About this file
This is a Past Performance Questionnaire (PPQ) template used by the 60th Contracting Squadron at Travis Air Force Base to evaluate contractor performance on similar projects. The questionnaire requests references from recent customers of companies bidding on the Building 177 Access Control System Replacement solicitation (FA442726Q1125), which requires a brand name LENEL system. Respondents are asked to provide detailed information about their company or organization, identify the contractor being evaluated, and describe relevant contract work performed, including contract name/number, description, performance period, and contract values for both prime and subcontractor roles.
The performance evaluation section uses a 1-5 scale rating system covering six key areas: quality of service, on-time delivery, management of service, communication, contract terms and conditions adherence, and overall contractor performance. Ratings are defined as Unsatisfactory (1), Marginal (2), Satisfactory (3), Very Good (4), or Exceptional (5), with narrative explanations required for ratings of 1, 2, or 5. The questionnaire also asks whether the contractor received Corrective Action Reports or similar deficiency notices and whether the respondent would award future contracts to the contractor. Completed questionnaires must be submitted as PDF files to the identified points of contact for the Air Force solicitation.
