SCOPE OF WORK

Pave Parking Lot at ½ Mile Track at Dix

Project Number: HEKP17-1036

Date: 01 Oct 19

GENERAL:

1. Contractor shall provide construction services to include: Personnel, equipment, tools, materials, transportation, and supervision necessary to remove an estimated 8” of stone material from parking area to be paved. NOTE: polypropylene fabric is under stone, if disturbed during stone removal it must be replaced with like item. Contractor shall replace subbase material with an estimated 4” of ¾” – 1” Trap Rock DGA material (no crushed concrete will be used), level, grade and compact to meet paving requirements for water drainage and to maintain a 6” curb height. Contractor shall use 2” compacted I-2 base asphalt and 2” compacted I-5 top asphalt. Contractor shall remove and replace an estimated 90 LF of curbing, repair, parge and install four each NJ DOT-DEP approved storm grates and risers, No slump test required on concrete.

Contractor shall stripe, paint arrows, and install two each one-way NJ DOT approved arrow signs and posts at parking lot entrance. Contractor shall install three each new concrete catch basins with DOT approved grates and an estimated 300 LF of 8” C900 green drain pipe with tracer wire, 45 degree fittings , compact and backfill to drain water in low area behind building. Contractor shall install an estimated 1,475 SF of new sidewalks, before installing sidewalks install 2” grey schedule 40 electrical conduit 18” deep under sidewalks, six locations for future use, temporary cap and mark location with an ID marker. Contractor shall regrade all disturbed areas to prevent ponding of water, install topsoil, rake and reseed disturbed areas. The period of performance for this delivery order shall be 300 days from notice to proceed date.

SCOPE OF WORK:

2.1 Contractor shall attend a site visit at the one mile track parking area with the Government QAR and

Project Manager to assess the project and actual working conditions.

2.2 Contractor shall stage his work in a manner that will minimize interference with the daily operation of

JB-MDL and employees. General tasks required to complete this project are as follows:

2.2.1 Contractor shall obtain all utility mark outs prior to starting work on this project. Water, sewer, storm, and local telecom shall be requested through the Government’s QAR and other utilities shall be requested through New Jersey One Call, dial 8-1-1 or 800-272-1000.

2.2.2 Contractor shall remove, transport, and dispose of all construction debris off base at the

2.2.3 Contractor shall remove an estimated 8” of stone base material from existing parking area to be paved.

2.2.4 Contractor shall replace subbase material with an estimated 4” of ¾” - 1” of Trap Rock

DGA material ~ NO CRUSHED CONCRETE WILL BE USED

2.2.5 Contractor shall load, haul and dump removed stone material to a location at building 5348 storage area or other area as per the QAR.

2.2.6 Contractor shall remove an estimated 90 LF of concrete curbing and replace to grade new curbing to match old curbing ~ no slump test required.

2.2.7 Level, grade and compact as required to meet new asphalt paving to maintain a 6” curb height.

2.2.8 Install 2” compacted I-2 base asphalt.

2.2.9 Install 2” compacted I-5 top asphalt

2.2.10 Contractor shall repair, parge and replace 4 each grates and risers to meet NJ DOT-DEP standards.

2.2.11 Paint 4” white and blue lines as per the QAR.

2.2.12 Paint traffic arrows as per QAR.

2.2.13 Install two each NJ DOT approved one-way signs and posts at parking lot entrances.

2.2.14 Replace any polypropylene fabric that is disturbed or removed during placement of subbase material.

2.2.15 Contractor shall dig and form out an estimated 1,475 SF for new sidewalks as per the QAR

~ 3” of ¾” clean stone under sidewalks.

2.2.16 Contractor shall install an estimated 130 LF of 2” grey schedule 40 electrical conduit 18” deep under sidewalks, temporary cap ends at six locations, place locator marks at end of pipe to be used to find pipe in the future.

2.2.17 Trench and install from new catch basins behind building ~ cross parking area ~ and enter road catch basin box , an estimated 300 LF of 8” C-900 green drain pipe with 45 degree fittings , compact and backfill. No 90 degree bends, 45 degree bends only.

2.2.18 Contractor shall install at grade three each pre-cast H-20 load concrete catch basin with a grate 22” X 48”, must be NJ DOT DEP approved. Fill with concrete to form flow channel.

2.2.19 Contractor shall provide traffic control measures to construction area.

2.2.20 Contractor shall submit a completed DD Form 1354 (attached) prior to final acceptance and final payment.

2.3 Contractor shall comply with the most current edition of the National Fire Codes, installation fire prevention standards, and Army Regulation 420-90. All open flame/hot work requires prior approval of the Dix Fire Prevention Bureau at 609-562-5484.

2.4 Contractor shall notify the QAR 48 hours prior to starting work and prior to final completion of the job.

3. Safety: Contractor shall comply with Installation safety regulations, Engineer Manual 385-1, and all applicable

OSHA Standards, AFOSH (Air Force Occupational Safety and Health) manuals. Contact the Joint Base Safety Office for further information at 754-SAFE or (609)562-3081.

4. Fire Prevention: Contractor shall comply with all requirements of the installation fire prevention standards, Army

Regulations, and the most current issue of the National Fire Code.

5. Environmental: Contractor shall ensure compliance with all Federal, State, and local environmental regulations.

The type and quantity of all hazardous materials brought to the work site shall be reported to the Dix Environmental office at (609)562-3050/5325.
