The following have been modified:

ADDENDUM TO 52.212-1

This provision is incorporated by reference with the following addenda to paragraphs

A. Submission of Offers. This Firm-fixed-price (FFP) contract is being issued as full and open competition. The contracting officer has determined there is a high probability of adequate price competition in this acquisition.

Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional price information will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit information to the extent necessary for the contracting officer to determine the reasonableness and affordability of the price.

B. Quotes must be complete, self-sufficient, and respond directly to the requirements of the Request for Quote

(RFQ). Failure to meet a requirement may result in an offer being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale.

The Government intends to evaluate offers and award this Firm fixed-price contract without discussions.

Therefore, the offeror’s initial quote should contain the offeror’s best terms from a price standpoint. The offeror’s quote response shall consist of three (3) separate parts; Part I - Technical, Part II – Past Performance Information, and Part III – Price.

C. Specific Instructions:

1. All quotes shall be submitted electronically via email to 2d Lt Matthew Hendricks at matthew.hendricks.2@us.af.mil, and Paula Reinhard at paula.reinhard@us.af.mil. Mailed and hand delivered quotes will not be accepted. Offeror is responsible for ensuring his/her quote is received by the date and time listed in block 8 of the SF1449.

2. Period for acceptance of offers. The offeror agrees to hold the prices in its offer firm for 90 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation.

3. All quotes are due NLT 3:00 PM Central Daylight Time (CDT), 10 April 2017.

4. (ADDED) No questions will be accepted after 4:00 PM CDT, 22 March 2017.

PART I: TECHNICAL

Subfactor 1 - FedRAMP Impact Level 4: Offeror shall submit a copy of the Authority to Operate (ATO) approval letter from DISA to validate FedRAMP Level 4 Compliance in accordance with SOW, paragraph 3.1.1. FedRAMP Level 4 ATO must cover the entire base period of performance which is 12 months from contract award.

Subfacrtor 2 – FedRAMP Impact Level 5: Offeror shall provide written documentation that the proposed Cloud Service Provider (CSP) has applied for FedRAMP Level 5 and include an estimated completion date. In accordance with SOW, paragraph 3.1.1., if sub-applications are required to be migrated to FedRAMP Level 5 due to sensitivity of data, contractor must provide proof the cloud environment is approved for FedRAMP Level 5 and has DoD ATO within 30 calendar days of notification from the Contracting Officer.

Subfactor 3 – Authorized CSP Partner: Provide written documentation that Offeror is an authorized partner with direct access to the proposed CSP in accordance with DoD CIO Memorandum, Updated Guidance on the Acquisition and Use of Commercial Cloud Computing Services, release on 15 December 2014, Appendix 1;

narrative of how Offeror will provide access to the FedRAMP Impact Level 4 Cloud Service Provider (CSP).

Provide Offeror’s Service Level Agreement outlining how Offeror will provide access to the FedRAMP Impact Level 4 Cloud Service Provider (CSP); and clearly describe Offeror’s technical approach/process to perform the System Administration functions and ensure Cyber Security compliance in accordance with SOW paragraphs 3.1.7 and 3.1.7.3.

Subfactor 4 – Hosting Environment: Provide a written plan showing how Offeror will deliver the required hosting environment, to include Development, Testing and Production, within 14 calendar days of contract award in accordance with SOW, paragraph 3.1.1.

Subfactor 5 – Small Business Participation Plan: The offer shall provide a clear, detailed, logical and realistic subcontracting plan for the base contract period and all option years which demonstrates adequate participation by small firms in terms of value of the total acquisition and adequate complexity and variety of work for small firms to perform. The offeror shall include names of firms, realistic commitment documents with such firms and a description of the requirement to be performed by the firms to include an estimated value of each subcontract. The offer shall include historical records that show compliance with corporate and individual contract subcontracting plans. NOTE: A small business offeror will receive a rating of “Not Applicable” for this subfactor. Of the total amount the offeror plans to subcontract for this contract, minimum percentages shall be*

21 % to small business 5 % to Small Businesses 5 % to Small Disadvantaged Business 5 % to Women-Owned Small Business 3 % to HUBZone Small Business 3 % to Serviced Disabled Veteran-Owned Small Business

*If no subcontracting opportunities exist, please provide written justification.

PART II: PAST PERFORMANCE

Offerors must submit at least two references that can provide recent (within the past three (3) years from the date of issuance of this solicitation) and relevant (similar or greater in size/scope/complexity of this instant requirement) Past Performance information to the Contracting Officer with quote.

PART III: PRICE

The offeror must submit a quote with unit prices and extended prices for CLINS 0001, 1001, 2001, and 3001 (pages 3 and 4 of the SF1449) in response to this solicitation. In doing so and without taking any exception to the requirements of this solicitation, including SOW, “Schedule of Supplies/Services”, and terms and conditions, the offeror accedes to all solicitation requirements. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale.

Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
