About this file
This document contains supplemental clauses for a Department of the Air Force contract acquisition.
The Ombudsman clause establishes an independent ombudsman position to hear and facilitate resolution of concerns from offerors and potential offerors. Interested parties must first address concerns directly to the contracting officer before consulting the ombudsman. The designated ombudsman is Mr. Robert Surrey at AFICC OI AMC, Scott AFB, IL, reachable at 618-229-0267, fax 618-256-5724, or robert.surrey.1@us.af.mil. Unresolved concerns may be escalated to the Department of the Air Force ombudsman at SAF/AQC, Washington DC, (571) 256-2395. The ombudsman maintains strict confidentiality, does not participate in proposal evaluation or source selection, and has no binding authority. Contractors should not contact the ombudsman for solicitation copies, due date verification, or technical clarifications—these inquiries must go to the contracting officer.
The Elimination of Class I Ozone Depleting Substances clause prohibits contractors from providing services, products, or specifications requiring Class I ODS in testing, operation, or maintenance of any system or component. Prohibited substances include Halons (1011, 1202, 1211, 1301, 2402), Chlorofluorocarbons (CFC-11 through CFC-217 and blends R-500 through R-503), Carbon Tetrachloride, Methyl Chloroform, and Methyl Bromide per AFI 32-7086.
The Contractor Access to Department of the Air Force Installations clause requires contractors to obtain base identification and vehicle passes for personnel making frequent visits to Air Force installations. Contractors must submit written requests on company letterhead to the contracting officer listing contract number, work site location, start/stop dates, and employee names. Authorized contractor personnel must provide valid driver's license, current vehicle registration, and vehicle insurance documentation. Contractors are responsible for obtaining identification for new personnel and promptly returning credentials upon separation. Personnel whose normal place of duty is on DoD installations must provide after-hours contact information for the Emergency Mass Notification System. Failure to comply may result in withholding final payment.

Supplemental Clauses
Ombudsman
1. An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern.
2. Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-
76 competition performance decisions).
3. If resolution cannot be made by the contracting officer, the interested party may contact the ombudsman, Mr. Robert Surrey, AFICC Ol AMC, 510 POW/MIA, Scott
AFB, IL 62225-5022, 618-229-0267, fax 618-256-5724, email robert.surrey.1@us.af.mil. Secretary (ADAS) (Contracting), SAF/AQC, 1060 Air Force Pentagon, Washington DC
20330-1060, phone number (571) 256-2395, facsimile number (571) 256-2431.
4. The ombudsman has no authority to render a decision that binds the agency.
5. Do not contact the ombudsman to request copies of the solicitation, verify offer due date, or clarify technical requirements. Such inquiries shall be directed to the contracting officer.
