Malmstrom Air Force Base (MAFB) Elevator Maintenance
This addendum is linked to solicitation number FA462625Q0029 and provides a platform for submitting proposals.
FAR 52.212-1 Addendum: Instructions to Offerors—Commercial Products and Commercial Services.
Instructions to Offerors
Note: The offerors must adhere to the instructions provided in the SF 1449 form and this addendum, as well as meeting all solicitation requirements. The offeror must adhere to the performance work statement (PWS) along with all its attachments. Failure to comply may result in the proposal being deemed ineligible for the award.
a. PWS and amendments - The Government reserves the right to revise or amend PWS or the solicitation, including all attachments, prior to the closing time for quotations. Any such revisions will be communicated through an amendment to this solicitation.
a.1. All amendments will be posted online with the solicitation documents without further notice. Offerors are responsible for checking and accessing all amendments before submitting quotations.
a.2. The offeror must acknowledge, sign, and submit all solicitation amendments along with their proposals.
b. Questions - All questions must be submitted within ten (10) calendar days of this solicitation posting.
c. Pricing - Offerors must complete the pricing for each of the contract line-item number (CLIN). This information can be found in the "Supplies or Services & Prices or Costs" section of SF 1449. Failure to provide prices for all CLINS on the pricing schedule will result in an incomplete proposal, potentially leading to its removal from further consideration. The unit prices should be rounded to two decimal places.
c.1. Firm Fixed Price - The resulting contract will be a firm fixed price. As such, the pricing will not be subject to revision or negotiation if selected for the contract award.
d. Past Performance Information – All completed past performance questionnaires must be submitted by the past performance point of contact (PP POC). Offerors should provide recent and relevant past performance POC's current email address and phone number for a maximum of five (5) Elevator maintenance services or services of a similar nature.
d.1. To be considered recently, for a particular contract or combination of contracts, services must have been and/or are currently being provided at any time within the last four (4) years from the date of this solicitation.
d.2. To be considered relevant, offerors must have performed services similar to the services described in the PWS.
e. Technical Capability/ANSI/ASME A17.1 - Offerors must submit either a copy of the ANSI certificate or a note in their offer indicating compliance with the safety code.
Additionally, offerors must possess a Montana Elevator License and include a mobilization plan. This documentation will be essential in demonstrating adherence to industry standards.
f. FAR 52.212-3 - To be eligible for an award, all vendors must be registered and active in SAM at the time of proposal submittal. Offerors are required to complete all necessary fill-ins and certifications found in the Federal Acquisition Regulation (FAR) 52.212-3, Offeror Representations and Certifications—Commercial Items, specifically in paragraph (b). The 52.212-3 document must be returned along with the proposal. Please provide the:
UEI Number/Cage Code and the Number of Employees or Total Yearly Revenue.
g. Site Visit at Malmstrom AFB – A site visit will be arranged upon request by one or or more offerors for the purpose of asking questions regarding this solicitation. The number of visitors must not exceed two (2) per firm.
All personnel requesting access to MAFB must possess a valid state or U.S. government-issued picture identification card. If individuals present an identification card from a state that is non-compliant with the REAL ID Act, they will need to provide additional documentation to gain unescorted access to the base. Acceptable additional documentation includes a valid U.S. or foreign government-issued passport, an employment authorization document with a photograph, or identification cards issued by federal, state, or local government agencies that include a photo and biographical information. A complete list of states that are compliant and non-compliant with the REAL ID Act can be found at https://www.dhs.gov/current-status-states-territories.
h. Notice to Offerors – Funds are presently available in the amount based on the Government estimate. The Government reserves the right to cancel this solicitation before or after the closing date. In the event the Government cancels this solicitation, it is not obligated to reimburse an offeror for any costs.
i. Quote Expiration – Offerors are encouraged to keep their proposed prices valid for a period of 60 calendar days from the date of submission.
j. Question pertaining to this solicitation are due on 15 July 2025/11:00 MST
**Please be advised that when submitting quotes** whether via mail, it is imperative to include an electronic version of all documentation at the time of submission. Please note that the Air Force Network System has specific restrictions: it does not accept submissions in a compressed “zip” file format when sent from non-government email addresses. And any files exceeding a size limit of 10 MB will be blocked and will not be received. Ensure your submissions are compliant to avoid any delays.
** Proposals submitted late will be handled according to FAR 52.212-1 (f): “Late submission, modifications, revisions, and withdrawals of offers.” **
FAR 52.212-2- Addendum: Evaluation—Commercial Products and Commercial Services.
Note: In accordance with FAR 12.602, the contract award process will adhere to Simplified Acquisition Procedures as outlined in FAR 13.106. The government intends to award a contract based on this solicitation to the proposer whose proposal meets the comprehensive requirements outlined in FAR 52.212-1 addenda. Proposals will be evaluated not only for their completeness but also for overall value to the government. This evaluation will consider price and other relevant criteria specified below:
1. Price
2. Past Performance
3. Technical Capability
The evaluation will be approached as a subjective trade-off, aligning past performance with price considerations.
The evaluation process shall be conducted as follows:
1. Price
a. The assessment concluded that there is a high likelihood of sufficient price competition for this requirement. If this competition diminishes, the Government will request that the offeror provide additional information to justify the reasonableness of the proposed price.
b. The Government will evaluate prices submitted in the SF section/CLINs boxes, as specified in FAR 52.212-1, paragraph c, pricing.
c. Offerors must provide pricing for each CLIN as well as the total price for all CLINs for the five-year ordering periods indicated in SF 1449.
d. In accordance with FAR Part 17, the Government will assess the potential for a service extension following the guidelines specified in FAR 52.217-8. To do this, the Total
Evaluated Price (TEP) will be calculated by dividing the total price for all CLIN by two.
The offeror must add the total price for all CLINs to the TEP to determine the final submitted price.
e. Price Reasonableness: The techniques and procedures described under FAR 13.106-3 will be the primary means of assessing the quotation’s reasonableness.
2. Past Performance
a. To be deemed recent, the previous services offered by the offeror must have been delivered at any point during the last four (4) years from the date this solicitation was issued. Services not provided within this timeframe will not be considered relevant for evaluation. Past performance can also include work done by predecessor companies, as well as services rendered as a subcontractor, through teaming arrangements, or in partnership with other companies. The offeror is required to submit a maximum of five
(5) past performance questionnaires. (See attachment 1) a.1. The past performance questionnaire should include the points of contact as specified in paragraph d of the instructions to offerors. The offeror must ensure that all CAGE numbers are provided for the company, any predecessor companies, key personnel who will be performing the job and possess relevant experience, as well as for any subcontractors or partnering companies.

b. All relevant recent and past performance references will be evaluated to assess the offeror's performance on previous contracts. The evaluation will take into account both the submitted past performance information and any additional data obtained from other sources. This process will result in an overall confidence rating for past performance, as detailed in the table below.
Table 2: Evaluation of Confidence Based on Past Performance Exceptional Given the offeror's recent and relevant performance record, the government has a high expectation that the offeror will successfully fulfill the required effort. The assessment of the element was completed with minor issues, for which corrective actions taken by the offerors were highly effective.
Very Good Given the offeror's recent and relevant performance record, the government has a reasonable expectation that the offeror will successfully perform the required effort. The assessment of the element was completed with minor issues, for which the contractor's corrective actions were effective.
Satisfactory Given the offeror's recent and relevant performance record, the assessed element contains some minor issues for which the contractor's corrective actions appear to be satisfactory.
Unsatisfactory Given the offeror's recent and relevant performance record, there is a low expectation that they will successfully complete the required work. The area being evaluated has significant issues, and the contractor's corrective actions seem to have been ineffective.
Neutral There is currently no recent or relevant performance record available, or the offeror's performance history is too limited to allow for a meaningful assessment. As a result, the offeror will not receive a favorable or unfavorable evaluation based on the factor of past performance.
b.1. If an offeror lacks a relevant record of past performance, or if information about the past performance is limited or unavailable, the offeror may not be evaluated favorably or unfavorably on past performance. In such cases, the offeror will be evaluated as neutral. Further, the offeror will have the opportunity to address any overall unfavorable past performance assessments.
b.2. Along with the past performance questionnaire, the government will obtain information from various other government sources, including the Supplier Performance Reporting System (SPRS), SAM, and any other known sources not provided by the offeror.
3. Technical Capability
a. ANSI/ASME A17.1 - while a certificate is not required, offerors may either submit a copy of the certificate or include a statement in their proposal indicating that the personnel performing the work are familiar with and trained in compliance with the safety code.
b. The Technical Capability section should include a written narrative that outlines the offeror’s technical, managerial, certification, and mobilization strategies to meet the requirements of the PWS. This narrative should effectively demonstrate the offeror’s ability to fulfill the technical requirements.
The written narrative must include the following components:
b.1. Qualifications/Certifications of Technicians The technicians possess current Montana Elevator Licenses or in the process of obtaining one. Montana elevator licenses are issued to inspectors, mechanics, and contractors. If the Offeror does not have current Montana elevator licenses, offeror must provide a plan for acquiring the required permits. *It is important to note that if a mechanic operates as a sole proprietor, they must also obtain a contractor's license.
b.2. Operations Plan The offers must outline the approach for mobilizing to the job sites. The mobilization plan should include a narrative explaining how the offeror intends to fulfill the emergency call requirements specified in PWS paragraph 5.2.
b.3 The Government will assess the offeror's proposed approach to ensure it is likely to achieve successful contract performance and will assign a rating to the technical proposal. The criteria for technical definition ratings are as follows:

Factor Description Exceptional Technical capability meets contractual requirements and provides significant benefits to the Government. The assessed elements indicate limited risk.
Very Good Technical capabilities align with contractual requirements. The elements being assessed indicate some potential risks.
Satisfactory The technical capabilities meet the contractual requirements, and the assessed element contains risks that are considered acceptable.
Unsatisfactory Technical capability does not meet the majority of contractual requirements, and recovery is unlikely to happen in a timely manner.
Evaluation Steps:
Step 1: The government will evaluate all proposals to ensure they comply with the relevant regulations. A compliant proposal is one that meets the government's requirements outlined in https://boards.bsd.dli.mt.gov/elevator-contractors/license-information/ sections 52.212-1 and 52.212-2. Proposals that do not fully comply may be excluded from consideration.
To be eligible for the award, offerors must qualify as a small business in SAM.gov at the time of submission. The link to small business administration (SBA) is https://sba.gov.
Step 2: The government will rank all eligible proposals based on the total submitted price, from lowest to highest.
Step 3: The government will evaluate the past performance of the lowest-priced offeror. If this lowest-priced offer is assessed with an "Exceptional" performance confidence rating and is judged to have an “Exceptional or Very good” technical capabilities, it will be considered the best value for the government. In this case, the evaluation process will end at this point, and the award will be granted to this offer without further consideration of any other proposals.
Step 4: If the government does not make an award under Step 3, it will evaluate the next lowest price offer. This process will continue in order of price until a proposal is found to have an "exceptional" past performance rating, along with "exceptional or very good" technical capabilities, or until all offers are evaluated. The government will make an integrated best-value award decision that considers price, past performance, and overall confidence.
Note: The government reserves the right to award a contract to offeror who meets the requirements specified in this addendum, not solely to the lowest-priced offeror.
INTERCHANGES – with one or more offerors may occur during the evaluation process in the manner of understanding and clarify aspects of an offeror’s proposal. The government intends to award a contract without engaging in interchanges with the respective offerors. However, the government reserves the right to conduct interchanges if it is deemed to be in its best interest.
