SYNOPSIS SOLICITATION
This is a combined synopsis solicitation for the installation of loop filtrations systems on selected hot water boilers at Fairchild AFB, WA. It is prepared in accordance with the format in FAR Parts 12 and 13.5 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation on FedBizOpps; a written solicitation will not be issued
This solicitation is a request for loop filtration installation at Fairchild AFB in accordance with the attached Statement of Work (SOW) and other associated documents. Solicitation FA4620-15-Q-A024 is issued as a request for quote, utilizing Simplified Acquisition Procedures in FAR Part 13 under NAICS code 238220 with a size standard of $15,000,000.00. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83 effective 3 Sep 2015. (d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph
(d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records--Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.
(e) (1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c), and (d) of this clause, the
Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1)in a subcontract for commercial items. Unless otherwise indicated below, the extent of the flow down shall be as required by the clause—

5352.201-9101 OMBUDSMAN (APRIL 2014)
(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern.
(b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).

52.219-28 POST-AWARD SMALL BUSINESS PROGRAM REREPRESENTATION (JULY 2013)
(a) Definitions. As used in this clause--
Long-term contract means a contract of more than five years in duration, including options. However, the term does not include contracts that exceed five years in duration because the period of performance has been extended for a cumulative period not to exceed six months under the clause at 52.217-8, Option to Extend Services, or other appropriate authority.
Small business concern means a concern, including its affiliates, that is independently owned and operated, not dominant in the field of operation in which it is bidding on Government contracts, and qualified as a small business under the criteria in 13 CFR part 121 and the size standard in paragraph (c) of this clause. Such a concern is ``not dominant in its field of operation'' when it does not exercise a controlling or major influence on a national basis in a kind of business activity in which a number of business concerns are primarily engaged. In determining whether dominance exists, consideration shall be given to all appropriate factors, including volume of business, number of employees, financial resources, competitive status or position, ownership or control of materials, processes, patents, license agreements, facilities, sales territory, and nature of business activity.
(b) If the Contractor represented that it was a small business concern prior to award of this contract, the Contractor shall rerepresent its size status according to paragraph (e) of this clause or, if applicable, paragraph (g) of this clause, upon the occurrence of any of the following:
(1) Within 30 days after execution of a novation agreement or within 30 days after modification of the contract to include this clause, if the novation agreement was executed prior to inclusion of this clause in the contract.
(2) Within 30 days after a merger or acquisition that does not require a novation or within 30 days after modification of the contract to include this clause, if the merger or acquisition occurred prior to inclusion of this clause in the contract.
(3) For long-term contracts--
(i) Within 60 to 120 days prior to the end of the fifth year of the contract; and
(ii) Within 60 to 120 days prior to the date specified in the contract for exercising any option thereafter.
(c) The Contractor shall rerepresent its size status in accordance with the size standard in effect at the time of this rerepresentation that corresponds to the North American Industry Classification System (NAICS) code assigned to this contract. (d) The small business size standard for a Contractor providing a product which it does not manufacture itself, for a contract other than a construction or service contract, is 500 employees.
(e) Except as provided in paragraph (g) of this clause, the Contractor shall make the representation required by paragraph (b) of this clause by validating or updating all its representations in the Representations and Certifications section of the System for Award Management (SAM) and its other data in SAM, as necessary, to ensure that they reflect the Contractor's current status. The
Contractor shall notify the contracting office in writing within the timeframes specified in paragraph (b) of this clause that the data have been validated or updated, and provide the date of the validation or update.
(f) If the Contractor represented that it was other than a small business concern prior to award of this contract, the
Contractor may, but is not required to, take the actions required by paragraphs (e) or (g) of this clause.
(g) If the Contractor does not have representations and certifications in SAM, or does not have a representation in

252.232-7006 WIDE AREA WORKFLOW PAYMENT INSTRUCTIONS (MAY 2013)
(a) Definitions. As used in this clause--
Department of Defense Activity Address Code (DoDAAC) is a six position code that uniquely identifies a unit, activity, or organization.
Document type means the type of payment request or receiving report available for creation in Wide Area WorkFlow
(WAWF).
Local processing office (LPO) is the office responsible for payment certification when payment certification is done external to the entitlement system.
(b) Electronic invoicing. The WAWF system is the method to electronically process vendor payment requests and receiving reports, as authorized by DFARS 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports.
(c) WAWF access. To access WAWF, the Contractor shall--
(1) Have a designated electronic business point of contact in the System for Award Management at https://www.acquisition.gov; and
(2) Be registered to use WAWF at https://wawf.eb.mil/ following the step-by-step procedures for self-registration available at this Web site.
(d) WAWF training. The Contractor should follow the training instructions of the WAWF Web-Based Training
Course and use the Practice Training Site before submitting payment requests through
WAWF. Both can be accessed by selecting the “Web Based Training” link on the WAWF home page at https://wawf.eb.mil/.
(e) WAWF methods of document submission. Document submissions may be via Web entry, Electronic Data
Interchange, or File Transfer Protocol.
(f) WAWF payment instructions. The Contractor must use the following information when submitting payment requests and receiving reports in WAWF for this contract/order:

Addendum to FAR 52.212-1 – Instructions to Offerors – Commercial Items
To ensure timely and equitable evaluation of the proposal, the offeror must follow the instructions contained herein.
The proposal must be complete, self-sufficient, and respond directly to the requirements of this solicitation.

A. Complete blocks 12, 17a, 30a, 30b, and 30c of the SF 1449. In doing so the contractor accedes to the contract terms and conditions as written in the solicitation with attachments. The SOLICITATION constitutes the model contract.
B. SUBMISSION OF OFFERS. Submit signed and dated offers to the office specified in this solicitation, block 9, at or before the exact time specified in this solicitation. Offers that are emailed or faxed may be delayed an average of one hour processing through AF infrastructure before being received by the government. Offerors who hand carry their offers may encounter average delays of one hour or more while processing through security.
Along with the completed SF 1449 proposal shall include the following three (3) items:
1. Price: Insert proposed unit and extended prices into the Solicitation’s Price Schedule. The unit price and total price for the line item shall be exact amounts.
2. Past Performance
a. Past Performance Surveys: The Government will evaluate the quality and extent of offeror’s performance deemed relevant to the requirements of this solicitation. The Government will use information submitted by the offeror and other sources such as other Federal Government offices, state and municipal offices, and commercial sources, to assess performance. Provide a list of no more than three (3), of the most relevant contracts performed for Federal agencies and commercial customers within the last two (2) years. Relevant contracts include those contracts that provide installation of loop filtration systems or similar to
Government or commercial organizations. The evaluation of past performance information will take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the current acquisition. However, subcontractor, and predecessor company information will not be considered as highly as past performance information for the principal offeror.
Furnish the following information for each contract referenced:
1. Company/Division name

6. Period of Performance
7. Verified, up-to-date name, address, FAX, and telephone number of the contracting officer

If no Past Performance is available the offeror may not be evaluated favorably or unfavorably on past performance: the offeror will receive a neutral rating.
b. Subcontractor Consent: Past performance information pertaining to a subcontractor cannot be disclosed to the prime offeror without the subcontractor’s consent. If applicable, provide with the quote a letter from all subcontractors that will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor.
3. Amendments (if applicable) Offers are to complete, sign and return all amendments issued against the solicitation
C. If the offer is not submitted on the SF 1449, include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.
ADDENDUM TO FAR 52.212-2
I. Evaluation Process
The Government will award a contract resultng from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other non-price factors considered. The following factors shall be used to evaluate offers:
1. Price and
2. Past Performance
Past performance is approximately equal to cost or price
II. Basis For Contract Award
The evaluation process shall proceed as follows:
A. PRICE – Proposals will be ranked according to the proposed price identified in the Contract Line Items.
An offeror’s proposed prices will be determined by multiplying the quantities identified in the Priced Line
Items by the unit price for each item to confirm the extended amount for each. The offeror’s Price proposal will be evaluated, using one or more of the techniques defined in FAR 15.404, in order to determine if it is reasonable. For a price to be reasonable, it must represent a price to the Government that a prudent person would pay in the conduct of competitive business. Normally, price reasonableness is established through price analysis techniques described in FAR 15.404. For additional information see FAR 31.201-3. At this point, past performance of the lowest priced offer will be evaluated.
B. PAST PERFORMANCE information will be evaluated based on its relevancy and the quality of work the offeror provided to its past performance references. The Past Performance Evaluation Team will evaluate relevancy of past performance information using the ratings found in the table below. In evaluating past performance relevancy, the Government reserves the right to give greater consideration to those contracts deemed most relevant to the effort described in the solicitation.

The Government will then evaluate past performance information meeting the “Relevant” rating on the quality or how well the contractor performed the contracts. Using the Past Performance questionnaire and other sources such as, but not limited to Past Performance (PPIRS), Federal Awardee Performance and
Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), or other databases; interviews with Program Managers, Contracting Officers, or Past perforamance reference. The
Past Performance Evaluation Team will review this past performance information and determine the quality and usefulness as it applies to performance confidence assessment.
In conducting a performance confidence assessment, each offeror shall be assigned one of the ratings in the table below.

Confidence (Neutral) No recent/relevant performance record is available or the offeror’s performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned.
If the lowest priced offer is evaluated to have a “Substantial Confidence” performance assessment is determined to be responsible, that offer represents the best value for the Government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration of any other proposals.
In the event that the Government does not make an award pursuant to the paragraph above, the Government reserves the right to award a contract to other than the lowest priced offer, if the offeror is judged to have a performance confidence assessment of “Unknown Confidence” or higher. If the lowest priced offeror is not judged to have a “Substantial Confidence” performance confidence assessment, the next lowest priced offer will be evaluated and the process will continue (in order by price) until an offeror is judged to have a
“Substantial Confidence” performance confidence assessment or until all offerors are evaluated. The
Contracting Officer shall then make an integrated assessment best value award source selection decision.
Offerors are cautioned to submit sufficient information and in the format specified in the proposal preparation instructions to permit a meaningful assessment of past performance. Offerors may be asked to clarify certain aspects of their proposal or respond to adverse past performance information to which the offeror has not previously had an opportunity to respond. Adverse past performance is defined as past performance information that supports a less than satisfactory rating on any evaluation element or any unfavorable comments received from sources without a formal rating system. Communication conducted to resolve minor or clerical errors will not constitute discussions and the Contracting Officer reserves the right to award a contract without the opportunity for proposal revision.
The Government intends to award a contract without discussions with respective offerors. The
Government, however, reserves the right to conduct discussions if deemed in its best interest.
