About this file

This is a Past Performance Questionnaire (PPQ) issued by the 60th Contracting Squadron at Travis Air Force Base, California, for evaluating contractor performance on washer and dryer maintenance and repair services.

The document requests that customers of bidding contractors complete the questionnaire to provide performance ratings that will be used in source selection. Respondents must submit completed questionnaires to Ms. Amelia Dighiera (Contract Specialist) at amelia.estrada_dighiera@us.af.mil or Ms. Christina Johnson (Contracting Officer) at christina.johnson.32@us.af.mil by September 15, 2026 at 12:00 PM PST. The questionnaire requires three sections of information: offeror's company details and point of contact, respondent's company/business/military base information, and contract details including contract name/number, description, performance period, and contract values. The performance evaluation section uses a 1-5 scale rating system addressing quality of service, on-time delivery, management of service, communication, contract terms and condition adherence, and overall performance. Ratings of 1 (Unsatisfactory), 2 (Marginal), or 5 (Exceptional) require narrative explanations. The questionnaire also asks whether the contractor received any Corrective Actions Reports (CAR) and whether the respondent would award another contract to the contractor. Completed questionnaires become part of the official source selection records.
