1. INTRODUCTION

1.1 Contractor shall provide all materials, equipment, labor and supervision necessary to repair interior finishes in Building 390 at March Air Reserve Base, California in accordance with the terms and conditions of the contract, contract statement of work, contract specifications, Facility Excellence Plan (FEP), federal, state and local codes, laws and regulations.

1.2 All quantities, dimensions and areas given within the statement of work are an approximation only. Contractor is responsible to verify all quantities, dimensions and areas.

1.3 The construction area is identified on drawing #1.

2. SCOPE OF WORK

2.1 Contractor shall install approximately 160 SF, ½” gypsum board, fire resistant, level 4 finish with one coat primer and two coats paint to match surrounding area in room 129.

The location is over 12 feet high and already has been framed with metal studs. Drawing #2

2.2 Contractor shall install six (6) separate 20 amp duplex electrical receptacles in room 129.

2.3 Contractor shall remove and replace approximately 700’ Linear Feet of vinyl cove base.

Install high impact PVC wall covering, “Acrovyn” or equivalent .060” thick, 48” high, approximately 3000 SF, with all associated hardware to include but not limited to divider bars, corner guards and top cap. Drawings #1, #3, #4, #5.

2.4 Contractor shall paint to a height of 8’ all areas above the Acrovyn or equivalent to match existing with level 4 satin finish, one coat primer, two coats paint. The art work above the desk will be protected and painted around. Drawings #1, #3, #4, #5.

3. GENERAL WORKSITE

3.1 Drawings provided are “For Reference Only” however, the contractor shall field verify, and submit a DD Form 1354, (Transfer and Acceptance of DoD Real Property) when project completed.

3.2 Contractor will dispose of and remove off base construction debris and materials.

Contactor shall prepare a recycling report for construction and recycled debris removed from March ARB during the course of this project. Report must include name and address of the receiving facility, tonnage and type of material disposed and recycled.

3.3 The contractor shall provide a signed Contract Progress Schedule, AF Form 3064, within ten (10) business days after contract award or at the pre-construction meeting. AF Form 3064 shall depict an overall contract progress schedule for the main elements of work for the period of performance for approval by the Contracting Officer. Once approved, the contract progress schedule may only be changed by the Contracting Officer.

3.4 The contractor shall provide a monthly signed Contract Progress Report, AF Form 3065.

The work elements and percentages for each element stated on the AF Form 3065 shall be identical to the work elements and percentages on the AF Form 3064.

3.5 The contractor shall prepare a pass information sheet identifying contractor personnel, including subcontractor personnel, who will be working on March ARB. The pass information sheet shall be submitted to the Contracting Office no later than the preconstruction meeting. Upon completion of the contract, or expiration of the passes/badges, the contractor must ensure all issued passes/badges are turned into the Contracting Office. Failure to comply with this requirement may result in withholding of final payment.

3.6 The prime contractor shall appoint a superintendent, in writing to the Contracting Office, with decision making authority prior to start of work. The superintendent will be the Governments point of contact at the construction site.

3.7 The contractor shall attend a Pre-construction meeting at the 452nd Operational Contracting Office located at 1940 Graeber Street Bldg. 449, March ARB, Ca 92518.

3.8 The contractor shall not order any equipment, materials or begin any phase of demolition or construction until the Contracting Officer issues a Notice to Proceed (NTP).

3.9 Pre-Final Inspection: The contractor shall request a pre-final inspection with the contracting officer representative (COR). Pre-final inspection shall be requested with sufficient time to allow punch list corrections, if any, prior to contract completion date.

3.10 Final Inspection: The contractor shall request a final inspection, in writing to the Contracting Officer, upon correction of pre-final punch list items. The final inspection will be formally scheduled by the Contracting Officer upon confirming that pre-final punch list items have been corrected. At a minimum, contractor Superintendent, Contracting Officer and COR will be in attendance.

3.11 The contractor shall obtain permits for work associated with hot work, welding, cutting, and brazing from the March ARB Fire Department prior to performing any such work associated with the permit. The Contractor shall allow seven (7) calendar days processing time for these permits. The Contractor is responsible for obtaining all necessary permits from all applicable federal, state and local agencies and is responsible for all associated fees.

3.12 The contractor shall submit, to the Contracting Office, material submittals as identified

Page3 of Page 4 in the Schedule of Material Submittals and as required by applicable specifications.

Submittals shall be submitted by the required submission date as identified on the Schedule of Materials Submittals. Submittal packages shall be provided to the Contracting Office accompanied with an AF IMT 3000-Material Approval Submittal. A copy of the approved/disapproved signed AF IMT 3000 will be provided to the contractor upon completion of government review.

3.13 The Contracting Officer shall have the sole authority and shall be responsible for managing, addressing and making any changes to any and all contractual and administrative issues relating to this task order. The contractor will not proceed with any changes to the task order until receiving notification from the Contracting Officer.

3.14 The 452 MSG/CE Construction Representative will provide technical support, project coordination, and design guidance as required for the project. The Construction Representative will be responsible for informing the Contracting Officer of any change(s) to the schedule due to delay, readjustments of priorities, or extensions requested by the U.S. Government. The contractor is reminded that the Construction Representative is not authorized to direct any changes to the contract.

3.15 The contractor is responsible for completion and submittal of March Form 103 “Base Civil Engineering (BCE) Work Clearance Request” to the BCE prior to beginning any work performance. The contractor shall allow a minimum of fourteen (14) calendar days for processing. The March Form 103 will be valid for the duration of the project.

3.16 The contractor shall be responsible for completion of the Air Field Driver’s Training in order to drive on the Air Field.

4. PERFORMANCE

4.1. The performance period is 90 calendar days from Notice to Proceed (NTP). The contractor shall begin work as described in the scope of work within 10 days of receiving NTP. All work, including any punch list items, must be completed no later than 90 calendar days after receipt of an NTP.
