PERFORMANCE WORK STATEMENT
FOR
SPAWAR FACILITIES MAINTENANCE SERVICES
1. DESCRIPTION OF SERVICE. The Contractor shall perform Preventive Maintenance and
Repair Services for SPAWAR Facilities located on Joint Base Charleston – Weapons Station, South Carolina (see Appendix A and Appendix B) in accordance with all applicable laws, regulations, standards, instructions and Industry Best Practices as described in the Performance
Work Statement (PWS) with the focus on safety and customer service. As a result of the 2005
Base Realignment and Closure (BRAC) Commission the Charleston Naval Weapons Station hereto in is referred to as Joint Base Charleston Weapons Station (JB CHS-Weapons Station).
The Contractor is encouraged and expected to use innovative approaches to efficiently and effectively accomplish PWS requirements in a timely manner.
The work shall be performed at various locations and will vary from location to location. The following are examples of the dispersion of work at the various locations, but not limited to:
(1) Water Chillers/Air Conditioning Units
(2) Air Handling Systems
(3) Pumps
(4) Electric Motors and Controllers
(5) Chilled Water Piping Systems
(6) Chilled Water System Chemical Treatment, including Chemicals
(7) Adjustable Frequency Drives
(8) Computer Room Air Conditioning Units
(9) Mechanical Thermal Insulation
(10) HVAC Control System, including repair and maintenance of 3 different types of DDC systems (Johnson Controls, Automated Logic, and T.A.C)
(11) Steam/Hot Water Boilers
(12) Dehumidifiers
(13) Steam Humidifiers
(14) Exhaust Fans
(15) Variable Air Volume Terminals
(16) Plumbing from the backflow preventer to the building exterior sewage lines to include
- Exterior: water supply lines, spigots, cut off valves, sewage lines, etc.
- Interior: piping, valves, fixtures, sinks, water closets, water fountains, ice machines, supplies and drains to HVAC, roof drains, etc.
(17) Electrical Low Voltage: Includes issues from the entrance cable to the point of use
(including the wires, switches, breakers, fixtures, lamps, motors, appliances, etc.)
(18) Corrosion control applications
(19) Ice thermal storage system (4000 ton-hr, 60,000 gal glycol-ice)
(20) Operation of Direct Digital Controls (DDC) maintenance, programming and repairs
(21) Service calls (Emergency Service calls, Urgent Service calls, and Routine Service calls)
(22) Any building and structure problems {including doors and door hardware, roof leaks, wall leaks, window repairs, lock repair or replacement, floor repair (VCT, sheet vinyl, carpet, Page 3 of 157 Attachment 1 FA4418-12-R-0003 ceramic tile, concrete, etc.), wall repairs (sheet rock, plaster, wall paper, ceramic tile, brick, concrete block, etc.), small painting projects in conjunction with other repairs, security sliding gates and arms}
(23) Irrigation system maintenance and repair
(24) Exterior lighting to include any lights attached to the buildings and parking lot lights
(street lights not included)
(25) Signage both inside and outside of the buildings, for example, temporary street signs for events, no parking, designated parking spaces IAW UFC 3-120-01: Air Force Sign Standard.
(26) Escort Services for vendors and outside Contractors. This shall be accomplished by service call.
(27) Roll-up Door services
(28) Backflow Preventers
(29) Generator Service Support
NOTE: CONSTRUCTION SHALL NOT BE DONE USING THIS CONTRACT
For clarification a list of Definitions and Acronyms has been included in Appendix C
1.1. SPECIFIC FUNCTIONS. The Contractor shall perform preventive maintenance (PM) and inspections to ensure reliable and continuous safe operation of all facilities, systems and equipment covered under this PWS. The Contractor shall submit the PM schedule to the government Contracting Officer’s Representative (COR) within 3 days after the contract performance period start and shall keep it current. The Contractor shall obtain the manufacturer’s written recommendations, instructions, specifications, and standards. All service and maintenance work performed by the Contractor shall conform to the applicable latest editions of the manufacturer’s written recommendations, instructions, specifications, and standards. Upon completion of maintenance and inspections the Contractor shall provide inspection reports as stated in Para 1.5.1.
1.1.1. GENERAL BUILDING MAINTENANCE SERVICES. The Contractor shall prepare and maintain a PM schedule for all facilities covered under this PWS. All service and maintenance work performed by the Contractor shall conform to the applicable latest editions of the following:

1.1.1.1. PREVENTIVE MAINTENANCE AND INSPECTIONS OF LARGE HVAC
SYSTEMS. The Contractor shall inspect and perform PM IAW manufacturer’s written instructions (verbatim compliance required), Appendix E ADDITIONAL PM
REQUIREMENTS, and industry best practices for equipment listed in Appendix D
EQUIPMENT INVENTORY. PM includes HVAC water treatment IAW Appendix K.

REQUIREMENT LISTING FOR SPAWAR FACILITIES
1.1.1.2. PREVENTIVE MAINTENANCE AND INSPECTIONS OF SMALLER HVAC
SYSTEMS. The Contractor shall inspect and perform PM IAW manufacturer’s written instructions (verbatim compliance required), Appendix E ADDITIONAL PM
REQUIREMENTS, and industry best practices for systems in Appendix G HVAC Condenser inventory.
1.1.2. DIESEL ENGINE MAINTENANCE SERVICES. The Contractor shall perform PM and inspections to ensure reliable and continuous safe operation of all diesel engines covered under this PWS (see Appendix H for inventory of diesel engines, generators, UPS, transfer switches, as well as Emergency Generator Engine Exerciser schedules, Monthly
Diesel/Generator Preventive Maintenance checks and logs, Diesel Engine Service, Generator
Service, Load Bank Testing, Transfer Switch Service, and other needed guides).
1.1.2.1. FUEL TESTING, CERTIFICATION, AND TANK CLEANING NOTIFICATION.
The Contractor shall perform annually: fuel testing using ASTM clear and bright test, chemically treat fuel with appropriate inhibitors, tag and label the tank noting the date and description of service, certify fuel quality as passing or failing, and notify COR if tank cleaning is required.
The Contractor shall provide a copy of all test results to the COR.
1.1.3. GENERATOR MAINTENANCE SERVICES. The Contractor shall perform PM and inspections to ensure reliable and continuous safe operation of all facility generators covered under this PWS to include topping off of all fluids (see Appendix H).
1.1.4. UPS (UNINTERRUPTIBLE POWER SYSTEM) MAINTENANCE SERVICES.
The Contractor shall perform PM and inspections to ensure reliable and continuous safe operation of all facility UPS systems covered under this PWS (see Appendix H).
1.1.5. TRANSFER SWITCH MAINTENANCE SERVICES. The Contractor shall perform
PM and inspections to ensure reliable and continuous safe operation of all facility transfer switches covered under this PWS (see Appendix H).
1.1.6. LARGE BREAKER MAINTENANCE SERVICES. The Contractor shall perform PM and inspections to ensure reliable and continuous safe operation of all facility electrical systems covered under this PWS (see Appendix H).
1.1.7. OVERHEAD AND ROLLING DOORS MAINTENANCE SERVICES. The
Contractor shall perform PM and inspections to ensure reliable and continuous safe operation of all facility Overhead or Rolling Door systems covered under this PWS (see Appendix N).
1.1.8. BOILER MAINTENANCE SERVICES. The Contractor shall perform PM and inspections to ensure reliable and continuous safe operation of all facility boilers covered under
Page 5 of 157 Attachment 1 FA4418-12-R-0003 this PWS (see Appendix J). This includes seasonal start-up and shut down as well as water testing and treatment.
1.2. MINOR BUILDING / EQUIPMENT REPAIRS AND SERVICE CALLS.
1.2.1. SERVICE CALLS. Service calls are defined as maintenance, repair, alteration or other miscellaneous work requirements which are considered above and beyond PM and inspection.
Service calls may be generated by SPAWAR work reception center, the COR, or by the
Contractor as a result of PM or inspections. Service calls at or below the standard limitation of liability of $2,500 (see Appendix I LIMITATIONS OF LIABILITY for exceptions) shall be accomplished by the Contractor at no additional cost to the Government. For service calls that exceed the limitation of liability, the Contractor shall submit a written quote IAW section 1.5.2.
to the COR and CO. Upon receipt of the quote, the CO shall approve or disapprove the work.
No work above the limitation of liability shall be initiated without CO approval. If the CO approves the work, the Government will be responsible for the amount in excess of the limitation of liability. If the Contractor performs work above the limitation of liability without prior CO approval, the Contractor shall be liable for costs incurred. Maintenance and repair requirements received for the same trade in the same building or structure at the same time will be combined into one service call.
Example: If a plumbing repair is projected to be $3,000, the Contractor shall provide a written fixed-price quote as stated in paragraph 1.5.2. The Contractor shall provide the repair quote to the Contracting Officer (CO) or their representative. The CO or their representative will notify the Contractor if and when the repair will be done. The
Contractor would be paid the $500 above the $2,500 threshold by submitting an invoice against the repair CLIN.
1.2.1.1 SERVICE CALL RECEPTION
1.2.1.1.1 Normal Working Hours. The Government's work reception center will receive service call requests during normal working hours and classify each call in accordance with the definitions provided below. A description of the problem or requested work, date and time received, location, and other appropriate information will be placed on a Service Call Work
Authorization Form and made available for pickup by the Contractor at the Government's work reception center. If the call is classified as emergency or urgent the Government's work receptionist will notify the Contractor by phone or radio that a call has been received and that a work authorization form is available for pickup.
1.2.1.1.2 AFTER NORMAL WORKING HOURS. The Contractor shall receive all service call requests directly from the work reception center or COR after normal working hours, on weekends, and holidays. Calls will be received and classified by the Contractor as emergency, urgent, or routine in accordance with the definitions provided in the "Service Call Classification" paragraph of this PWS, and responded to accordingly. If the call is classified as emergency or urgent, the Contractor shall fill out a Service Call Work Authorization Form, including order number, description of the problem, date and time received, building number, and caller's name and telephone number. If the call is classified as routine the Contractor shall record the same
Page 6 of 157 Attachment 1 FA4418-12-R-0003 information, and will fill out a work authorization form. One copy of each emergency and urgent work authorization form and a log of all routine calls received shall be delivered to the
Government's work reception center by 0900 the next regular working day. Authorized
Government representatives may upgrade or downgrade the classification of any service call received by the Contractor.
1.2.1.2 SERVICE CALL CLASSIFICATION
1.2.1.2.1 EMERGENCY CALLS. Service calls will be classified as emergency at the discretion of the Government Work Reception Center. Generally, emergency calls will consist of correcting failures, which constitute an immediate danger to personnel, threaten to damage property, threaten to disrupt the mission, or affect the operation of critical equipment or systems.
An estimated 5% of the service calls issued to the Contractor will be classified as emergency.
(Approximately 500/year.)
1.2.1.2.2 URGENT CALLS. Service calls will be classified as urgent at the discretion of the
Government Work Reception Center. Generally, urgent calls will consist of providing services or correcting failures which do not immediately threaten personnel, property, or activity missions; but which would soon inconvenience and/or affect the health or well-being of personnel, lead to property damage, or lead to disruptions in the mission. Calls will also be classified as urgent when the service or failure has upper level command/management attention.
An estimated 20% of the service calls issued to the Contractor will be classified as urgent.
(Approximately 2,000/year.)
1.2.1.2.3 ROUTINE CALLS. Service calls will be classified as routine calls when the work does not qualify as an emergency or urgent call. (Approximately 6,500/year.)
1.2.1.3 RESPONSE TO SERVICE CALLS. The Contractor shall have adequate procedures for picking up service call work authorizations from the Government's work reception center during normal working hours, and for receiving and responding to emergency and urgent service calls 24 hours per day, including weekends and during holidays. A single local point of contact shall be provided by the Contractor for receipt of all service calls. Emergency and urgent calls shall be considered received by the Contractor at the time and date the call is placed by the work reception center or COR. Routine calls shall be considered as received by the Contractor at the time and date the work reception center makes the work authorization available for pick up.
1.2.1.3.1 RESPONSE BY CLASSIFICATION
1.2.1.3.1.1 EMERGENCY CALLS. The Contractor shall respond immediately and must be on the job site and working within 2 hours after receipt of an emergency service call. The
Contractor shall work continuously without interruption and shall arrest the emergency condition before departing the job site (e.g. return critical system to operation, secure power to eliminate electrical hazard, etc.). If the estimated cost to arrest or downgrade the situation will go above the limitation of liability, the Contractor shall provide a written quote to the COR and CO within
4 hours before proceeding with work above the limitation of liability. If further labor and material are required to complete the repair, the call will be reclassified as either urgent or
Page 7 of 157 Attachment 1 FA4418-12-R-0003 routine, as appropriate, and the corresponding completion time will then apply. Notification of reclassification shall be reported to the Work Reception Center or COR by 0900 the next business day. Such follow-up work shall be considered part of the original service call.
1.2.1.3.1.2 URGENT CALLS. The Contractor shall be on the job site and working within 24 hours after receipt of an urgent service call received during normal working hours, and within 24 hours for urgent calls received after normal working hours, on weekends, or holidays. Once begun, the work shall be prosecuted to completion and must be completed within 72 hours. If the estimated cost to correct or downgrade the situation will go above the limitation of liability, the
Contractor shall provide a written quote to the COR and CO within 24 hours before proceeding with work above the limitation of liability. If further labor and material are required to complete the repair, the call will be reclassified routine, as appropriate, and the corresponding completion time will then apply. Notification of reclassification shall be reported to the Work Reception
Center or COR by 0900 the next business day. Such follow-up work shall be considered part of the original service call.
1.2.1.3.1.3 ROUTINE CALLS. Work for routine calls must be initiated within 3 work days after receipt and completed within 7 work days. Routine calls shall normally be accomplished during normal work hours, Monday through Friday.
1.2.1.3.5 COMPLETED CALLS. Within one working day after completion of each service call, the Contractor shall add the following information to the work authorization form and return to the work reception center:
Description of work actually completed
Brief description of material and parts used, including quantities and material cost
Date and time work began
Date and time work was completed
Hours of labor (by craft) expended
Signature or initials of the Contractor's craftsman performing the work (or supervisor), indicating that the work has been completed
1.2.2. WARRANTY ON SERVICE CALL REPAIRS. The Contractor will warranty repairs for the length of time consistent with the equipment manufacturer’s recommended commercial standard warranty to include parts and labor.
1.3 INSPECTION, TESTING, AND CERTIFICATION. The Contractor shall provide inspection, testing, and certification services to ensure equipment is maintained in a safe and fully operational condition. This shall include an Integrated Maintenance Program (IMP). IMP is a recurring state-of-the-art, reliability-centered inspection, testing, maintenance and repair program that determines best practices for managing the functions and consequences of failures of facilities equipment and system components. IMP encompasses accepted commercial practices, including reactive, preventive, predictive and proactive maintenance, into one optimal program. The IMP approach gives the Contractor full responsibility to maintain systems and equipment and perform repairs whenever necessary to ensure equipment and systems are
Page 8 of 157 Attachment 1 FA4418-12-R-0003 operational and remain in a constant state of readiness. Service calls will be issued for accomplishment of repairs on systems and equipment maintained under IMP.
1.3.1 FACILITY INSPECTION All facilities in Appendix A Building Inventory as well as all exterior systems such as irrigation system and exterior lighting shall be inspected to ensure equipment is maintained in a safe and fully operational condition at least quarterly. These quarterly inspections shall include all disciplines, such as structural, electrical, and mechanical.
Structural inspections shall include discovering such items as broken windows, leaves on the roof, mildew on the exterior of the building, locks needing servicing, broken skirting around structures, etc. Electrical inspections shall include such items as light lamps burned out, inoperative light sensors, switches that don’t operate correctly, emergency and exit lights (see
Appendix O), parking lot lights, etc. Mechanical inspections shall include such items as heating and A/C operation, exhaust fans, pumps, etc. All shall be done to Industries Best Practices and to provide IMP.
1.3.2 BOILER INSPECTION, TESTING, AND CERTIFICATION. The Contractor shall annually clean, prepare, and operate boilers and UPVs (Unfired Pressure Vessels) to support certification.
The boiler inventory and date of last certification is provided in Appendix J.
The Contractor shall prepare boilers and UPV for inspection per the National Board of Boiler and Pressure Vessel Inspectors Code and UFC 3-430-07, Operations and Maintenance:
Inspection and Certification of Boilers and Unfired Pressure Vessels. Boiler inspection safety certificates shall be void immediately on the discovery of a safety deficiency regardless of the expiration date on the certificate. The certificate will again be valid only after the deficiency has been corrected by the Contractor and the boiler has been re-certified.
The Contractor shall provide a qualified Boiler Inspector for certification of boilers and UPVs.
Chapter 2 of UFC 3-430-07, Operations and Maintenance: Inspection and Certification of
Boilers and Unfired Pressure Vessels, specifies the qualification, certification, and licensing requirements for all Contractor employees that perform inspections, witness tests, prepare reports, and issue certificates.
A current and valid certificate, or authorized commercial equivalent, must be posted on, or near, the equipment, under a clear protective covering as specified in Chapter 9 of UFC 3-430-07, Operations and Maintenance: Inspection and Certification of Boilers and Unfired Pressure
Vessels.
Repairs identified during PM, inspection, testing, and certification shall be accomplished as a service call.
1.3.3 INTERIOR AND EXTERIOR LIGHTING SYSTEMS The Contractor shall develop and implement a program to inspect, re-lamp, and make all necessary repairs to interior and exterior lighting systems to ensure trouble-free lighting at least quarterly. The Contractor shall develop and implement a program to inspect/test, re-lamp and make all necessary repairs to
Page 9 of 157 Attachment 1 FA4418-12-R-0003 emergency lighting systems at least annually. These plans shall be submitted at the pre-performance meeting.
1.3.4 ANNUAL TESTING/INSPECTION/REPAIR OF SPAWAR FACILITY
BACKFLOW PREVENTERS. The Contractor shall develop and implement a program to perform Annual Testing/Inspection/Repair of SPAWAR Facility Backflow Preventers, and make all necessary repairs to SPAWAR Facility Backflow Preventers systems to ensure trouble-free operation and state certification annually. The plan shall be submitted at the pre-performance meeting and shall include the firm doing the required certification. The SPAWAR Facility
Backflow Preventers inventory is provided in Appendix R SPAWAR BACKFLOW
PREVENTERS.
1.4. MANAGEMENT. The Contractor shall manage the total work effort associated with the services required herein to meet the performance objectives and standards. Such management includes, but is not limited, to planning, scheduling, cost accounting, report preparation, establishing and maintaining records, and quality control. The Contractor shall continuously maintain an adequate staff with the necessary management expertise to assure performance objectives and standards are met.
1.4.1. WORK CONTROL. Verbal scheduling and status reports shall be provided when requested by the COR or CO. A written status report of any item of work must be provided within four hours of the inquiry during regular working hours, and 24 hours following work day for inquiries after regular working hours.
1.4.2. WORK SCHEDULE. The Contractor's work shall not interfere with normal
Government business. In those cases where some interference is unavoidable, the Contractor shall minimize the impact and effects of the interference. The Contractor shall provide advance access of all of its work schedules to the Government. The Contractor shall notify the COR of any difficulty in scheduling work due to Government controls.
1.4.2.1 NOTICE OF EQUIPMENT SHUT-DOWNS. Prior approval shall be obtained from
COR, except in emergencies, for work requiring shut-down of equipment for more than 30 minutes during regular work hours. All such requests must be submitted at least 72 hours in advance. In cases where shutdown is necessary, the Contractor shall coordinate the shut-down with the Building Custodian or other designated representative in that affected building.
1.4.3. SERVICE INTERRUPTIONS. If any utilities or other services must be discontinued
(temporary or otherwise) due to scheduled contract work, the Contractor shall notify the COR, affected tenants, and customers within 7 calendar days. If the discontinued service is due to an emergency breakdown the Contractor shall notify the COR, affected tenants and customers as soon as practicable.
1.4.4. CONTRACTOR'S COMPUTERIZED MAINTENANCE MANAGEMENT
SYSTEMS (CMMS). The Contractor shall maintain current and accurate records stored in their
CMMS. The Contractor shall make their system available for the Government’s use in managing the effort required under this contract. The Government will not be allowed to make any
Page 10 of 157 Attachment 1 FA4418-12-R-0003 changes within the Contractor’s system. The Contractor’s CMMS should have the following capabilities:
Organize and track inventory
Manage equipment costs
Track equipment history
Schedule preventive maintenance tasks
Maintain confidential labor records
Allocate resources
Generate work orders
Requisition and purchase parts
Project equipment failures
SPAWAR’s preferable choice for a CMMS is the MP2 Software, or a CMMS that is similar to
MP2.
1.4.5. WARRANTY MANAGEMENT. The Contractor shall report to the COR all defects in workmanship, material, parts, or improper installation by others and found by the Contractor to be covered by a warranty prior to performing repair work. The Contractor is responsible for knowing which equipment and components are covered by the original warranty and the warranty duration. The COR will provide available warranty documents.
1.5. CONTRACTOR-FURNISHED REPORTS. The Contractor shall furnish the following reports to the government.
1.5.1. INSPECTION REPORTS. The Contractor shall prepare and submit a written report within 2 business days after each preventive maintenance inspection to the building custodian and COR. The report shall identify the type of equipment, the location’s building number, maintenance work performed, repairs needed, date of inspection, name of the technician(s) who performed the work, and the overall condition of the equipment. If the needed repairs, annotated in the inspection report, require work considered over and above routine maintenance work, the
Contractor shall provide a written estimate as stated in paragraph 1.5.3.
1.5.2. WRITTEN REPAIR QUOTE. For service calls that exceed the limitation of liability, the Contractor shall submit a written quote to the COR and CO. This written quote shall include the estimated cost of parts and labor, the specified time needed to repair and the projected equipment downtime. See Appendix I LoL (Limits of Liability) above the FFP (Firm Fixed
Price) portion of the contract.
1.5.3. WRITTEN REPAIR REPORT. The Contractor shall prepare and submit a written repair report within two business days after the routine and emergency service call repairs are completed. The report shall include the date and time of the service call, if it was an emergency or a routine service call, the type of equipment, the location’s building number, the repairs performed to include replacement parts and labor breakout, the actual equipment downtime, and the name of the technician performing the repairs.

1.6. ESSENTIAL SERVICES. Emergency service calls as defined in 1.2.1.2.1 and all PM and servicing of equipment identified in Appendix D shall be considered an essential service.
2. SERVICE SUMMARY. The SS (Service Summary) includes Performance Objectives that describe the desired end result or outcome the Contractor shall achieve. The Performance
Thresholds states the minimum acceptable level of the performance in terms of quality, timeliness, and quantity in realistic and achievable standards. These thresholds are critical to mission success. The PWS paragraphs reference the details of the performance objective in the
PWS.

GENERAL INFORMATION
3. GOVERNMENT-FURNISHED PROPERTY, MATERIALS AND SERVICES.
The Government shall provide the facilities, fixtures, equipment, materials and services listed here. Materials, equipment, labor and supplies necessary for the successful performance of this contract not specifically identified shall be provided by the contractor. The Contractor shall maintain all work areas and equipment in a neat, clean, and orderly fashion.
3.1. GOVERNMENT-FURNISHED FACILITIES (GFF). These facilities are representative and may change. The Contractor shall assume responsibility and accountability of such facilities provided for its use and shall take adequate precautions to prevent fire hazards, odors and vermin. The Contractor may not alter facilities without written approval of the CO. At the completion of the contract, all facilities shall be returned to the Government in the same condition as received, except for normal wear and tear and any approved modifications. The
Contractor shall be held responsible for the cost of any repairs caused by negligence or abuse on his part, or on the part of his employees and will not be issued service calls for these repairs. The
Contractor shall act as the Facilities Manager for the GFF.
The Government will provide facilities for office space, warehouse space, trailer space and land for lay down if required. Specifically:
Partial use of T10 office and T10 warehouse and area for an office trailer.
Partial use of 3420 and 3431 warehouse.
3.2. GOVERNMENT-FURNISHED UTILITIES. The Government will furnish the following utility services at existing outlets, as may be required for the work to be performed under the contract:
Electricity and fresh water. Information concerning the location of existing outlets may be obtained from the COR. The Contractor shall provide and maintain, at his/her expense, the necessary service lines from existing Government outlets to the site of work.
Refuse services at existing refuse cans or dumpsters.
Electricity at T10 and 3431
Water and sewer at area for an office trailer.
Utilities specified above will be furnished at no cost to the Contractor.
3.3. GOVERNMENT-FURNISHED MATERIALS (GFM). The Government shall turn over the consumable material and supplies in stock to the Contractor for use in connection with this contract. The GFM will be furnished on a one-time basis and will not be replenished by the
Government.
Any materials necessary for satisfactory performance of the contract above and beyond those issued by the Government shall be the responsibility of the contractor. Ownership of unconsumed materials provided by the Contractor shall remain with the Contractor after contract expiration.

The Contractor and the COR will perform a joint inventory within 15 days of the start of the contract and annually thereafter. The inventory will include equipment condition and the disposition of unserviceable equipment. Unserviceable materials shall be disposed of through
Defense Reutilization/Marketing Office (DRMO). The contractor shall coordinate with COR then Central Supply for DRMO turn-in procedures. Upon completion or termination of the contract, all unconsumed, Government owned material shall be returned to the Government.
Equipment available for the contractor’s use includes:
Pre-expended Bin Materials and Supplies currently in T10 warehouse and 3431 warehouse
3.4. GOVERNMENT-FURNISHED EQUIPMENT (GFE). All items over $250 or highly pilferable items as designated by the CO will be included in this inventory. This equipment is furnished on a one-time basis and will not be replenished.
Any tools or equipment necessary for satisfactory performance of the contract above and beyond those issued by the Government shall be the responsibility of the contractor. Ownership of tools provided by the Contractor shall remain with the Contractor after contract expiration.
The Contractor and the COR will perform a joint inventory within 15 days of the start of the contract and annually thereafter. The inventory will include equipment condition and the disposition of unserviceable equipment. The contractor shall be responsible for the maintenance of equipment used for this PWS. Unserviceable equipment shall be disposed of through Defense

3.5 GOVERNMENT FURNISHED SERVICES. The following is a list of government-provided services. The Contractor shall notify the COR immediately if there is disruption of services or there is a situation that requires a government furnished service (i.e., power outage, pests in building, dumpsters overflowing, fire, or security problem).
PEST CONTROL. The Government shall furnish pest control through service calls.
The contractor shall contact the facility manager in the event of a pest issue.
REFUSE COLLECTION. Refuse collection will be provided at existing dumpsters.
The Government reserves the right to adjust the numbers and locations of dumpsters based on observed utilization and requirements. If the Contractor is required to perform services that create an unusually large requirement for refuse disposal, the Contractor will coordinate this requirement with the COR no less than 10 days in advance, so that special pick-ups can be scheduled.

POLICE AND FIRE SERVICE. The complex currently has a security and fire department. These services will be furnished to the Contractor at no cost. The
Contractor shall be liable for the cost of services that are required due to the negligence of the Contractor.
4. GENERAL INFORMATION
4.1. HOURS OF OPERATION. The base hours of operation are 7:00AM to 5:00PM, Monday through Friday. The Contractor is not required to provide preventive maintenance and repair services for equipment on Federal Holidays. If a scheduled service falls on an observed Federal
Holiday, service will be performed on the next workday, (i.e., if a holiday falls on Monday, service will be provided on Tuesday). Exceptions to the regular hours of operation are detailed in subsequent sections of this PWS. Work in certain annexes or sub-annexes may require
Contractor continuous operations, 24 hours a day, every day of the year including holidays. The performance of other work requirements shall be accomplished within the Government’s regular working hours unless the specific work requirement specified herein necessitates otherwise. Any other work outside Government regular working hours requires a request submittal to COR for approval.

4.2. QUALITY CONTROL. Contractor shall develop and maintain a quality program to ensure services are performed in accordance with this Performance Work Statement and other commonly accepted Industry Best Practices. The Contractor shall develop and implement procedures to identify and prevent defective services from reoccurring. As a minimum, the
Contractor shall develop quality control procedures that address the areas identified in paragraph
2, Service Summary. These procedures need not be submitted during solicitation or award;
however, they must be available upon request by the Government. The Contractor will supply the government with a single point of contact in which to address customer complaints and quality control issues.
4.3. QUALITY ASSURANCE. The government will periodically evaluate the Contractor’s performance in accordance with the Quality Assurance Surveillance Plan. After the Contractor performs inspections, preventive maintenance or repairs, the government reserves the right to contact and have an outside third party “certified equipment specific inspector” to conduct a
Page 17 of 157 Attachment 1 FA4418-12-R-0003 follow-up inspection to ensure 100% compliance verification in accordance with all applicable directives, regulations and AFI’s in the best interest of the government.
4.4. FIRE SAFETY. The Contractor shall obtain a burning permit from the Base Fire
Department prior to welding, burning or cutting operations each day he anticipates performing that type of work. All welding and burning operations necessary for the completion of this contract shall be accomplished in strict compliance with base fire safety standards. A copy of these standards is available from Civil Engineering upon request.
4.5. SAFETY. The Contractor shall comply with all safety regulations and procedures stated above. The Contractor will maintain and provide a copy of a written safety plan to the wing safety office prior to start of work IAW AFPAM 91-210 6.4. Contractors will be required to maintain a mishap record at or below the national average for their industry. Upon request, Contractor will provide OSHA 300 and 301 logs to the safety office for mishaps occurring on the installation. Any mishaps involving damage to USAF property or injury to USAF personnel will be immediately reported to the base safety office. Contractors, whether regularly involved in routine site operations or engaged in temporary projects must follow all federal and state safety standards. In cases where the contactor will work with USAF personnel, USAF safety standards will be followed.
4.6. SECURITY REQUIREMENTS. 4.6 SECURITY REQUIREMENTS. The contractor shall comply with all federal, state, and local security statutes, regulations and requirements.
Upon request, submit the name and address of each employee hired for work on this contract.
Security may request questionnaires and other security related forms be filled out prior to work or access by employees. The Contractor shall become acquainted with and comply with all
Government regulations as posted, or as requested by the CO when required to enter a
Government site.
The Contractor shall not disclose or disseminate information concerning the operations of the activity which would result in or increase the likelihood of the possibility of a breach of the activities security or interrupt the continuity of its operation.
Disclosure of information relating to the services hereunder to any person not entitled to receive it, or failure to safeguard any classified information that may come to the Contractor in connection with work under this contract, may subject the Contractor, to criminal liability under
18 U.S.C., Sections 793 and 798.
Inquiries, comments, or complaints arising from any matter observed, experienced, or learned as a result of, or in connection with, the performance of this contract, the resolution of which may require the dissemination of official information will be directed to the CO.
4.6.1. EMPLOYEE LISTING. The Contractor shall maintain and provide, upon request, a current listing of employees. The list shall include employee’s name, contact number and level of security clearance.

4.6.2. VEHICLES. The company name shall be displayed on each of the Contractor's vehicles in a manner and size that is clearly visible. All vehicles shall display a valid state license plate that complies with State Vehicle Code. Vehicles shall meet all other requirement of the State
Vehicle Code, such as safety standards, and shall carry proof of insurance and state registration, if applicable. Operators shall carry their license with them at all times while operating a vehicle.
4.6.3. PASSES AND BADGES. All Contractor employees shall obtain the required employee and vehicle passes. Prior to the start of the contract, submit to the CO an estimate of the number of personnel expected to be utilized at any one time on the contract. The Government will issue badges without charge. Each employee shall wear the Government issued badge over the front of the outer clothing. When an employee leaves the Contractor's service, the employee's pass and badge shall be returned within 3 calendar days.
4.6.4 ACCESS TO BUILDINGS. It shall be the Contractor’s responsibility, through the CO or
COR, to obtain access to building and facilities and arrange for them to be opened and closed.
Keys may be issued to the Contractor; however, it shall be the Contractor’s responsibility to arrange for security of the buildings and facilities. The Contractor shall be responsible for the cost of replacing any keys that are furnished and lost. If the CO or COR decides that a lock must be replaced because of the loss of a key by the Contractor, the Contractor shall pay the cost of that replacement. The Contractor shall pay the cost of changing a combination if the CO or COR has reasonable cause to believe that the combination has been compromised. The contractor is not authorized to duplicate any keys.
4.6.5. SECURITY CLEARANCES. The Contractor shall obtain all required corporate and personnel Secret Level Security Clearances prior to commencement of work. The Contractor shall ensure that a list of all personnel with Secret Level Security Clearances is maintained current, including clearances that are pending. The Contractor shall submit a Security Clearance
DD Form 254 for each employee. All prime contractor employees shall have a secret security clearance. This clearance shall be obtained by and at the cost of the Contractor through their own security management. During clearance processing, the contractor will be permitted to continue work with an interim security clearance confirmed through SPAWAR Security office.
If the prime contractor chooses to sub-contract any work, the sub-contractor is not required to have a secret clearance, but shall be escorted by the prime contractor.
4.6.6. BASE ACCESS. The Government will conduct a background check before a pass is issued to enter Joint Base Charleston. A pass is mandatory before any work may be performed on JB CHS. All personnel employed by the Contractor in the performance of this contract, or any representative of this Contractor entering the Government installation, shall submit and abide by
Standard Form 74/DOD Contract Security Classification specifications which is issued by the
Contract Administrator.
4.7. HAZARDOUS MATERIAL IDENTIFICATION. Material Safety Data Sheets (MSDS) are required as specified in the latest version of Federal Standard No. 313 (including revisions adopted during the term of the contract) for any other material designated by a government technical representative as potentially hazardous and requiring safety controls. MSDSs must be submitted by the Contractor upon contract award. Failure to provide MSDSs or certificate when
Page 19 of 157 Attachment 1 FA4418-12-R-0003 requested could result in the Contractor being considered non-responsive and result in termination of the contract.
4.8. SPILL RESPONSE. The Contractor will be briefed on Joint Base, Charleston spill response procedures at the pre-performance conference. The Contractor is responsible to report and promptly cleanup all spills in a manner consistent with current environmental regulations, in the event that it is necessary to utilize government material, equipment or personnel to clean up a
Contractor caused spill, the Contractor shall be required to reimburse the government for all associated costs.
4.9. HAZARDOUS MATERIAL/ WASTE MANAGEMENT. The Contractor will be briefed on Joint Base – Charleston Hazardous Material / Waste Management Plan at the pre-performance conference by a representative from the environmental department.
4.10. HAZARDOUS MATERIAL HANDLING. The Contractor shall have approval from the base Bio-Environmental (BEE) and HAZMART sections prior to purchasing, handling, using, and storing any chemicals, solvents, lubricants, and other products that require MSDSs. The
Contractor shall identify these materials and products on Air Force Form 3952
Chemical/Hazardous Material Authorization Request monthly to the COR, Process Identification
Form, and Shop Disposal Form. The Contractor shall provide one copy of the MSDSs for each item to BEE and HAZMART sections for review prior to any chemicals being brought onto Joint
Base – Charleston. The Contractor shall maintain one copy of the MSDSs for each hazardous material line item used within the work center. Storage of HAZMAT shall be done in compliance state, local, and base regulations.
The Contractor shall not use products that are or contain Toxic 17 chemicals, Extremely
Hazardous Substances (EHS), Ozone Depleting Substances (ODS), and/or Persistent
Bioaccumulative and Toxic (PBT) chemicals. Any HM (Hazardous Material) containing one of these banned substances will not be allowed on base.
