On-base haul routes shall be maintained such that a passenger vehicle may travel safely on it at all times. Dust shall be controlled to prevent creation of a nuisance in the surrounding area. Haul routes shall be restored to existing condition upon completion of the project at no additional cost to the Government.
3.3 DRAWINGS AND SPECIFICATIONS: At least one set of drawings and specifications shall be kept at the job site by each superintendent.
3.4 WORK CONDUCT, CORRELATION, EXECUTION, ETC.:
3.4.1 Work Performance: Contractor shall conduct all work, including materials distribution in an orderly manner with minimum disturbance and inconvenience to traffic operations. Contractor shall confine and limit his personnel to only those areas required for work performance. All work shall be performed in a neat, orderly fashion, with extreme care exercised to avoid damage to all property in and around project areas. Any damage caused by the Contractor shall be replaced, repaired, or restored at no cost to the Government. Upon completion, all work and storage areas shall be cleaned of all evidence of Contractor's debris accumulations. Contractor's proposed work schedule shall be coordinated with 5 OSS/A3AA and the Contracting Officer or his designated representative 14 days prior to the start of any work. All work areas shall be barricaded properly for day and nighttime vehicular safety warning.
3.4.2 The Contractor will be required to coordinate with 5 OSS/A3AA (221 Flightline Drive) daily. The Contractor will attend a weekly meeting with the Air Force to report progress and coordinate future work. This is essential to avoid conflict with other projects that will be constructed concurrently.
3.4.3 Storage: At close of each day's operation, all excess
materials and equipment shall be moved to Contractor's staging area. If Contractor should have storage structure or equipment located on-base, it shall be removed and storage area restored to its original condition within 21 days after final inspection of this project. The Government shall not be liable for the security of any of the Contractor's equipment or material while on Minot Air Force Base.
3.5 QUALITY CONTROL: The Contractor shall provide a qualified Quality Control Inspector who is familiar with all phases of operation. This individual shall be named and qualifications stated in the Quality Control Plan. The Quality Control Inspector's sole responsibility shall be to make
01000-3 constant quality control inspections of all phases of the operation, in order to assure constant compliance with the contract specifications. A Quality Control Plan shall be submitted prior to the start of any work.
3.6 PERMITS AND WORK CLEARANCES:
3.6.1 Clearance Work Request:
3.6.1.1 Contractor shall coordinate with appropriate utility
locating agencies, including base communications cable maintenance (Item 1 through 15 of AF Form 103, Base Civil Engineering Work Clearance Request). AF Form 103 will be given to the Contractor during the pre-construction meeting.
Contractor shall complete and follow all instructions on AF Form 103 before start of work on base. A copy of AF Form 103 instructions will be available to the Contractor during the pre-construction meeting.
3.6.1.2 Before any excavation, alteration, or other
construction work is begun, the Contractor shall obtain and fully process an AF Form 103, Base Civil Engineering Work Clearance Request. The form requires extensive coordination with several base agencies, including furnishing all locations, detailed information, and sketches regarding any proposed excavation(s). Processing a separate request for each planned excavation is required; each phase of processing the form and its attachments shall be completed prior to commencing work involving that phase. The AF Form 103 shall be prominently displayed on the job site at all times for review by the Contracting Officer and/or his or her designated representative. A copy of the signed form shall be submitted to the Contracting Officer and a copy to the project inspector. Completion of the AF Form 103 includes certifying compliance with institutional controls indicated by CEAN, coordination in getting the form coordinated, and returning the form to the Environmental office when the project has been completed.
3.6.1.3 Contractor shall schedule a meeting time to meet with each utility agency at the work site to locate affected utilities.
3.6.1.4 Contractor shall maintain all marked utilities in the field once they are initially marked by each utility agency. If markings are damaged, Contractor shall request new locates and shall give the locating agency 48 hours to perform the new locates. Expenses for time and materials to perform repeat locates shall be borne by the Contractor, unless otherwise approved by the Approving Officer. Cost to do the repeat locates will be deducted from the final invoice. Cost will be calculated using the average shop rate of each agency. In cases where utilities were marked late in the year and operations had to be shut down for winter, marked utilities would still have to be maintained by the Contractor by placing stakes or transcribing the marks on a drawing. When construction resumes, Contractor shall re-mark the site and may request verification of utility locations at no
01000-4 additional cost to him. Off-base utility locates agencies might request additional cost for repeat locates.
3.6.1.5 Clearance request is not valid until signed by the
Approving Officer, block 17 of the AF Form 103. Approving Officer will not sign until all locates are marked and all utility locating agencies signatures are on the form. Excavation work shall commence within 30 days after the Approving Officer's signature; otherwise, clearance request is invalid and a new request is required. Notify Contracting Officer immediately if new utilities were installed by others and not marked after all initial locates are made.
3.6.1.6 Protection of Existing Utilities: The Contractor
shall safeguard and protect from damage all utilities encountered or uncovered during digging, excavation, and backfilling.
3.6.2 AF Form 592: When any welding, cutting, brazing or use of any open flame device is to be used, an AF Form 592 permit must be obtained from the Base Fire Department (Building 896, 320 Flightline Drive) prior to beginning such work. ENVIRONMENT PROTECTION
1.1 The Contractor shall perform all work in such manner as to minimize the polluting of air, water, or land, and shall, within reasonable limits, control noise and the disposal of solid waste materials, as well as other pollutants.
1.2 IMPLEMENTATION: Within 20 calendar days after Notice to Proceed and prior to commencement of the work at the site, the Contractor shall:
1.2.1 Detail his proposal for implementing the requirements for environmental pollution control specified herein.
2. PRODUCTS: (Not Used)
3. EXECUTION:
3.1 PRECONSTRUCTION SURVEY: Prior to start of any on-site construction activities, the Contractor shall make a condition survey after which the Contractor shall record on a layout plan the condition of trees, shrubs and grassed areas immediately adjacent to the site of the work and adjacent to his assigned storage area and access route as applicable.
3.2 PROTECTION OF LAND AREAS: Except for work areas, storage areas, or access routes specifically assigned for the use of the Contractor under this contract, the land areas outside the limits of permanent work performed under this contract shall be preserved in their present condition. The Contractor shall confine his construction activities to areas defined for work on the plans or specifically assigned for his use. Storage and related areas and access routes required temporarily by the Contractor in the performance of the work will be assigned by the Contracting Officer.
3.3 PROTECTION OF WATER RESOURCES: The Contractor shall control the disposal of fuels, oils, bitumens, calcium chloride, acids or harmful materials, both on and off the Government premises and shall comply with applicable federal, state, county and municipal laws concerning pollution of rivers, streams, and wetlands while performing work under this contract.
Special measures shall be taken to prevent chemicals, fuels, oils, greases, bituminous materials, herbicides, and insecticides from entering public waters. Water used in on-site material processing, concrete curing, 01431-2 foundation and concrete cleanup, and other waste waters shall not be allowed to reenter a stream if an increase in the turbidity of the stream could result therefrom.
3.3.1 Disposal of water generated during construction dewatering.
3.3.1.1 Dewatering of Groundwater: For temporary dewatering
of groundwater encountered during construction, the Contractor shall land apply all water generated in accordance with Paragraph: LAND APPLICATION.
3.3.1.1.1 Land Application: Land application shall
follow state requirements; however, no permit will be necessary. "Land application" implies that the Contractor shall discharge water at a rate which allows the water to percolate into the soil. No sheeting action, soil erosion, discharge into storm sewers, defined drainage areas, or discharge into the "waters of the State" shall occur. The Contractor shall contact the State of North Dakota, Division of Water Quality and Permit Program, to discuss the water quality parameters for chemical contaminants (such as chlorine), oil, grease, and total suspended solids and shall ensure that all water intended for land application will meet the state requirements.
3.4 WASTE DISPOSAL: As part of his proposed implementation under paragraph 1.2, and prior to on-site construction, the Contractor shall submit a description of his scheme for handling and disposing of all waste resulting from the work under this contract. If any waste is dumped in unauthorized areas, the Contractor shall remove the waste and restore the area to the condition of the adjacent undisturbed areas. Where directed, soil which is contaminated by the Contractor shall be excavated, disposed of as approved by the Contracting Officer, and replaced with suitable fill material, all at the expense of the Contractor.
3.5 BURNING: Burning shall be prohibited on the Government premises.
3.6 DUST CONTROL: The Contractor shall maintain all excavations, embankments, stockpiles, access roads, plant sites, waste areas, borrow areas, and all other work areas free from excess dust to such reasonable degree as to avoid causing a hazard or nuisance. Temporary methods consisting of sprinkling, chemical treatment, light bituminous treatment or similar methods will be permitted to control dust. Dust control shall be performed as the work proceeds and whenever a dust nuisance or hazard occurs.
3.7 EROSION AND SEDIMENT CONTROL: The Contractor shall be responsible for providing erosion and sediment control measures in accordance with local, state, and federal regulations. The erosion and sediment controls selected and maintained by the Contractor shall be such that local, state, and federal water quality standards are not violated as a result of the Contractor's construction activities. The area of bare soil exposed at any one time by
01431-3 construction operations should be held to a minimum. For areas subject to erosion including, but not limited to, steep slopes, soils spreading areas, and stockpile areas, the Contractor shall disturb as little vegetation as possible, minimize the period of time that the soil is exposed, prevent surface water runoff from flowing across disturbed areas, stabilize disturbed soils as soon as possible, and provide adequate drainage for the increased runoff. Sedimentation/erosion control features may include, but not be limited to, vegetation cover, stream bank stabilization, slope stabilization, construction of terraces, interceptor channels, sediment traps, inlet and outfall protection, diversion channels, and sedimentation basins. Runoff management practices should be employed in areas where the potential for storm water contamination exists that can not be addressed by a prevention or source reduction measure. Runoff management procedures may include vegetative swales, reuse of collected storm water, infiltration devices, and detention/sedimentation basins. Any erosion and sediment control measures which are damaged or found to be ineffective shall be remedied by the Contractor at no additional cost to the Government. Any areas damaged by erosion shall be repaired by the Contractor at no additional cost to the Government.
3.8 CORRECTIVE ACTION: The Contractor shall, upon receipt of a notice in writing of any noncompliance with the foregoing provisions, take immediate corrective action. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work until satisfactory corrective action has been taken. No part of the time lost due to any such stop orders shall be made the subject of a claim for extension of time or for excess costs of damages by the Contractor unless it is later determined that the Contractor was in compliance.
3.9 POST-CONSTRUCTION CLEANUP OR OBLITERATION: The Contractor shall, unless otherwise instructed in writing by the Contracting Officer, obliterate all signs of temporary construction facilities such as haul roads, work areas, structures, foundations of temporary structures, stockpiles of excess or waste materials, and other vestiges of construction prior to final acceptance of the work. The disturbed areas shall be graded and filled and the entire area seeded.
