TECHNICAL PROVISIONS

SECTION 01 11 00

GENERAL

1. SCOPE: The work covered by this project consists of the Contractor furnishing all layout, survey, plant, labor, supervision, quality control, materials, equipment, machines, tools, appliances, services, supplies, and incidentals and of performing all operations in connection with the Repair of Critical Water Mains, Dyess AFB, Texas, complete and in strict accordance with the plans and specifications."

2. WORKING CONDITIONS:

2.1 PHASING REQUIREMENTS: The work of this project (“Repair Critical Water Mains”) is to be performed in six (6) areas as indicated on the drawings. The six areas are designated as: Area 1A, Area 8, Area 9, Area 10, Area 11 and Area 12.

The work is to be done by area in numerical order, with the exception that the work in Area 10 is to be done after the work in Area 8 and prior to the work in Area 9.

In order to minimize interruptions to service and to maintain the best possible system reliability during the water main replacements, a sequence generally as follows shall be utilized for each area:

1. New water main pipe segment is installed (generally paralleling, or in proximity to the existing main).

2. The new main segment is connected to a water “source” at each end.

3. Individual water services, or laterals are disconnected from the existing main and re-connected / re-served from the new main.

4. After all services, etc. are disconnected from the existing main, the existing main piping is excavated and removed, or capped and abandoned as applicable.

5. The above sequence is repeated for remaining water main segments until all replacements for a given area have been made.

Refer to the “CONSTRUCTION SEQUENCE NOTES” provided in the project drawings for additional information and requirements.

Prior to any cutover of any service(s), the Contractor shall perform all possible preliminary work to limit the service interruption to the minimum time possible as specified in Section 01 11 10, Par. 3. Similarly, all possible preliminary work shall be performed prior to the cutting/capping of existing services at their connection points to existing mains or branch lines.

Work by 7CES/CEOI personnel will be required in conjunction with the line replacements and cutovers. Due to 7CES/CEOI manpower limitations, the Contractor shall be limited to not more than two (2) service cutovers per workday. Additional service cutovers will be considered, contingent upon 7CES/CEOI having available manpower. Requests for consideration of additional cutovers on a given day shall be included in the Contractor’s “Work Plan”. Any

Section 01 11 00 Page 1 requests for additional cutovers, etc., on a given day after the block Work Plan has been approved shall be made in writing to the Contracting Officer not less than five (5) working days prior to the requested date.

2.2 WORK AREA AVAILABILITY:

The work in each of the six (6) areas will typically be broken down into “sub-areas”. Said “sub-areas” will be defined by the segment of main piping which is being replaced. Generally, the work area available to the contractor at any given time will be limited to a “sub-area”. The work in all “sub-areas” within an “area” shall be completed before work is initiated in the next “area”.

2.3 JOINT OCCUPANCY:

Generally, all streets are to be kept open, access to all buildings is to be maintained and access to all open areas is to be maintained. The contractor may barricade specific “open areas” where work is being performed.

2.4 SCHEDULING: The Contractor shall submit a work plan. (Refer to “Required Work Plans” paragraph appearing hereinafter.) Included in each plan shall be the planned date for each line or service cutover and the planned date for the de-activation and capping of each existing line or service.

2.5 DOWN TIME: Refer to Par. 3.1.

2.6 AIRCRAFT TRAFFIC OR OTHER INTERRUPTIONS: Refer to Section 01 14 20 DYESS

2.7 REQUIRED NOTICE REQUIREMENTS:

2.8 REQUIREMENTS FOR CONTRACTOR EMPLOYEES TO ENTER SECURED WORK AREAS:

Refer to Par. 13 SECURITY REQUIREMENTS of (this) SECTION 01 11 00 GENERAL.

2.9 TRAFFIC CONTROL:

Refer to Par. 5 STREET CLOSINGS of (this) SECTION 01 11 00 GENERAL.

2.10 REQUIRED WORK PLANS:

Not less than three (3) weeks prior to beginning work the Contractor shall submit a work plan for the work to the Contracting Officer for approval. As a minimum, this plan shall include:

• Planned dates for each main installation and activation.

• Planned dates for cutovers/tie-ins. This shall include a listing of all facilities which will have service interrupted and a listing of all facilities to be cutover/tied in.

• Planned dates for the capping of each existing abandoned main.

• Identification of Controlled Access and Restricted Access areas where work is to be performed.

• Any required barricading.

• Any anticipated “long down time” cutovers. The maximum allowable water service interruption for a building shall be eight (8) hours. If the Contractor anticipates that a longer interruption will be needed for a given building(s), he shall identify the building(s) and the anticipated duration of the interruptions.

• For each facility which will have its water service interrupted, the contractor shall provide a temporary (portable) washing station and port-a-potties. These items shall be in place prior to the service interruption and shall be removed after service is restored. The number of lavatories and toilets provided shall be as appropriate for the number of people occupying the facility and the duration of the outage.

• Any other anticipated problem areas and proposed solutions.

3. WORK SCHEDULE:

Working hours for the Contractor will normally be between the hours of 7:30 a.m. and 4:30 p.m. excluding Saturdays, Sundays, and Federal holidays. If the Contractor desires to work during periods other than above, additional government inspection forces may be required. The Contractor must make his/her request to the Contracting Officer five (5) calendar days in advance of his/her intention to work during other periods to allow assignment of additional inspection forces. If such force is reasonably available, the Contracting

Officer may authorize the Contractor to perform work during periods other than normal duty hours/days.

3.1 DOWN TIME: Down time shall be defined as delays in excess of one hour at a given period, caused by the Government through no fault of the Contractor.

a. MEASUREMENT:

(1) Down time will be measured by the unit time-of-the-clock hour which will include all Contractor and sub-Contractor (at any tier) work forces scheduled for, or working during the hour.

(2) No delays less than one hour in duration at any given period will be measured nor considered for down time, nor will delays of less than one hour be cumulative over several time periods.

(3) Down time will not be measured/considered for delays before or after contract work schedule hours as defined in the contract General Provisions.

(4) No time after 10 minutes from the time that Contractor is notified that he can resume work after a stoppage will be considered down time.

Contractor shall comply with all security requirements under paragraph 1A-13 of this specification section to qualify for down time compensation.

b. PAYMENT: Payment for down time will be made at the contract unit price for down time as shown on the bid schedule in accordance with the Contact Clause entitled Payments Under Fixed Price Construction Contracts.

4. SAFETY AND HEALTH:

4.1 All Contractor operations shall be conducted and performed in accordance with Department of Labor, OSHA requirements found in 29 CFR 1910 and 29 CFR 1926, project identified national standards, military manuals, instructions, pamphlets, standards and handbooks, and with the Corps of Engineers (COE) Safety Manual 385-1-1 dated 03 Sep 96.

4.2 All companies who conduct business within the state of Texas must, in accordance with Texas Workman Compensation laws (Texas House Bill 62), have an approved company safety policy and an Accident Prevention Plan. The plan, approved by the Texas Workman Compensation Commission (TWCC), shall be submitted For Information Only (FIO) in accordance with paragraph SUBMITTAL REQUIREMENTS. In addition to meeting the TWCC requirements; the plan must also include the requirements of COE Safety Manual 385-1-1.

4.3 All holes/pits/trenches/manway openings, etc, that are to be left open shall be surrounded with a 48 inch high mesh fence with highly visible orange plastic coating. The fence shall be securely anchored with tension wires and posts as required to prevent sagging and located a minimum of 3 feet from the opening so as to prevent an individual, should they fall across the fencing, from falling into the opening. Holes shall also be covered, when not being worked in, with three quarter inch plywood or a metal grating that will prevent small children from entering the hole.

4.4 Radiation Permits and Authorizations: Contractors contemplating the use of devices containing radioactive materials (i.e., soil moisture/density probes) or non-ionizing radiation producing equipment (radio frequency radiation emitters or lasers) while performing work on this contract must obtain written authorization/permit from the Dyess AFB Radiation Safety Officer (RSO), 7 ADOS/SGGB. To obtain the required authorization permit, an application to bring a radioactive device on Dyess AFB must be forwarded to 7 ADOS/SGGB, 697 Hospital Road, Dyess AFB, TX 79607-1367 (this is mailing address, physical address is 880 Third St.) or (915)696-2325/3289, at least 45 days prior to the anticipated use. Without the proper authorization, Contractors will not be allowed to bring these devices on base. Also, Contractor shall provide Bioenvironmental Engineering (7 ADOS/SGGB) a copy of contractor’s radioactive materials permit and training certificates, if radioactive material will be brought onto Dyess AFB.

4.5 Confined Space Entry: All operations involving entry into confined spaces shall meet the requirements of OSHA 29 CFR 1910.146, AFOSH Std 91-25, and specifically the following:

a. All entry supervisors, attendees, and confined space entrants shall have been properly trained in the safety hazards, proper use of Personal Protective Equipment (PPE), entry procedures, and self-rescue. Records of this training must be readily available.

b. Entry supervisors shall maintain a Master Entry Plan (MEP) consisting of:

(1) Descriptions of confined spaces to be entered including location, classification, and acceptable entry conditions

(2) Designation of authorized entry supervisors, entrants, and attendees

(3) Identification of the types of tasks to be performed in the confined space, including duration

(4) Procedures for entry, and emergency rescue

(5) Identification of Personal Protective Equipment (PPE), communication equipment, rescue equipment, and monitoring equipment; conditions under which they will be used; and verification of condition of equipment

(6) Designation of frequency and type of atmospheric monitoring

(7) Designation of controls required (e.g., lockout/tagout, ventilation, etc.)

(8) Procedures for communication during confined space operations

4.6 Accident Prevention: Refer to FAR 52.236-13 Accident Prevention.

4.7 Airfield Safety Precautions: Refer to DFARS 252.236-7005.

5. STREET CLOSINGS:

5.1 One lane of traffic shall be maintained at all times unless otherwise approved in writing by the Contracting Officer.

5.2 Contractor shall provide 14-day advance written notice to Contracting Officer prior to any street closures to insure that Fire Department and Emergency personnel are notified.

5.3 The final street repair shall be completed within 14 days after the start of any street demolition for utility crossings or other purposes. Any part of

Section 01 11 00 Page 5 the street returned to service prior to final repair shall be maintained smooth with hot-mix cold-lay surface course.

6. DISPOSITION OF WASTE, EXCESS, SCRAP, AND SALVAGE MATERIALS:

The title to all waste, excess, scrap, and salvage materials generated as a direct result of this contract is vested in the Contractor - unless specifically excepted below. Further (unless listed below in exceptions), these materials shall be disposed of off the Base by the Contractor in accordance with all applicable Federal, State, and Local Regulations. Following is a list of materials (Exceptions) that will be generated as a result of this contract and shall be turned-in to the Government at a location at Dyess AFB designated by the Contracting Officer:

Exceptions: None.

7. STORAGE AREA (I.E. TEMPORARY FIELD OFFICE, STAGING AREAS, TOOL/JOB

SHACKS, AND OTHER CONSTRUCTION FACILITIES):

There are no Government furnished covered or secure storage areas. Limited on-base, offsite storage will be permitted on a space available basis. The location on Dyess AFB of the Contractor’s temporary field office, storage, and other construction buildings required temporarily in the performance of the work, shall require written approval of the Contracting Officer. Plans showing temporary field office, storage, and other construction buildings shall be submitted for approval (GA) of the Contracting Officer. Utilities at the storage area may or may not be available for Contractor use. The Government implies no responsibility for lost or stolen materials, equipment or tools, the security of which lies solely with the Contractor. Contractor shall keep his storage areas clean, neat and orderly and will keep grass mowed to a maximum height of 4 inches. Temporary fencing used by the Contractor to delineate constructor sites shall be securely anchored with tension wires and posts as required to prevent sagging and an unsightly appearance. Fencing shall be maintained by the Contractor in this manner throughout the life of the contract. Due to high winds in west Texas, Contractor shall take every precaution to preclude trash from blowing off site.

8. TOILET FACILITIES: There are no toilet facilities available for Contractor use. Contractor shall provide his own portable/temporary toilet facilities.

9. CLEAN-UP: The Contractor shall at all times keep the construction site and storage area(s) free from accumulation of waste, rubbish, or construction debris. All loose or lightweight materials shall be secured to prevent blowing or scattering. The burning of trash or construction debris is strictly prohibited on Dyess AFB. Prior to final inspection, the Contractor shall remove all construction debris, tools, equipment, and materials not the property of the Government. Upon completion of the work, the Contractor shall leave the work site and storage area(s) in a clean, neat and workmanlike condition satisfactory to the Contracting Officer.

10. FINAL INSPECTION:

The Contractor shall advise the Contracting Officer of the Contractor's desired final inspection date seven (7) calendar days in advance of that desired date to permit proper coordination. The date selected shall provide adequate time for Contractor performed corrections of final inspection deficiencies within

Section 01 11 00 Page 6 the contract performance time. The Contracting Officer will be the final authority for determining whether or not the Contractor's performance is sufficiently advanced to warrant a final inspection.

11. TESTING:

Costs of all tests, unless specifically indicated as being performed by the Government, will be at the Contractor's expense. It is the responsibility of the Contractor to schedule all tests and to notify the Contracting Officer or his representative in a timely manner prior to any required testing. All test results shall be submitted to the Contracting Officer on AF Form 3000, Material Approval Submittal.

11.1 Fire hydrant flow tests: Provide flow tests for all new fire hydrants which are installed under this project. Refer to SECTION 35 11 00 for testing requirements.

12. AS-BUILT DRAWINGS, INSTRUCTION MANUALS, AND TRAINING:

As-Built Drawings: As-Built Drawings shall be provided in accordance with Section 01 71 00 AS-BUILT DRAWINGS.

13. SECURITY REQUIREMENTS:

Dyess AFB security may be viewed as being one of three broad levels, the level in effect being dependent on the location or AREA of Dyess AFB in question.

a. The lowest level of security exists in GENERAL ACCESS AREAS. These AREAS are all of Dyess AFB outside of the "USAF Controlled" and "USAF Restricted" AREAS.

b. USAF controlled access AREAS are the mid-level security AREAS. Presence within the CONTROLLED ACCESS AREA is by authorization; however, the movement of authorized personnel in and out of these AREAS is generally not impeded.

Contractor personnel are given access to these AREAS.

c. The third and highest level of security on Dyess AFB is that within the USAF RESTRICTED AREAS. These AREAS are further subdivided to be known as Protection Level 1, 2 or 3 AREAS.

13.1 Referencing the above, the work on this contract will be in General Access, USAF Controlled Access and USAF Restricted areas.

13.2 Requests for changes to the work schedule for work in USAF restricted areas, must be submitted a minimum of 7 calendar days in advance for approval and coordination of Dyess AFB Security Forces.

13.3 Dyess Air Force Base is a closed base. All personnel entering must have specific permission of the installation commander for entry. This permission is granted when a Contractor employee is issued an identification card.

Information necessary to obtain identification cards will be provided by the Contracting Officer at the pre-performance conference. The Contractor shall be responsible to ensure that all of its employees obtain, and keep on their person at all times while working on the base, a USAF identification card.

Upon completion of the work or termination of an employee, the Contractor shall be responsible for turn in of identification cards no longer needed to the Contracting Officer or to the Pass and Registration Office of the Security

Forces. Failure to turn in badges will result in last payment of payroll to be withheld and revocation of any future badges issued by Pass and Registration Office to the employee and the contractor.

DYESS FORM 173:

Effective 1 Nov 04, a completed signed original Dyess Form 173 (Mar 04) must be delivered to Pass & ID by 0900 on the duty day prior to the desired day of entry onto Dyess AFB. The Visitor Control Center will no longer issue temporary passes to contractor personnel unless they have already been previously identified on an executed Dyess Form 173.

All information on the Dyess Form 173 must be typed and the authorizing signature must be in blue or black ink.

Personal information provided on the Dyess Form 173 will be cross-referenced against state and federal criminal databases to identify those individuals possessing outstanding warrants. Those individuals with outstanding warrants will be detained at the Visitor Control Center on the desired day of entry, and turned over to Abilene Police Department.

If any errors are noted on the form, the approving authority (individual on the 577) can make ink changes in red ink. All changes have to be initialed by the approval authority. (Effective Date: Immediately)

All individuals with Dyess Form 173’s will still continue to be processed via the Texas Law Enforcement Terminal System (TLETS—police wants/warrants check).

Once it is determined the individual is cleared from any wants or warrants, a copy of the Dyess Form 173 will be placed at the Tye Gate entry point. The Tye Gate Guard will use this form as an Entry Authority List (EAL) allowing contractors onto the installation. The contractor will then be able to go to the Gate directly (not the Visitor Control Center (VCC) or Arnold Gate) and verify if their Dyess Form 173 is there. Should their Dyess Form 173 not be there, they will be directed to the VCC for issuance of an AF Form 75. At this time someone will physically have to meet them there to sponsor them onto the installation. (Reasons why the form would not be at the guard shack would be erroneous driver’s license number, erroneous information, warrant, or identified as illegal alien). (Effective Date: Immediately)

Should their Dyess Form 173 be at the Tye Gate entry point, the contractor will be told to proceed directly to the Pass and Registration Section for the 7 SFS Contractor Badge issue. The Pass Section will have the original Dyess Form 173. Once a badge is obtained, they need only show the badge at the installation gate for entry onto the installation. Please note that neither the Security Forces Control Center nor the Tye Gate Guards will check the Dyess Form 173 for errors. This is the responsibility of the Pass and Registration Section. (Effective Date: Immediately) The Dyess Form 173 will be maintained at the Tye Gate entry point for 48 hours (two business days). After the specified time has expired, it will be destroyed. Should the contractor not process through Tye Gate in that 48 hour period, the contractor will require someone to physically vouch them on base to get a one day pass at the VCC so they may proceed to the Pass and Registration office for their badge. The hold period for these forms at the gate can and will be extended should rain days occur or through prior coordination with any affected agency. (Effective Date: Immediately)

TRUSTED AGENT LETTER FOR DAY LABOR REQUESTS:

Purpose of Letter: To provide sponsoring authority to specific contractor individuals as identified by their sponsoring agency (7 CONS, Army Corp of Engineers, etc) to sponsor day laborers on for one day periods. (Effective Date: Immediately)

The Letter Process:

The sponsoring agency, in coordination with their contractor, will identify contractor individuals that require sponsoring/vouching authority. This authority is given to the contractor for the purpose of sponsoring day laborers/job site personnel sub-contracted as a result of an unexpected or new requirement not previously identified in the current contract. This authority will not be given to contractors for convenience only, but should be limited to the smallest group of manager or foreman contractor personnel needed to complete their assigned contract. It is the responsibility of the sponsoring agency to ensure the Trusted Agent Letter remains current. Additionally, all reasonable actions should occur to prevent use of this process when a known laborer/sub-contractor was required well in advance of the day labor/sub-contractor request. (Effective Date: Immediately)

The sponsoring agency will provide this letter to the Pass and Registration Section who will maintain it on file. A copy of the letter will be posted by the Pass and Registration Section at the Visitor Control Center as a reference to verify which contractor personnel have sponsoring/vouching authority.

The Process to get Day Labors/Job Site Personnel Sub-Contracted onto the Installation:

The contractor individuals identified in the Trusted Agent Letter will provide a typed or hand-written form (form will be provided by the 7 SFS) with all individuals they will sponsor onto the installation to the VCC Guard. The VCC Guard will verify the identity of all individuals on the form by checking the information against a valid State/Federal Identification card. The VCC Guard will randomly check a percentage of the individuals via the TLETS. Should any individual not possess a valid identification card, they will be turned away at the VCC and entry to the installation denied. Pen and ink changes can be made to the form, but all information must be validated by the guard before the contractor will be allowed to proceed onto the installation with the sponsored individuals. No AF Form 75/Visitor Pass will be issued to any individual who is sponsored by the Trusted Agent Contractor, unless the sponsored individual will be driving a vehicle onto the installation. Trusted Agent Contractors are encouraged to consolidate all sponsored individuals into the smallest number of vehicles as possible to avoid the issuance of multiple AF Forms 75. Should an AF Form 75 be issued, the driver will be required to produce a valid driver’s license, current insurance card, and current registration for the vehicle being operated. Once on the installation, the Trusted Agent Contractor will be responsible for the actions of the sponsored individuals. Should an incident occur with a sponsored guest, and negligence be proven on the part of the Trusted Agent Contractor, their vouching/sponsoring authority will be revoked. The Trusted Agent Letter can be adjusted to add and delete names of Trusted Agent Contractors as needed, as long as the provisions of this paragraph are followed. Although the 7 SFS form is used in lieu of the AF Form 75, it carries the same authority and responsibilities. Additionally, the 7 SFS Form will expire at 0001 daily or

Section 01 11 00 Page 9 at the end of the contractor duty day, whichever occurs first. At no time will the Trusted Agent Contractor depart the installation and leave any of his/her sponsored individuals with another contractor that does not have the same vouching/sponsoring privileges. (Effective Date: 15 Dec 04)

All contractors must attend a contractor briefing given to the contractors by Pass and Registration who will provide a more detailed explanation regarding contractor badges and their uses. Pass and Registration reserves the right to not issue a contractor badge due to erroneous information or failure to disclose information on the Dyess Form 173. Any derogatory information received from the background check will result in the non-issuance of the contractor badge.

The Dyess AFB Form Letter 173 is subject to the privacy act of 1974, Authority:

10 U.S.C. 8012 and 5 U.S.C. 552a(b)(7).

Principle purposes: To provide the 7th Security Forces Squadron, and other authorized agencies, with a listing of personnel authorized access to Dyess Air Force Base, Texas as Contractors or Contractor employees.

Routine Uses: To verify the identity of personnel entering Dyess AFB as Contractors.

Disclosure is Mandatory: Failure to provide the information and Social Security Number (SSN) could result in denied access to the Dyess AFB installation.

13.4 Restricted Areas and Flightline Controlled Areas: Contractor shall be required to enclose his contract work area, with airport barriers similar to model MS4202-5, “safety orange” color, manufactured by MARKSTAAR or approved equal. Supplement the barriers with red lights, either flashing or steady-burning. Red lights shall meet the luminance requirements of the Texas State Highway Department. The barriers shall be spaced 50 feet apart or less. The intent of this requirement is to comply with FAA Advisory Circular 150/5370- 2E “Operational Safety on Airports During Construction” and that all personnel (Contractors', Air Force, and visitors) will know the authorized boundary of each Contractor's area and thereby minimize unauthorized movement, and to help promote safety.

14. IDENTIFICATION OF CONTRACTOR VEHICLES: Contractor vehicles must be marked on each side with company name with either permanent or semi-permanent/magnetic signage.

15. WARRANTIES:

Any warranties given to the Contractor or sub-Contractor at any tier from a manufacturer of equipment or other items which are provided under this contract shall be transferred to the government upon final acceptance. Contractor shall submit in writing a single listing with all applicable warranties attached.

Negative responses are likewise required in writing.

16. SUBMITTALS:

16.1 Required Submittals:

All submittals required by the Contract Documents shall be submitted for approval; and as a minimum, all submittals listed/scheduled on the AF Form 66 shall be submitted to Contracting Officer for approval using AF Form 3000 as a transmittal document. Where a submittal cannot be provided within the required

Section 01 11 00 Page 10 submission date, Contractor shall submit in writing a letter stating the reasons why and furnishing a new projected submission date.

16.2 Variations/Deviations/Departures from the Contract Documents:

Contractor proposed variations, deviations or departures from the contract requirements shall be noted/marked in red on each copy of the submittal data and shall be provided with a letter attachment to the AF Form 3000 summarizing the proposed variation, deviation, or departure. Variations, deviations, or departures shall contain sufficient information to permit complete evaluation.

Additional sheets may be used to fully explain why a variation, deviation, or departure is requested. The Government reserves the right to disapprove or rescind inadvertent approval of submittals containing unnoted/unmarked/not-clearly-defined variations, deviations or departures.

SECTION 01 11 10

UTILITIES (CONTRACTOR IDENTIFIED)

1. SCOPE: This section covers identification, interruption and use of utilities.

2. IDENTIFICATION: THE GOVERNMENT DOES NOT KNOW THE LOCATION OF UTILITIES IN

THE WORK AREA. Accordingly, the Contractor shall be SOLELY RESPONSIBLE for locating and marking the exact location of all existing utilities within the contract work area prior to any excavating, trenching, backfilling or disturbance.

The Contractor is SOLELY RESPONSIBLE for any and all damage to existing utilities in the contract work area. Upon request by the Contractor, the Government shall furnish all available information in its possession concerning utilities in the contract work area. However, the accuracy of the information provided by the Government is not guaranteed and is only intended to provide some measure of assistance to the Contractor. The Contractor shall call 7 CS/SCMO (Mr. Lowe) at 696-4391 a minimum of five (5) calendar days in advance to have underground communications cable routes marked. Base Civil Engineering does not have nor shall it provide record drawings of Bell Telephone cable plant. In the event the Contractor identifies utilities in the contract work area which interfere with the newly proposed construction, the Contracting Officer shall be immediately notified and the Government shall take necessary corrective action at no cost to the Contractor. The Contractor shall furnish to the Contracting Officer as-built drawings clearly identifying the exact location of all utilities identified in the work area prior to project final inspection.

2.1 Contractor must initiate a Work Clearance Request AF Form 103 (digging permit) from 7 CES/ CECC a minimum of seven (7) calendar days prior to the start of any construction work. Excavation is not authorized without issuance of a completed and approved AF Form 103. After initial issue, it is the Contractor's responsibility to keep the Work Clearance Request coordinated and up-to-date/current through the remainder of the contract.

2.2 Any removal/relocation/reconnection of any communication device shall be coordinated in advance with 7 CS/SCMO (Mr. Lowe) at 696-4391. Any removal/relocation/reconnection of any Cable TV device shall be also be coordinated in advance with 7 CS/SCMO (ask for the NCOIC of Cable TV) at 696- 2400. Communications and Cable TV devices to remain shall be protected as required when work proximity dictates.

2.3 Any removal/relocation/reconnection of any fire protection device or intrusion detection systems shall be coordinated in advance with 7 CES/CEOIE at 696-5184.

2.4 The locating and marking devices utilized by the contractor for existing utilities or proposed work shall be 2” wooden stakes with multicolored whiskers 4” to 6” in length.

3. INTERRUPTIONS:

3.1 Planned Utility Outages: The Contractor shall coordinate all requests for utility outages with the Contracting Officer in writing fourteen (14) calendar days prior to date of requested outage. Water, gas, steam, sewer and electrical outages shall be held to a maximum duration of 2 hours unless otherwise approved in writing. See Contractor Request for Utility Outage included on page 1B-3.

3.2 Unplanned Utility Outages (Accidental Disruption of Utilities): In the event of accidental disruption of any utility, the Contractor shall immediately notify the Contracting officer of the unplanned outage. The Contractor shall immediately take every reasonable step to repair the damage in a manner acceptable to the Government and will restore the utility to full use as soon as practicable. If the Contractor so desires, and the Government agrees, the Government may complete necessary repairs to the damaged utility and withhold from payments due to the Contractor the necessary amount to defray all costs associated with the repair of the utility.

4. USE: All reasonable quantities of existing utilities will be made available to the Contractor without charge. Any temporary connections or lines required shall be installed, maintained, and removed at the Contractor's expense. Any damage associated with the use of these utilities shall be repaired and/or replaced in a manner satisfactory to the Contracting Officer at Contractor's expense. See Contractor Request for Use of Dyess AFB Fire Hydrants included on page 1B-4.

a. The company providing a suitable connection with a Class III Back Flow Preventer (reduced pressure principle device) and screw type globe valve to be attached to the hydrant. The connection will be 2 1/2" National Standard fire thread. The backflow device and valve shall be properly supported to prevent damage to fire hydrant threads.

b. Leaving the connection in place during approval period.

c. Insuring the hydrant is fully opened and left in that position during approval period, except in periods of freezing weather.

d. Insuring an approved fire hydrant wrench is used to open/close the hydrant.

e. Insuring all servicing from the hydrant is done at the top of the vehicle or tank. No bottom servicing will be permitted.

f. Using no quick opening valves causing excess water hammer in the main.

g. Discontinuing hydrant use if there is any hydrant malfunction or leakage from underground and reporting same to the fire department, 696-2486.

SECTION 01 14 20

DYESS AFB FLIGHTLINE RULES

1. SAFETY PLAN: Contractor shall prepare a safety plan for all airfield/flightline work and submit such plan to Contracting Officer for approval. Work cannot commence until this Safety Plan has been approved by the Contracting Officer. This plan shall schedule all work in all airfield/flightline areas pursuant to these working conditions. This Safety Plan shall be kept current. This safety plan shall include as a minimum an airfield safety training schedule for employees, construction vehicles hauling routes, airfield driving training schedule, a list of suitable hazard markings/lights/beacons/flags for vehicles to be used during construction, construction area barricade layout, and the steps the contractor will follow every day to keep his employees, the working area and its vicinity in compliance with all local, state and federal safety requirements while performing work under this contract.

1.1. FOD MITIGATION PLAN: Contractor will prepare a comprehensive foreign object debris (FOD) mitigation plan. The plan shall show the contractor’s FOD mitigation area of responsibility and address, as a minimum, a FOD sweep schedule, a FOD check station when leaving work area (free zone), an approved “free zone”, and a driving lane or contractor’s route.

2. COORDINATION:

2.1 Contracting Officer: Contractor shall coordinate with the Contracting Officer more than 7 calendar days in advance of performing any flightline work at Dyess AFB.

2.2 Airfield Manager: Contractor shall check in with Base Operations at Dyess AFB at 674 Alert Avenue prior to the start of each work day for aircraft traffic advisories and any unusual working conditions.

3. SCHEDULED MILITARY OPERATIONS: No airfield/flightline work areas may be available for contract accomplishment during 14 calendar days in 7 calendar day increments during an individual task order performance period due to scheduled military operations. In these cases of non-availability of work areas due to scheduled military operations, the Contractor will receive notice from the Government not less than 72 hours in advance of such non-availability for weekdays work or by close of business Wednesday for weekend work. The Contractor shall not be eligible for additional compensation when a work area is not available for each of these two 7 calendar day periods of non-availability.

4. ADDITIONAL CRITERIA: Contractor shall comply with the following additional criteria:

4.1 Storage areas and areas for access to and from the work area will be designated for the Contractor and Contractor’s vehicles. Materials and equipment are not permitted on other areas of the airfield without coordination with the Airfield Manager and approval of the Contracting Officer.

4.2 The Contractor shall remove all equipment and objectionable matter from the work site upon completion of each day’s work and have the work area in a safe, clean, and orderly manner to the satisfaction of the Airfield Manager and the Contracting Officer. FOD will be monitored throughout the workday to

Section 01 14 20 Page 1 ensure the work area and its surroundings are kept clear to prevent migration of FOD by vehicles and/or taxiing aircraft.

4.3 The Contractor shall fully cooperate with operations and security personnel. The Contractor shall not enter or be within 100-feet of the active runway unless equipped with two ways radio(s) and authorized by the Air Traffic Control Tower. Contractor shall provide the radio(s) in sufficient quantity for control of his runway operations. After approval is given by the base frequency manager for the Contractor to use the frequency, the Contractor will be provided the ramp net frequency. Additionally, the Contractor’s jobsite superintendent shall continuously monitor this ramp net frequency by radio while working anywhere on the airfield – to receive aircraft advisories and/or coordinate work activities.

4.4 The Contractor shall give right-of-way to aircraft at all times.

4.5 Fire trucks and emergency equipment shall have ingress and egress rights to all areas at all times.

4.6 All equipment used on the runway or within 100-feet of the near edge of the runway shall be manned and operational at all times.

4.7 The Contractor and his personnel shall comply with current Air Force, FAA, and Dyess AFB, safety procedures, rules and regulations reference throughout these specifications and with the base temporary airfield waiver (waivers are reviewed/revised/approved every year). Temporary construction vehicles and equipment in excess of 10 feet in height operating on the airfield shall have suitable hazard markings/lights/beacons, in accordance with FAA Advisory Circular 70/7460-1, “Obstruction Marking and Lighting”, Chapters 3, 4, 5, and

12. Construction vehicles/equipment over 20 feet in height are not covered under the current approved construction waiver and FAA aeronautical study (No.

2008-ASW-6172-OE or must current); if such vehicles are required for a construction project, contractor shall request authorization through the Contracting Officer at least 45 calendar days prior to the date the equipment will be needed to allow the government ample time to process the request.

Also, hearing protection is required for Contractor’s personnel while working on the airfield due to the excessive sound levels generated by aircraft.

Restrict movement of construction vehicles by flagging and barricading, erecting temporary fencing or providing escorts as appropriate. Ensure no employees, subcontractor or suppliers enter any part of the aircraft operation areas from the construction site unless authorized.

4.8 No work shall commence on any area until all required equipment, personnel, materials, tools, and supplies are on hand and the Contractor is prepared to accomplish the specified work or task.

4.9 Upon clearance notification, the Contractor shall clear the active runway, north and south accesses, and taxiway areas within 100-feet of the active runway of his personnel, supplies, materials, debris, etc. within 5-minutes after being notified by the Air Traffic Control Tower or the Airfield Manager to clear the runway. The contractor shall immediately notify the Contracting Officer and the government inspector after clearing the work area to allow proper documentation of the action taken in the event construction performance is compromised and/or if contractor would seek compensation under the downtime contract line item.

4.10 The Contractor shall be responsible for signage, signaling, lighting, and barricades in the immediate work area where applicable and conforming to the provisions of the UFC 3-260-01”Airfiel and Heliport Planning and Design”, section 14 “Construction Phasing Plan and Operational Safety on Airfields

During Construction” and FAA Advisory Circular 150/5370-2E “Operational Safety of Airports During Construction” .

4.11 For the protection of Government personnel and property, the Contractor shall comply with these minimum safety requirements while performing work under this contract. These requirements are additional to and do not replace the standards promulgated by the Department of Labor under the Occupational Safety and Health Act (OSHA). In the event of a conflict between the OSHA Standards and these requirements, the most stringent shall apply.

4.12 All vehicle operators, before driving on the flight line, shall receive special instructions on standard flight line traffic controls/signals/speed limits, be advised of the particular hazards involved and obtain an AF Form 483, Certificate of Competency card and DAFB Form 301 POV pass, if required.

Contracting Officer will coordinate with Airfield Management for driver’s training. All training will be completed as soon as possible within five working days from the request. Airfield Management will issue the AF Form 483 and DAFB Form 301. Under no circumstances will an individual who did not receive the training operate a vehicle on the flight line or in the airfield environment. The minimum number of vehicles required to complete the contract shall be used while operating on the airfield. The primary contractor is responsible for all subcontractors used to complete a project.

4.13 Vehicles shall not be driven within 50-feet of an aircraft or under any part of an aircraft. Vehicles shall not be driven within 50-feet of aircraft fueling or defueling operations. When parked on the flight line, vehicles and all equipment mounted on wheels, which do not have integral braking systems, shall be chocked when left unattended. Vehicles left unattended and parked on the flight line shall be left unlocked with keys in the ignition system.

4.14 Under no circumstances shall vehicles be allowed to stand in front of, or be driven into, the path of taxiing aircraft. No vehicle shall be parked or driven closer than 50-feet in front or 200-feet to the rear of any aircraft when engines are in operation or about to be put in operation. Vehicle shall not be parked facing the direction of an aircraft.

4.15 Vehicle operators shall obey the Dyess AFB speed limits for the area in question.

4.16 Airfield Operations: Before airfield operations (entering the airfield), vehicle operators shall insure that all equipment carried on their vehicles is properly stowed and secured and that the vehicles are inspected for objects that could damage aircraft. After vehicles are operated on surfaces off the airfield pavement, operators shall stop when reaching the airfield pavement and remove any rocks or other FOD that are wedged between the tires or treads prior to driving onto the airfield. Same applies when vehicles leaves improved pavement to unimproved pavement.

4.17 Vehicle operations on the airfield are under the control of Air Control Tower personnel. Runways shall not be accessed, unless required to do so for completion of the contract. Control tower light signals and radio instructions shall be observed and obeyed by vehicle operators. The following light signals if flashed from the control tower shall also control vehicle traffic:

Steady Green Light - Cleared to cross Flashing Green Light - Cleared to proceed and watch for lights Steady Red Light - Stop. Vehicle shall not be moved Flashing Red Light - Clear active runway immediately Red and Green Light - General warning. Exercise extreme caution

4.18 Other contractors and/or Air Force Personnel (Performing painting, rubber removal, concrete slab replacement operations, or other repair/construction operations) may be working on the airfield during the execution of this contract. Contractor shall cooperate fully with Government and these other contractors to allow an orderly progression of work. Weekly meetings may be held at the Contracting Officer’s office with the working Contractor(s) and other interested parties to discuss and adjust all Contractors’ upcoming work schedules. Contractor shall attend these meetings when and if scheduled.

4.19 Continuous cleanup of debris from Contractor’s activities (Outside of his actual work areas) on streets and the airfield is required with a Contractor provided vacuum sweeper. Any loose debris that could damage aircraft is not permitted on the airfield. Continuously sweeps haul routes with a sweeper.

An especially thorough cleanup is required at the end of each work day.

4.20 Avoid overloading and overfilling trucks. Load trucks to less than maximum legal (Unpermitted, H20 or 32,000 pound axle load) loads to allow transportation without damage to existing roads, streets, and shoulders at Dyess AFB. Also utilize high load bearing capacity portion of airfield runways, taxiways, and ramp; do not drive on asphalt shoulders.

4.21 Minimize work areas. Contractor shall generally complete a single work area before moving to another work area – unless specifically approved otherwise.
