High-Altitude Electromagnetic Pulse (HEMP) Hardness Maintenance/Hardness Surveillance (HM/HS) Engineering/Technical Support Site Visit Announcement

An organized site visit has been scheduled for :

17 November 2017 @ 0900

Participants will meet at:

Offutt Visitor Control Center

Located at the SAC Gate off of Capehart Road

Offutt AFB NE, 68113

1(a) All participants are required to meet and sign in. All participants will board transportation provided by the Government regardless of base access privileges.

1(b) Please RSVP by submitting a completed Contractractor Personal Information List and Offutt Air

Force Base Entry Authority List (EAL-55 SFS Form 31), attached hereto, to Angela LeRoy at angela.leroy@us.af.mil and Melissa Hergenrader at Melissa.Hergenrader@us.af.mil by 9 November

2017 by 10:00 A.M. CST. Any person who fails to sumbit a completed Contractractor Personal

Information List and EAL-55 SFS Form 31 by the required date/time will be excluded from the site visit.

Contractors will be limited to two (2) participants for each contractor for the site visit due to the limited number of escorts available. If there are any questions contact Angela LeRoy and TSgt Daniel H.

Halverson by the same means listed above.

1(c) Photographs are not authorized to be taken by participants.

1(d) Attached is the required information to participate in the site visit. All Contractors must complete and submit a complete Contractor Personal Information List and EAL-55 SFS Form 31.

1(e) All questions generated during the site visit must be submitted in writing to the contract specialist and the contracting officer for an official response. Address, in writing, technical and contractual questions to angela.leroy@us.af.mil and melissa.hergenrader@us.af.mil by close of business (COB) no later than three (3) days after the site visit. If questions are received after that date, the Government may or may not answer them.

1(f) All participants must wear the following personal protective equipment (PPE) during the site visit to

Building 1000: Reflective Safety Vest, Hard Hat, Gloves, Safety Glasses and Safety Boots. Participants must supply their own PPE.

1(g) This site visit will be physically intensive as it requires climbing numerous steps, a great deal of walking and inspecting filters located outside the building(s).

(SSN). Purpose: Information collected by Security Forces personnel will be used to authorize installation entry to persons shown on the entry authority listing for the specific purpose provided by the sponsor.

Information will also be used to notify person(s) designated as the visitor sponsor of arrival and other pertinent information relating to entry on the installation. Routine Uses: Records may be disclosed to applicable Federal, State, or local agency charged with the responsibility of investigating or prosecuting a violation or enforcing/implementing a statute, rule, regulation, order issued pursuant thereto.
