Evaluation Factors for Award
Miscellaneous text in this section has been modified to:
FAR 52.212-2 ADDENDUM
M-1 BASIS FOR CONTRACT AWARD: This requirement will be awarded to the responsible Offeror whose quote conforming to the solicitation is determined to represent the best value for the Government based on proposals meeting price, technical acceptability and acceptability standards for all non-cost factors (i.e. Past Performance, Attachment 4). This acquisition is being conducted in accordance with FAR Part 13 Simplified Acquisition Procedures. By submission of its offer, the Offeror accedes to all solicitation requirements, including terms and conditions, representations and certifications, technical requirements, in addition to those identified as evaluation factors or sub-factors. The Offeror accedes to the terms and conditions of the model contract and all such offers shall be treated equally except for their prices, technical evaluation and past performance evaluation. An "unacceptable" rating in any factor will result in the overall technical proposal being rated "unacceptable "and the proposal will therefore be ineligible for award. The Government shall utilize information provided in Attachment 3 Technical Questionnaire, attachment 4 Past Performance Questionnaire and Attachment 5 Teaming Arrangement as well as information available from Government Past Performance collection systems when identifying the offer that represents the Best Value.
Negative Past Performance information identified by the Government could result in an "Unacceptable" Past Performance rating. Past Performance is relatively the same in importance as Price and Technical Acceptability.
While the Government will strive for maximum objectivity, the selection process is by its nature subjective.
Therefore, professional judgment is implicit throughout the entire process. The evaluation process shall proceed as follows: The following documents will be evaluated for completeness and compliance with the requirements of the RFQ.
1. Part I, SF1449
(a) Signed copy of the SF1449 Solicitation / Contract / Order for Commercial Items document. Is the SF1449 completed and signed by an official authorized to bind the company? (Blocks 12, 17a, 26, 30a, 30b, 30c)
(b) SF1449, Page 3, Supplies or Services and Prices/Cost Section. Is the price schedule accurately and completely filled in by the offeror? Are the prices rounded to the whole dollar? (Extended amount for all CLINs 0001 - 5011)
(c) SF1449, Page 1, Block 26. Is Block 26 accurately filled in with the Total Evaluated Price (TEP) from Attachment 2, Pricing Schedule, Final Prices tab? The TEP is derived by totaling all CLINs 0001-5011.
(d) Solicitation Amendments (if applicable). Did the offeror acknowledge all amendments to the Request for Quote?
(e) System for Award Management (SAM) Registration. Has the offeror completed SAM registration and is the registration active? (Government will validate via SAM website) Part II, Attachment 2 - Pricing Schedule.
(a) For each ordering period tab, has the offeror filled in unit prices for all items that have an estimated quantity other than zero, and are mathematical calculations correct, to include extended prices for each priced line item and the summary totals on the Final Prices tab.
FA461022Q00120002
(b) Is all the information reflected on the SF1449 accurately?
(c) Price Quotes will be ranked from lowest to highest Total Evaluated Price. The Total Evaluated Price will be derived from Attachment 2, Pricing Schedule, Final Prices tab; this same figure shall be entered in Block 26, SF1449, Page 1. The offeror is responsible for ensuring the accuracy of these values.
Note: Due to funding limitations any Total Evaluated Price in excess of $1,400,000.00 will not receive further evaluation and will be ineligible for award.
Part III, Attachment 3 - Technical Questionnaire
(a) The Government technical evaluation team shall evaluate the technical quotes on an acceptable/unacceptable basis, assigning a rating of "Acceptable" or "Unacceptable" rating. An offer must be rated "Acceptable" in ALL factors to be eligible for award. An "Unacceptable" rating in any factor will result in the overall technical proposal being rated "Unacceptable" and the proposal will therefore be ineligible for award. Technical acceptability will be established based on if the proposed narrative and the Governments minimum requirements as identified in the Performance Work Statement. This includes completely filling out Attachment 3, Technical Questionnaire and specifically addressing each factor in their entirety. Failure to address each factor within the Technical Questionnaire will result in an "Unacceptable" rating.
(b) The following adjectival rating and rating definitions will be used to assign a rating for each technical factor and subfactor and an overall rating to each technical quote.
Rating Definition Acceptable: All of the minimum criteria as defined in the technical questionnaire are clearly met by the quote.
The offeror's quote meets the performance and technical capability requirements defined in the Performance Work Statement.
Unacceptable: Not all the minimum acceptable criteria are met by the quote. An unacceptable quote contains one or more deficiencies. Quote fails to meet specified minimum performance and technical capability requirements defined in the Performance Work Statement.
Part IV, Attachment 4 - Past Performance Questionnaire Upon receiving a rating of Technically Acceptable, the Lowest TEP Technically Acceptable offer shall be given a Past Performance rating of Good, Acceptable, or Unacceptable. A rating of unacceptable will result in the offer being ineligible for award. If the lowest TEP Technically Acceptable offer receives a rating of Good the Government shall conclude evaluations and make an award decision. No further offers will be evaluated.
However, the Government may at its discretion evaluate the next lowest TEP Technically Acceptable offer if deemed in the Government's best interest. If the lowest TEP Technically Acceptable Proposal receives a rating of Acceptable for Technical Experience, the Government will continue to evaluate offers until an offer receives a rating of Good. If no offer receives a rating of Good, the Government may award to the Lowest TEP Technically Acceptable offer receiving a rating of Acceptable, pending a Determination of Price Fair and Reasonable by the Contracting Officer. The Government reserves the right to review one, some, or all quotes received in response to the solicitation. The Government reserves the right to cancel the solicitation at any time during the review process.
Factor 1, Past Performance Portable Toilet Rental and Servicing. At least three (3) years of experience within the five (5) year period immediately preceding release of this RFQ providing Portable Toilet Rental and Servicing.
Standard of Acceptability: Upon receiving a rating of "Acceptable" for technical acceptability, the offeror's responses evidence the offeror (or the entity other than the offeror which the offeror intends or proposes to use to fulfill a requirement or portion thereof) has at least (3) years of experience providing Portable Toilet Rental and Servicing in accordance with the Performance Work Statement within the last five (five) years preceding the release of this Request for Quote.

Acceptable- The Offeror' s past performance meets the minimum requirements stated in the Performance Work Statement, the minimum years of experience and Factor 1 Past Performance.
Unacceptable- The offeror's experience does not meet the requirements stated in the Performance Work Statement, the minimum years of experience and Factor 1 Past Performance.
M-2 CONTRACT AWARD. The Government intends to award, without discussion, to the offeror whose proposal represents the best value to the Government. Offerors are cautioned to submit sufficient information and in the format specified in 52.212-1 Addendum. Offeror's may be asked to clarify certain aspects of their proposal. Communication conducted to resolve minor or clerical errors will not constitute discussions and the Contracting Officer reserves the right to award a contract without the opportunity for proposal revision.
Interchanges. Offerors are cautioned to submit sufficient information and in the format specified in 52.212-1 Addendum. The Government intends to award without discussion to the offeror whose proposal represents the Best Value to the Government. Offeror's may be asked to clarify certain aspects of their proposal.

The Government intends to award a contract without interchanges with respective offerors. M-3 CONTRACTOR RESPONSIBILITY. Prior to an award, an offeror may be requested to provide business information for the purpose of a responsibility determination in accordance with FAR Part 9.
