Addendum to 52.212-1(c), Period for Acceptance of Offers. The paragraph is tailored as follows: “The offeror agrees to hold the prices in its offer firm for 120 calendar days from the date specified for receipt of offers.”
A. To assure timely and equitable evaluation of the proposal, the offeror must follow the instructions contained herein. The proposal must be complete, self-sufficient, and respond directly to the requirements of this solicitation.
The response shall consist of two separate parts; Part I – Price Proposal, Part II – Present/Past Performance Information.
The contracting officer has determined there is a high probability of adequate price competition in this acquisition.
Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional data will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit price data to the extent necessary for the contracting officer to determine the reasonableness and affordability of the price.
C. Specific Instructions:
1. PART I – PRICE PROPOSAL - Submit one (1) copy.
(a) Complete blocks 12, 17a, and 30a, b, and c of the SF1449. In doing so, the offeror accedes to the contract terms and conditions as written in the solicitation, with attachments. The solicitation constitutes the model contract.
(b) Insert proposed unit and extended prices in the Pricing Schedule. The extended amount must equal the whole dollar unit price multiplied by the number of units. The proposal(s) must be submitted for a 12-month base period and four (4) 12-month option years.
(c) Complete the necessary fill-ins and certifications in provisions. The provisions FAR 52.212-3, Offeror Representations and Certifications – Commercial Items, shall be returned along with the proposal.

(b) Present/Past Performance References. The government will evaluate the quality and extent of offeror's present/past performance deemed relevant to the requirements of this RFP. The government will use information submitted by the offeror, and any other sources of information available to the government, to assess present/past performance. Provide a list of no more than ten (10), of the most relevant present/past performance references performed for Federal agencies and commercial customers within the last five (5) years. Relevant contracts include logistical support services that are similar to or greater in scope, magnitude, and complexity than the effort described in the PWS. The evaluation of present/past performance information will take into account present/past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the instant acquisition.

ADDENDUM TO 52.212-2
ADDENDUM TO 52.212-2 EVALUATION – COMMERCIAL ITEMS
BASIS FOR CONTRACT AWARD. This is a competitive selection in which competing offerors’ present/past performance history will be evaluated on a basis significantly more important than price. By submission of its offer in accordance with the instructions provided in clause FAR 52.212-1, Instructions To Offerors, the offeror accedes to the terms of this model contract and all such offers shall be treated equally. The tradeoff will only occur between the prices and performance records as stated in the below evaluation process:
A. Price Evaluation. Initially, the government shall rank all offers by price, including any option prices. An offeror’s proposed prices will be determined by multiplying the quantities identified in Pricing Schedule by the proposed unit price for each Contract Line Item Number or Subcontract Line Item Number to confirm the extended amount for each. FAR Clause 52.217-8 Option To Extend Services will be evaluated by adding 50% of the 4th option period estimated price to the total estimated contract price to establish a total evaluated price.
B. Performance Confidence Assessment. The contracting officer shall seek recent and relevant present/past performance information on offerors based on (1) the present/past efforts provided by the offeror and (2) data independently obtained from other government and commercial sources.
The government reserves the right to seek information on higher priced offerors if none of the lower priced offerors receive a “Substantial Confidence” performance assessment rating. The purpose of the present/past performance evaluation is to assess the degree of confidence the government has in the offeror’s ability to provide EOS Logistics Support as defined in the PWS and the solicitation that meet users’ needs, based on the offeror’s demonstrated record of performance. The assessment process will result in an overall performance confidence assessment rating of Substantial Confidence, Satisfactory Confidence, Neutral Confidence, Limited Confidence and No Confidence as defined in DoD Source Selection Procedures, Table 5, Performance Confidence Assessments. Present/past performance regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement will be considered as highly as present/past performance information for the principal offeror. Offerors with no recent/relevant performance record or the offeror’s performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned will receive a rating of “Neutral Confidence” meaning the offeror may not be evaluated favorably or unfavorably on the factor of past performance.
Performance Confidence Assesments Adjectival Rating Description
Substantial Confidence Based on the offeror’s recent/relevant performance record, the government has a high expectation that the offeror will successfully perform the required effort.
Satisfactory Confidence Based on the offeror’s recent/relevant performance record, the government has a reasonable expectation that the offeror will successfully perform the required effort.
Neutral Confidence
No recent/relevant performance record is available or the offeror’s performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned. The offeror may not be evaluated favorably or unfavorably on the factor of past performance.
Limited Confidence Based on the offeror’s recent/relevant performance record, the government has a low expectation that the offeror will successfully perform the required effort.
http://www.acq.osd.mil/dpap/policy/policyvault/USA007183-10-DPAP.pdf
No Confidence Based on the offeror’s recent/relevant performance record, the government has no expectation that the offeror will be able to successfully perform the required effort.
D. In evaluating present/past performance, the government reserves the right to give greater consideration to information on those contracts deemed most relevant to the effort described in this solicitation. The government will use the following relevancy definitions as defined in DoD Source Selection Procedures, Table 4, Past Performance Relevancy Ratings, when assessing recent, relevant contracts.
Past Performance Relevancy Ratings Adjectival Rating Description
Very Relevant Present/past performance effort involved essentially the same scope and magnitude of effort and complexities this solicitation requires.
Relevant Present/past performance effort involved similar scope and magnitude of effort and complexities this solicitation requires.
Somewhat Relevant Present/past performance effort involved some of the scope and magnitude of effort and complexities this solicitation requires.
Not Relevant Present/past performance effort involved little or none of the scope and magnitude of effort and complexities this solicitation requires.
E. If the lowest priced offer is judged to have a “Substantial Confidence” performance confidence assessment and the offeror is determined to be responsible, that offer represents the best value for the government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration of any other offers.
F. If the lowest priced offeror is not judged to have a “Substantial Confidence” performance confidence assessment, the next lowest priced offeror will be evaluated and the process will continue (in order by price) until an offeror is judged to have a “Substantial Confidence” performance confidence assessment or until all offerors are evaluated.
The SSA shall then make an integrated assessment best value award decision.
G. Offerors are cautioned to submit sufficient information and in the format specified in the proposal preparation instructions to permit a meaningful assessment of present/past performance. Offerors may be asked to clarify certain aspects of their proposal or respond to adverse present/past performance information to which the offeror has not previously had an opportunity to respond. Adverse present/past performance is defined as present/past performance information that supports a less than satisfactory rating on any evaluation element or any unfavorable comments received from sources without a formal rating system. Communication conducted to resolve minor or clerical errors will not constitute discussions and the contracting officer reserves the right to award a contract without the opportunity for proposal revision.
H. If, during the evaluation period, it is determined to be in the best interest of the government to hold discussions, offeror responses to evaluation notices (ENs) and the final proposal revision (FPR) will be considered in making the source selection decision. If the offeror’s proposal has been evaluated as acceptable at the time discussions are closed, any changes or exceptions in the final proposal revision are subject to evaluation and may introduce risk that the offeror’s proposal be determined unacceptable and ineligible for award.
The following were previously included by reference and are now included by full text:
52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days prior to end of current performance year.
