About this file
This is a Justification and Approval (J&A) Memorandum for Record documenting an only-one-source procurement determination for Raumedic Intracranial Pressure (ICP) monitors under simplified acquisition procedures.
The Department of the Air Force, 763d Enterprise Sourcing Squadron at Scott Air Force Base, seeks to procure three (3) Raumedic ICP monitors with supplemental stands and cables valued at $109,173.00 for Headquarters Air Mobility Command/Surgeon General's Office. The equipment will support the Aeromedical Evacuation mission of the Critical Care Air Transport Team (CCATT) to continuously monitor patients with Traumatic Brain Injuries (TBIs) and other neurological emergencies for intracranial pressure, intracranial temperature, and oxygen partial pressure simultaneously. The justification for single-source procurement rests on three primary factors: (1) Compatibility—the Raumedic ICP monitor is the only system compatible with implantable catheters currently used by Military Health System neurosurgeons, and alternatives would create gaps in patient care; (2) Aeromedical Environment—the system is specifically designed to mitigate in-flight challenges including vibration, noise, altitude and pressure fluctuations, and aircraft space and power constraints; and (3) Multimodal Neuromonitoring—the Raumedic system uniquely provides simultaneous measurement of brain tissue oxygen (PbtO2) and temperature in addition to standard ICP monitoring. Market research confirmed Raumedic as the only source meeting minimum government requirements. The Contracting Officer, Emily Fishbein, USAF, certified the circumstances justify only-one-source procurement with pricing to be determined fair and reasonable through historical price comparison and independent cost estimate.

MEMORANDUM FOR RECORD (MFR)
SUBJECT: FA445226Q0017 Raumedic Intracranial Pressure (ICP) Monitors, Only One Source Justification and Approval (J&A) MFR
ACQUISITION VALUE: $109,173.00
1. PURPOSE AND AUTHORITY:
This memorandum documents the justification to procure three (3) ICP monitors as an Only One Source requirement under the Simplified Acquisition Threshold (SAT). In accordance with RFO FAR 12.201-1, this document outlines the specific mission circumstances deeming only one source reasonably available to fulfill the operational requirement.
2. DESCRIPTION OF REQUIREMENT:
Headquarters Air Mobility Command/Surgeon General’s Office (HQ AMC/SGXL) intends to purchase three (3) ICP monitors along with supplemental stands and cables to support the Aeromedical Evacuation of patients with injuries utilized by the Critical Care Air Transport Team (CCATT). This specialized equipment allows neurological medical personnel to continuously monitor patients with Traumatic Brain Injuries (TBI's) or other neurological emergencies for Intracranial Pressure, Intracranial Temperature, and Oxygen Partial Pressure simultaneously. The Raumedic ICP monitors are strategically designed to mitigate the nuances associated with the in-flight care, such as noise and vibrations, altitude and pressure fluctuations during ascent and descent, and power and space constraints.
3. JUSTIFICATION FOR ONLY ONE SOURCE:
Raumedic ICP monitors is the only product compatible with the current implantable catheters the Military Health System (MHS) is already using on patients with Traumatic Brain Injuries or other neurological emergencies. The Critical Care Air Transport Team (CCATT) operates in a highly demanding and resource-limited environment, requiring medical equipment that is optimized for rapid deployment and ease of use in the air. The Raumedic ICP monitors allow for continuous monitoring of multimodal abilities, critical when it comes to patients with TBI’s or other neurological emergencies. The Raumedic ICP monitors have been specifically designed to mitigate in-flight challenges due to vibrations, noise, altitude and pressure changes, and other constraints associated with the aeromedical environment. The justification for restricting consideration to this specific source is based on the following factors:
Compatibility: The Raumedic ICP is the only monitoring system compatible with implantable catheters currently utilized by neurosurgeons in the MHS. Procuring an alternative monitor would result in a severe gap in care, bringing forth detrimental results for patient treatment and monitoring.
CUI
Aeromedical Environment: The Raumedic ICP monitor has been specifically designed to mitigate the challenging environment with air patient transport. The system addresses critical in-flight variables, including constant vibration and noise that can interfere with sensitive electronics; altitude and pressure fluctuations during accent and descent; and the limited space and power constraints of an aircraft cabin.
Multimodal Neuromonitoring: In addition to standard ICP, the Raumedic system offers multimodal neuromonitoring capabilities, including the measurement of brain tissue oxygen (PbtO2) and temperature. Monitoring these multiple parameters simultaneously provides a comprehensive picture of the patient's condition, allowing for earlier detection of complications and individualized treatment.
4. MARKET RESEARCH:
Market research revealed Raumedic is the only source which can satisfy the Government’s need for the ICP monitoring system. While other manufacturers produce similar devices, none possess the required compatibility with the implantable catheters the MHS Neurology department are currently using. Therefore, only Raumedic monitoring system can meet the Government's minimum needs.
5. CONTRACTING OFFICER CERTIFICATION:
I determine the circumstances of this contract action deem only one source reasonably available.
The anticipated cost to the Government will be determined fair and reasonable based on the comparison of historical prices paid and the Independent Cost Estimate.
