SAMPLE QUESTIONNAIRE COVER LETTER

SUBJECT: Request for Past Performance Evaluation

TO:

The 436 CONS/LGCB of the 436 Air Mobility Command at Dover AFB DE is in the process of selecting a contractor for the Grounds Maintenance Contract. This is an acquisition for a service contract to provide firm-fixed grounds maintenance to support Dover AFB, DE. The contractor provides all personnel, equipment, tools, supervision, and other items and services necessary to ensure that grounds maintenance is performed at Dover Air Force Base in a manner that will maintain healthy grass, trees, shrubs, and plants and present a clean, neat, and professional appearance.

One of the considerations in proposal evaluation is the verification of the past and present performance on contracts which reflect the contractor’s ability to perform on the proposed effort.

The areas of interest are summarized in the enclosed questionnaire. (As discussed in our initial phone contact with your office, the government’s schedule is extremely tight and we need your written response to be forwarded to 436 CONS/LGCB not later than 7 calendar days after your receipt of this letter.)

To assist you in preparing your response and expediting your reply, the questionnaire may be filled out by hand and “faxed” to 302-677-2309, 436 CONS/LGCA (Attention: Luz Santiago). Please call Luz Santiago at 302-677-5234 prior to transmission or if you have any questions. Your completed questionnaire will become a part of the official Source Selection records.

Your help is greatly appreciated and your proposed response will be one of the keys to the successful and timely completion of this Source Selection.
