesives. Waterproof, stabilized type as recommended by wall base manufacturer to suit material and wall conditions.
1.1.9.6.
The contractor shall provide and install aluminum anodized edge strip at all transitions from carpeted to non-carpeted areas.
1.1.9.7.
Samples for verification purposes of each different color and pattern of Vinyl Composition Tile, showing full range of variations.
1.2. Vinyl Composition Tile (VCT) Flooring. The contractor shall furnish and install VCT tile flooring as directed. Installation procedures shall be in accordance with the product manufacturer’s recommendations. Removal of existing materials shall be as required to support the work.
1.2.1.
Materials.
A.
Vinyl composition floor tile. Products complying with ASTM F 1066, Composition 1 (non asbestos formulated)
B. Class: Class 2 (through pattern tile)
C. Wearing Surface: Smooth
D. Thickness: 1/8 inch E. Size: 12-by-12 inches 1.2.2.
Provide product data for vinyl composition floor tile. Samples of material shall be submitted for color selection to the QAP.
1.2.3.
Maintenance data for VCT floor tile. Contractor shall submit maintenance data to the QAP.
1.2.4.
Delivery, Storage, and Handling. Deliver tiles and installation accessories to project site in original manufacturer's unopened cartons and containers each bearing names of product and manufacturer, project identification and shipping and handling instructions.
1.2.5.
Store flooring materials in dry spaces protected from the weather in accordance with manufacturer’s instructions. Store tiles on flat surfaces. Move tiles and installation accessories into spaces where they will be installed at least 48 hours in advance of installation.
1.2.6.
Sequencing and Scheduling. Contractor shall coordinate with the facility manager and the QAP for the installation of the floor tile. Install tiles and accessories after other finishing operations, including painting, have been completed. Do not install tiles over concrete slabs until the slabs have cured and are sufficiently dry to bond with adhesive as determined by tile manufacturer's recommended bond and moisture test.
1.2.7.
Preparation. Comply with manufacturer's installation specifications to prepare substrates indicated to receive tile.
1.2.8. Apply concrete slab primer, if recommended by flooring manufacturer, prior to applying adhesive. Apply according to manufacturer's directions.
1.2.9. Installation. Comply with tile manufacturer's installation directions and other requirements indicated as applicable to each type of tile installation included in project.
1.3. Ceramic Tile. Installation procedures shall be in accordance with the product manufacturer’s recommendations. Removal of existing materials shall be as required to support the work.
1.3.1.
Materials.

Size: 12-by-12 inches
1.3.2.
Provide product data for ceramic tile. Samples of material shall be submitted for color selection.
1.3.3.

1.3.4.
Maintenance data for ceramic tile. Contractor shall submit maintenance data to the QAP.
1.3.5.

1.3.6.

1.3.7.

1.3.10. Grout tile to comply with manufacturer’s instructions.
1.4. Furniture Movement. The contractor shall relocate any government furniture as specified per Delivery Order from the area where the carpet/padding, vinyl cove base, ceramic tile or Vinyl Composition Tile will be installed, removed or altered.

3.0. GOVERNMENT-FURNISHED PROPERTY AND SERVICES.
3.1.1.
The government may provide carpet to the contractor to accomplish the subject contract. The contractor shall arrange for pick up of government-furnished carpet by contacting the QAP. The contractor shall be responsible for safeguarding all government Property provided for contract use.
3.1.2.
The following government furnished material may be provided to the contractor.

3.1.3.
The contractor shall make sure carpet, padding, tack strips and vinyl cove base, as applicable, are not damaged upon receipt. The contractor shall replace any items lost at the contractor's expense.
3.1.4.
The contractor shall return any unused government-furnished material to the warehouse (Bldg. 49) at the end of each project. Each project is tracked by the QAP.
3.1.5.
Office and Storage Facilities: The contractor is not required to have an office or storage facilities on HAFB. If the contractor chooses to have an office the government will provide a 50’ x 100’ lot within a reasonable distance of utilities. The contractor shall be responsible for securing the area. The contractor shall be responsible for any costs associated with their on-site office and storage facilities to include mobilization, demobilization, trailers, skirting, utility lines, connections and site restoration. Contractor on-site offices shall comply with handicapped accessibility laws and regulations. The contractor shall restore the site to its original condition within 30 days of contract completion.
3.2.
Available utilities: Subject to available supply, the government, without charge to the contractor and from existing outlets and supplies will furnish electricity, gas, water and sanitary sewer capacity. The contractor at their own expense, shall install, maintain and remove any necessary temporary connections and distribution lines. If temporary lines are associated with a specific Task Order, they shall be removed prior to the final acceptance meeting.
3.3.
Conservation of Utilities. The contractor shall instruct employees in utilities conservation practices. The contractor shall be responsible for operating under conditions, which prelude the waste of utilities.
3.4.
CONTRACTOR-FURNISHED ITEMS AND SERVICES.
3.4.1.
Accessories. The contractor shall furnish carpet, edge strips, glue, adhesive, filler, and like items for a complete installation and finished appearance.
3.4.2.
Notification to Commence Work. The contractor shall begin carpet/padding installation within five (5) working days after service notification is received from the contracting officer. The contractor will receive work order information from the QAP by means of face-to-face communication, e-mail, fax or phone and acknowledgment of job order must be submitted to QAP. The government will provide the contractor with the facility number and floor plan.
3.5.
Contractor-Furnished Material Submittals. Except for those items or services specifically stated to be Government-Furnished in Section 3 of the PWS, the contractor shall be responsible for providing the necessary materials/supplies required to perform the requirements of this PWS. This includes, but not limited to, Cove base, edge strips, VCT, and ceramic tiles. The contractor shall submit Material Approval Submittal (AF Form 3000), along with a sample of each product for the contracting officer’s approval prior to start of each job order. Asbestos-free certification is required for any possible ACM (i.e., glue/adhesive, joint compound, etc,.) when you submit AF Form 3000 for approval. The installation may recommend the substitution of a non-hazardous chemical if a hazardous material is used.
4.0.
GENERAL INFORMATION.
4.1.
Quality Control. The contractor shall develop and maintain a quality control program to ensure the systematic of the contract are provided as specified in accordance with the standards accepted in commercial practices. The contractor shall develop and implement procedures to identify and prevent defective services from recurring. As a minimum, the contractor shall develop quality control procedures addressing the areas identified in the paragraph 2, Service Summary. The contractor shall submit the Quality Control Plan (QCP) ten (10) days after contract award. The QAP and contracting officer review the contractor’s QCP.
4.1.1.
Material Application Guarantee. The contractor shall guarantee the workmanship for a one-year period from the date of completion.
4.2.
Quality Assurance. The government will periodically evaluate the contractor’s performance by appointing a representative(s) to monitor performance to ensure services are received. The government representative will evaluate the contractor’s performance through intermittent on-site inspections of the contractor’s quality control program and receipt of complaints from base personnel. The government may inspect each task as completed or increase the number of quality control inspections if deemed appropriate because of repeated failures discovered during quality control inspections or because of repeated customer complaints. Likewise, the government will also receive and investigate complaints from various customers located on the installation. The QAP shall be responsible for initially validating customer complaints. However, the government representative shall make final determination of the validity of customer complaint(s) in cases of disagreement with customer(s).
4.3.
Hours of Operation. The contractor shall perform the on-site services required under this contract during the following hours: 7:30 am - 4:30 pm, Monday through Friday, except on U.S. holidays specified in section 4.4.1. of this PWS. No on-site work shall be scheduled or performed on holidays unless previously approved or directed by the C.O. If the contractor desires to work on non-scheduled workdays, he/she must submit a written request to the C.O. for approval.
4.3.1.
Holidays. The contractor is required to provide service, except as noted on the following days:

*If holiday falls on a Saturday, it will be observed on the preceding Friday. If it falls on a Sunday, it will be observed on the following Monday.
4.4. Security Requirements. Project is unclassified – access to classified information not required.
4.4.1. Illegal Aliens. The contractor shall not hire an illegal alien to work or perform any service on Holloman AFB. Employees who are not U.S. citizens can only be persons possessing a current resident alien immigrant status (Green Card) or if a non-resident immigrant must possess a current Employment Authorization Document. While on Holloman AFB, non-U.S. citizen employees must be able to produce for inspection the appropriate credential: Green Card or Employment Authorization Document. Any illegal alien found on the installation shall be immediately detained by the base security forces and barred from the installation.
4.4.2. Foreign National Visitor Approval. The contractor must obtain foreign national visitor approval for all employees requiring access to Holloman AFB who are not U.S. citizens. The 49 MSG/CC is the approval authority; the 49 MSG/CC may delegate approval authority to the 49 SFS/CC.
4.4.2.1. The contractor must submit visit approval requests for all foreign national employees through the Contracting Officer to the 49 MSG/CC or 49 SFS/CC (if delegated) using the Holloman AFB Foreign Visit Request Form (available at the Holloman AFB Welcome Center).
4.4.2.2. Once coordination through the Contracting Officer has been completed, the Holloman AFB Foreign Visit Request Form shall be forwarded to 49 SFS/S5AV for coordination.
49 SFS/S5AV will submit the completed form to the 49 MSG/CC or 49 SFS/CC (if delegated) for approval. The Contracting Officer listed on the Foreign Visit Request Form will be contacted when a final decision has been made.
4.4.2.3. The Foreign Visit Request Form will be maintained by 49 SFS/S5AV and the Air Force Office of Special Investigation until the visit end date.
4.4.2.4. Employees who are not U.S. citizens will not be allowed to enter the installation until properly approved via the procedures outlined above.
4.4.3. Listing of Employees. The contractor shall maintain a current listing of employees that require access to Holloman AFB. The list shall include the employee's name, social security number and type of investigation if contract work involves unescorted entry to Air Force (AF) restricted areas or other sensitive areas designated by the installation commander. The list shall be provided to the Program Manager and sponsoring agency security manager. An updated listing shall be provided when an employee's status or information changes (i.e. terminated for cause, reassignment, retirement, etc.) or when a new employee is added.
4.4.4. Security Training. The contractor ensures all employees receive initial and recurring (annual) security education training from the sponsoring agency security manager. Training must be conducted IAW DOD 5200.1-R, Information Security Program, and AFI 31-401, Information Security Program Management. Contractor personnel who work in AF controlled/restricted areas must be trained IAW AFI 31- 101, Integrated Defense.
4.4.5. Pass and Identification Items. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees. Employees shall only be granted access to the installation to perform functions directly related to their respective contract.
4.4.6. Retrieving Identification Media. The contractor shall return all identification media from employees who depart for any reason before the contract expires; i.e., terminated for cause, reassignment, retirement, etc. The contractor will notify the Government within 24 hours when unable to retrieve identification media from contract employees.
4.4.7. Travel Restrictions: Contractors are prohibited in areas of the installation other than their appropriate place of work and the routes to and from installation entry/exit points.
4.4.8. Traffic Laws. The contractor and its employees shall comply with base traffic regulations. Traffic regulations include, but are not limited to:
4.4.8.1. Employees shall wear a seatbelt when operating or riding in any military, contractor-owned, or privately-owned vehicle while on the installation.
4.4.8.2. Speed limits on Holloman AFB are 30 MPH unless otherwise posted. Be aware of reduced speed limits and exercise caution when driving in base housing and school zones.
4.4.8.3. All vehicle operators on the installation shall not use cell phones unless the vehicle is safely parked or unless they are using a hands-free device. The wearing of any other portable headphones, earphones or other listening devices (except for hand-free cellular phones) while operating a motor vehicle is prohibited.
4.4.9. Weapons, Firearms, and Ammunition. Employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within military, contractor or privately-owned vehicles while on Holloman AFB.
4.4.10. For Official Use Only (FOUO). The contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.
4.4.11. Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DOD personnel, contractor personnel, resources, and classified or unclassified defense information.
4.4.12. Physical Security. The contractor shall be responsible for safeguarding all Government property and controlled forms provided for contractor use. At the end of each work period, all Government facilities, equipment, and materials shall be secured.
4.4.13. Controlled/Restricted Areas. The contractor shall implement local base procedures for entry to AF controlled/restricted areas where contractor personnel will work.
4.4.13.1. Escorted Entry Procedures: Unless granted unescorted entry (see procedures below), all contractor employees will be escorted while in AF controlled/restricted areas on HAFB. When escorted, employees shall remain with their assigned escort at all times while in controlled/restricted areas. Escort responsibility rests with the sponsoring agency. An escort briefing will be provided to all escorted employees prior to entering a controlled/restricted area. Persons providing escort are responsible for providing this briefing. Escort officials assume responsibility for the safe and secure conduct of escorted employees and are required to maintain constant surveillance and control of employees at all times while in a controlled/restricted area.
4.4.13.2. Unescorted Entry Procedures: An AF Form 2586, Unescorted Entry Authorization Certificate, must be completed and signed by the sponsoring agency security manager before a restricted area badge will be issued. Employees must have a completed and favorable National Agency Check with written Inquiries (NACI) security investigation before receiving a restricted area badge. Interim access can be granted in accordance with AFI 31-501, Personnel Security Program Management. Unescorted entry will never be granted solely to avoid the inconvenience of escorted entry.
4.4.14. Photography. Photography in or within the close proximity of an AF controlled/restricted area, flightline, or taxiway is strictly PROHIBITED.
4.4.15. Cell Phones w/Digital Camera Capability. Cellular phones with integral digital cameras and other devices capable of capturing digital images are prohibited for use or possession, without written authorization, within any restricted or controlled area. The use or possession of hand held computers (Palm Pilots), electronic notebooks, or personal digital assistants (PDA) with cameras and/or voice recording capability and network camera surveillance equipment (NetCams, Webcams) are also prohibited, without written authorization, within any restricted or controlled area. The installation commander or his/her designated representative approves their use/possession in restricted and controlled areas.
4.5.
Contractor Personnel Requirements.
4.5.1.
Contract Manager. The contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of this person, and alternate(s), who shall act for the contractor when the contract manager is absent, shall be designated in writing to the C.O. five (5) working days prior to project start day. The contract manager or alternate shall have full authority to act for the contractor on all contract matter relating to the daily operation of this contract. The contract manager or alternate shall be available during normal duty hours within two (2) hours to meet on the installation with the designated personnel to discuss problem areas. After normal duty hours, the manager and alternate shall be available within two (2) hours. They must be able to read, write, speak, and understand English.
4.5.2.
Contractor Personnel. Contractor personnel shall present a neat appearance and be easily recognized as contractor employees. This shall be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contains the company and employee name in English. Contractor personnel shall use protective clothing whenever they are performing tasks that could cause harm to the employees or damage to property.
4.5.3.
The contractor shall not employ persons to work on this contract if such person is identified to the contractor by the government as a potential threat to the health, safety, security, general well being or operational mission of the installation and its population.
4.5.4.
Performance of Services during crisis declared by the National Command Authority. IAW DoDI 3020.37 (Continuation of Essential DoD Contractor Services During Crisis) and aforementioned Air Force implementation, and unless otherwise directed by an authorized government representative, it is determined that services as identified in this PWS are non-essential for performance during a crisis.
4.6.
Completion Time. The contractor shall be required to notify QAP when job is completed.
4.7.
Retreat. Retreat will be sounded, via the installation “giant voice” audio system, every Monday through Friday at 1630 hours to signal the official end of the duty day and to pay respect to the U.S. Flag. All vehicle operators shall come to a complete stop until the last note of retreat. Contractor personnel outside of a vehicle or facility shall face toward the direction of the sounding of retreat or towards the base flag pole (if visible) and remain silent until the last note of retreat is played.
