ATTACHMENT 5: PAST PERFORMANCE QUESTIONNAIRE

Past performance information is required for each offeror and/or each team member if a subcontractor, teaming or partnering arrangement exists. The offeror shall ensure that a past performance questionnaire is completed for each project used to demonstrate relevant experience. Relevant past performance information is defined as work of a similar nature to the Lackland AFB Multi-media Services requirement of which the offeror accomplished during the past three (3) years. Questionnaires are to be completed by the contracting activity (owners, clients, prime contractors, etc.) responsible for the particular projects and furnished by the completing party directly to Gina Gipson, Contract Administrator, telephone 210-671-1770, fax 210-671-1433, email gina.gipson@lackland.af.mil. If returning by fax, please call to confirm receipt of fax.
