MEMORANDUM FOR PROSPECTIVE EVALUATORS
FROM: 628th Contracting Squadron 101 E. Hill Blvd.
Joint Base Charleston, SC 29404 SUBJECT: Past Performance Questionnaire
1. The 628th Contracting Squadron, Joint Base Charleston, SC is in the process of selecting a contractor for Facility Maintenance services. Here, Facility Maintenance services means Preventative Maintenance and Inspections (PMI), Facility Repairs and Equipment Repairs. Facility Maintenance actions are expected to include Industrial HVAC PMI/Repair (650-800T Trane Chillers, DDC Controls); Food Service Equipment (i.e. garbage grinders, combination ovens, sculleries, tray conveyors, steam kettles, etc.); Hot Water Boilers, Steam Boilers, including condensate, hot water and steam lines; Electronic Locks; Roofs (i.e. roof leaks); 24/7/365 On-Site Response Capability; and General electrical, mechanical, and plumbing maintenance.
2. One of the considerations in proposal evaluation is the verification of the Offeror’s past and present performance on contracts which reflect the Offeror's ability to perform on the proposed effort. We depend on information received from agencies such as yours, which have had firsthand experience with an Offeror, for the evaluation of the Offeror's performance on those contracts.
3. Our areas of interest in the Offeror are summarized in the enclosed questionnaire. Please complete the questionnaire by indicating your rating and providing comments wherever applicable. For any ratings above or below satisfactory, please provide an explanation on how the contractor either exceeded your requirement or how the contractor failed to meet your requirement and attach it to your response. This can be a copy of a performance evaluation, discrepancy report, or other documentation. Once complete, please sign the document in the space provided.
4. Prospective offerors may be contacted regarding problem areas or concerns identified to see what corrective actions have been taken or will be taken. However, the individual's name completing the questionnaire will not be disclosed to the Offeror.
5. Your time and effort in providing this information is greatly appreciated and is critical to our evaluation. Please return the completed questionnaire as soon as possible but NLT COB on 3 January 2019 to Mr. Jonathan Hirsch at jonathan.hirsch@us.af.mil.

D. PERFORMANCE INFORMATION
The Air Force will use your survey responses to determine a level of confidence, based on the offeror’s past performance record, concerning the Offeror’s ability to successfully perform our requirement in accordance with the contract. Please assign the following performance level codes to each of the questioned areas listed below based on the contractor’s performance at your organization or installation. The confidence codes selected will indicate the degree of doubt concerning performance in each area.
RESPONDENT WILL NOT FURNISH ANY PAST PERFORMANCE INFORMATION TO OFFEROR SUBMITTING A PROPOSAL FOR THIS SOLICITATION.
Please indicate your satisfaction with the contractor’s performance by placing an “X” in the appropriate block using the scale provided to the right of each question. This scale is defined as follows:

S SATISFACTORY – The contractor’s performance meets contractual requirements. The contractual performance contained some minor problems for which corrective actions taken by the contractor appear or was satisfactory.
M MARGINAL – Performance does not meet some contractual requirements. The contractual performance reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor’s proposed actions appear only marginally effective or were not fully implemented.
UN
UNSATISFACTORY – Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance contains serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
