PART 1.0 – SCOPE OF WORK
1.1 GENERAL: The work to be performed under this contract and in accordance with this Statement of Work shall consist of furnishing all necessary parts, labor, tools, transportation, supplies, supervision, equipment, materials, and incidentals necessary for providing all work shown in the project documents, and all applicable codes, regulations, standards, and criteria in effect at the date of solicitation.
1.2 PROJECT DESCRIPTION: The Scope of Work includes the replacement of flooring for buildings
3226, 3303, and 430. B3226 is the Marines Charge of Quarters. This building is occupied by the Marines chain of command. Building 3226 will consist of removing and installing carpet for all offices and conference rooms. In addition, the lobby and kitchen area will have carpet removed and replaced with Luxury Vinyl Plank (LVP/LVT). B3303 is the Civilian Personnel facility. The facility consists of human resources and support personnel for civilians. Building 3303 will consist of replacing existing carpet in the hallways, storage room, and office spaces with LVP. B430 is the 17 Training Wing Headquarters. The area being worked on consists of the Military Personnel Flight. Building 430 will consist of replacing existing carpet in the MPF Working Area.
1.3 LOCATION: Goodfellow Air Force Base is located in Tom Green County, on the southeast side of San Angelo, TX. and is bounded to the north by Highway #388 (Paint Rock Rd.), to the west by Fort McKavitt Rd and Bell Street/Christoval Road, to the south by Highway #1223 (San Antonio Hwy.) and to the east by the eastern city limits. The buildings numbers are 3226, 3303, and 430.
1.4 WORK AND MECHANICS: The work for this project shall be executed in the best and most workmanlike manner, by qualified and efficient mechanics/tradesmen, skilled in their respective trades. Only certified journeymen in each respective trade, or apprentices under the direct supervision of certified journeymen, shall be permitted to install and/or supervise installation for this project. Individual trade work for this project shall be performed and quality maintained by the applicable trade, only. All trades shall coordinate their work with that of other trades. The Contractor shall coordinate and perform all operations in a manner that will result in a professional and expeditiously completed project. The work shall be in strict accordance with prevailing industry standards and manufacturer’s instructions.

1.5 PERFORMANCE PERIOD: The Performance Period for all Basic CLINs shall be ninety (90) calendar days.
BUILDING PHASING PLAN: The Contractor shall follow the specific plans for B3226, B3303, and B430 to eliminate or minimize disruptions during work hours described below. Work shall be incrementally performed throughout each phase to ensure government functionality. The Contractor shall submit a project schedule in the form of an AF 3064/3065.
1.5.1 BUILDING 3226: Phase 1 – Rooms 103/104, 108, Phase 2 – Rooms 105, 106/107, Phase 3 – Rooms 102, 109, Phase 4 – Lobby 101/116
1.5.2 BUILDING 3303: Phase 1 – Hallway 3106, Phase 2 – Room 3131, Phase 3 – Room 3130, Phase 4 – Room 3129

1.6 WORKING CONDITIONS FOR B3226/B3303: Working hours for the Contractor will normally be between the hours of 7:30 A.M. and 4:30 P.M. excluding Saturdays, Sundays, Federal holidays, and days designated as “Family Days” by Air Education and Training Command (AETC).
Government escorts are not needed for this area. See the table below for reference for the 2023- 2024 dates and refer to Section H of the solicitation/contract document for further information on working days. Any holiday falling on a Saturday will be observed the preceding Friday. Any Holiday falling on a Sunday will be observed the following Monday.
1.7 WORKING CONDITIONS FOR B430: Working hours for the Contractor for B430 will be from 8:00 A.M. to 4:00 P.M from Saturday to Tuesday for Phase 1. Government escorts will be needed at that time. Once Phase 1 is complete, the working hours for the Contractor will be between the hours of 7:30 A.M. and 4:30 P.M. excluding Saturdays, Sundays, Federal holidays, and days designated as “Family Days” by Air Education and Training Command (AETC).
If the Contractor desires to work during periods other than above, a request must be made to the Contracting Officer in writing three (3) calendar days in advance of his/her intention. If the required base personnel are reasonably available, the Contracting Officer may authorize the Contractor to perform work during periods other than normal duty hours/days.

The base could be closed because of security problems, adverse weather, or other events. Unless otherwise notified by the Government, the Contractor should monitor local television stations, radio stations, or Goodfellow’s Facebook page for notification of a possible base closure or late opening. The Contractor may not receive any other form of notification of a base closure from the Government, unless contacted by the Contracting Officer (CO) or the COR. The Contractor is responsible for notifying his/her employees. Contractor(s) do not report when the base is closed due to security problems and/or adverse weather.
The Contractor shall maintain the worksite conditions for the period stated in paragraph PERFORMANCE PERIOD. The Contractor shall submit an Indoor Air Quality Management Plan.
Building 430 will require at least four workspaces within Area 128K available to be occupied at all times. The Contractor shall coordinate the work of all activities whereby both the Government and the Contractor can continue operations with the least possible interference and inconvenience.
The Contractor shall conduct all work such that means of facility and egress are maintained at all times. The Contractor shall be responsible for providing suitable, approved signs, barricades, rope barriers, etc., to warn occupants of hazardous areas at the job site for the duration of the contract at no additional cost to the Government. Signage shall also be provided by the Contractor to redirect personnel occupying the facilities. The Contractor shall report any findings of PII immediately to the Contracting Officer. All Contractor operated/owned vehicles, equipment, tools, etc., shall be stored as prescribed in paragraph DELIVERY, STORAGE, AND PARKING.
Applicable traffic control signage where required by the contract shall remain overnight. Parking on turf is not permitted without prior approval.
1.7.1 WORKER SAFETY CONSIDERATIONS: All Contractor operations shall be conducted and performed in accordance with Department of Labor, OSHA requirements found in 29 CFR 1910 (1910.146 and 1910.147) and 29 CFR 1926, and Air Force Occupational Safety & Health (AFOSH) standards including AFI 91-203, Air Force Consolidated Occupational Safety Instruction. The Contractor shall also ensure that all work is performed in accordance with project identified national standards, military manuals, instructions, pamphlets, standards, and handbooks, and with the edition in effect on the date of this solicitation of the Corps of Engineers (COE) Safety Manual 385-1-1. All job sites are subject to inspections by the Department of Labor. In the event of conflicts between the OSHA standards and these requirements, the most stringent shall apply.
1.7.2 WORKER’S COMPENSATION LAWS: All companies who conduct business within the state of Texas must, in accordance with Texas Worker’s Compensation laws (Texas House Bill 62), have an approved Company Safety Policy and an Accident Prevention Plan. In addition to meeting the TWCC requirements, the plan must also include the requirements of Safety Manual
EM 385-1-1.
1.7.3 VIOLATIONS OF SAFETY & HEALTH STANDARDS: Resolution of Department of Labor citations for violations of Occupational Safety and Health Standards is a Contractor responsibility and shall provide for no basis of a claim against the Goodfellow AFB.
1.7.4 TOILET FACILITIES: There are toilet facilities available for Contractor use.

1.8 TOBACCO USE ON AETC FACILTIES: Contractors are advised that the Commander has placed restrictions on the smoking of tobacco products in AETC facilities. AFI 40-102, Tobacco Use in the Air Force, outlines the procedures used by the Commander to control smoking in our facilities.
Contractor employees and visitors are subject to the same restrictions as government personnel.
Smoking is permitted only in designated smoking areas. Locations of designated smoking areas can be provided upon request.
1.9 SECURITY FORCES: The Contractor shall always comply with base law enforcement and security requirements to include base pass requirements.
1.9.1.1 BASE ACCESS REQUIREMENTS: Goodfellow Air Force Base is a closed/controlled access base. The Government will conduct security/background checks on all construction personnel as required. Requirements are subject to change dependent upon current world situations, potential threats, and base exercises. Contractor should anticipate certain notification and reporting requirements, preparation of forms, and lists for pass and identification of employees and their vehicles, entry restrictions, key/lock control, and compliance with all base traffic rules and regulations. The Jacobson Gate Visitor Control Center 325-654-4122 is the primary point of contact for passes.
1.9.1.2 INSTALLATION ACCESS PASS: Before arrival, a government card-holding person will submit a request for base access using the 17 Training Wing’s Base Access List (BAL) memorandum as a form of registration for each applicant. The BAL will include pertinent visitor information, reason for entry, frequency of entry, destination, times each day requiring entry, and duration of request.
1.9.1.3 ACCESS DENIAL: If it is determined a Contractor requesting access has been convicted of a felony or pled guilty to a felony charge within the past 10 years, or is considered not fit to obtain authorized access based on the information obtained during the identity vetting, or criminal history indicates the individual may present a threat to the good order, discipline and morale of the installation, Security Forces personnel will deny entry. The Contractor will be informed of the access denial, will be issued an Access Denial Letter, and will be informed on how they may appeal this order.
1.9.1.4 DENIAL APPEAL PROCESS. When denied access, contract visitors will be informed to report back their manager. If the contract worker and management are considering an appeal, it will be submitted by letter to the 17 SFS Commander, within 30 days of access denial. The 17 TRW/CC will approve/disapprove all appeals for entry.
1.9.1.5 ABNORMAL WORK SCHEDULE: For access on non-duty hours or down days:
a) Identify which workers need access for times not listed in 1.6 WORKING CONDITIONS.
b) Ensure only workers that are already vetted are on the Extended-Hours request (no new personnel). New personnel require a new BAL and formal vetting.
c) Complete the “After Hours” BAL for workers requiring down day access to the installation.
If deliveries are not made during the operational hours listed in the paragraph NORMAL WORK SCHEDULE, the request may be declined.

1.9.1.6 PASS HANDLING: All BALs have a duration not to exceed 180 days. When an employee is no longer employed by the Contractor or Sub-Contractor, all ex-employee passes are to be returned to the Government. If an issued pass is not returned, the Contract Officer may withhold funds, or the Installation Commander may consider debarment to the installation.
1.9.1.7 ANTI-TERRORISM TRAINING: Anti-Terrorism (AT) Level I Training is available to all Contractor personnel. The Government will provide/make available AT Level I training/information at the Contractor’s request. This information can be provided through the 17 TRW Contractors AT Level I awareness pamphlet/binders, through computer-based training programs, or through the unit/installation AT Officer. To request training, contact the 17th TRW AT, 17 TRW/CCW at (325) 654-3527.
1.9.1.8 FPCON LEVELS:
1. FPCON NORMAL—This condition applies at all times as a general threat of terrorist attacks, hostile acts, or other security threats always exists in the world.
2. FPCON ALPHA—This condition applies to a non-specific threat of a terrorist attack or hostile act directed against DOD elements and personnel. Commanders declare FPCON Alpha when a terrorist attack or hostile act is possible, but no specific information exists indicating a direct or indirect threat to DOD elements and personnel. Commanders must be able to sustain applicable FPCON Alpha measures indefinitely.
3. FPCON BRAVO—This condition applies when an increased or more predictable threat of a terrorist attack or hostile act exists and is directed against DOD elements and personnel.
Commanders should be able to sustain all applicable FPCON Bravo measures indefinitely and understand FPCON Bravo will likely affect missions and base support operations during prolonged implementation.
4. FPCON CHARLIE—This condition applies when a terrorist or hostile act incident occurs within the commander’s area of interest, or intelligence is received indicating a hostile act or some form of terrorist action or targeting against DOD elements, personnel, or facilities is likely. FPCON CHARLIE measures will very likely affect missions and base support.
Commanders should ensure they can sustain applicable FPCON Charlie measures throughout the entirety of the threat.
5. FPCON DELTA—This condition applies when a terrorist attack or hostile act has occurred or is anticipated against specific installations or operating areas. FPCON Delta should be maintained on a limited basis and only be declared so long as the necessary response capabilities are required.
1.9.1.9 The Contractor will ensure areas controlled by Contractor employees comply with base operations plans/instructions for FPCON procedures and local search/identification requirements.
The Contractor will be responsible for the above FPCON requirements in all assigned facilities.
The Contractor will safeguard all government property, including controlled forms, provided for their use. At the close of each work period, government training equipment, support equipment, SOW: Page 8 of 18 and other valuable materials will be secured. For instructions, regulations, and forms, contact the
CO.

In the event of conflicts between the OSHA standards and these requirements, the most stringent shall apply. 1.11 REFERENCES: The Contractor shall identify and comply with all applicable federal, state, and local laws; statutes; Executive Orders; Department of Defense/Air Force manuals, handbooks, regulations, guidance, and policies. The Contractor shall comply with the National Institute of Building Sciences’ Whole Building Design Guide Construction Criteria Base Unified Facility Criteria (UFC) and all related libraries. It is the Contractor’s responsibility to identify and comply with all applicable requirements. In addition to the documents referenced above, all publications listed herein shall be the most current editions in effect at the time of solicitation and form a part of this SOW:

1.12 SITE VISITS: The Contractor may visit the premises to become thoroughly familiar with details of the work and working conditions, verify dimensions in the field, and shall advise the Contracting Officer of any discrepancies before starting the work.
1.13 SUBMITTALS: The Contractor shall provide submittals in the form of manufacturer’s data, certificates of compliance and samples for all items provided and installed. The Contractor will not be permitted to perform any work on site without approved submittals. The submittals listed shall be required and shall be submitted for Government Approved (GA) or For Information Only (FIO). Use AF Form 3000 to process submittals. Submit four copies of submittals to Contracting Officer.
1.13.1 The required submittals are as follows: Project Schedule in the form of an AF 3064/3065, Indoor Air Quality Management Plan, Base Access, Material Submission, Construction Waste Management Form, 7-calender day lead time written notification, a minimum 24 hours written notice prior to the scheduled inspection, Project Closeout Checklist, samples, Manufacturer’s Warranty, Contractor Quality Control Daily Reports, Utility Outages, Street Closings, Hazardous Material List with worksheet for each SDS, EMS Awareness Training Certification, and Solid Waste Diversion Report. More information about each submittal will be underlined or referenced in the Environmental Requirements.
PART 2.0 – PRODUCTS
2.1 REFERENCES TO MATERIALS, MANUFACTURERS AND PRODUCTS: Materials shall be the standard product of manufacturer’s regularly engaged in the manufacture of such products.
The products furnished shall meet the quality and specifications indicated herein.
2.1.1 Tier II Contractors who shall purchase and install carpet from one of the awarded Tier I vendors. All terms and conditions, including the NTE pricing established in the Tier I Contracts, shall be incorporated into all subsequent Tier II contracts. Based upon the recommendation of the Air Force Furnishings Commodity Council (AF FCC) and the Furnishing Co-Commodity Sourcing Officials, it has been determined a mandatory-use policy is warranted for the procurement of carpet for AF installations located within CONUS. Deviation from this policy is only authorized by receiving an approved waiver from the AF FCC.
2.1.2 Verification of dimensions and conditions: The Contractor is highly encouraged to visit the premises to become thoroughly familiar with the details of the work and working conditions. The

Contractor shall field verify all dimensions, equipment, and conditions prior to the start of work and advise the Contracting Officer of any discrepancies prior to the work commencement.
2.1.3 All materials shall be submitted by the Contractor and approved by the Contracting Officer prior to installation.
2.1.4 The Contractor shall provide an additional minimum of 10% supply to account for any facility repairs.
2.2 PRODUCT DATA:
2.2.1 LUXURY VINYL PLANK/TILES (LVP/T) AND CARPET (FOR BUILDING 3226):
Contractor shall conform to ASTM f 1700-04, asbestos-free, floating/interlocking floor type.
Flooring in any one continuous area shall have the same shade and pattern of the approved sample.
LVP should have coating to eliminate the need for polishes, wax, and harsh chemicals to help reduce maintenance cost. LVP should be hard wearing, providing for long lasting appearance in heavy commercial traffic environments. Glue down adhesives shall be provided by the Contractor and applied per the manufacturer's installation guidelines.

2.2.2 LUXURY VINYL PLANK/TILES (LVP/T) (FOR BUILDING 3303): Contractor shall conform to ASTM f 1700-04, asbestos-free, floating/interlocking floor type. Flooring in any one continuous area shall have the same shade and pattern of the approved sample. LVP should have coating to eliminate the need for polishes, wax, and harsh chemicals to help reduce maintenance

2.2.3 CARPET (FOR BUILDING 430): Contractor shall conform to ASTM f 1700-04, asbestos-free, floating/interlocking floor type. Flooring in any one continuous area shall have the same shade and pattern of the approved sample. Glue down adhesives shall be provided by the Contractor and applied per the manufacturer's installation guidelines.

Contractor shall submit Carpet Tile flooring based on the above stated criteria for (CO) approval.
RUBBER COVE BASE: Rubber cove base shall conform to Federal Specifications, Type I (rubber), Style B (cove). Rubber cove base shall be 6in. high roll stock (minimum 100ft. and/or longest practical length), minimum 0.125in thick. The color for the cove base for B3226 shall be in the shade of Roppe 624 Chameleon or similar to the Carpet Tile and/or LVP/T being installed.
For B3303, the color for the cove base shall match existing base cove where the current LVP is.
For B430, the color for the cove base shall be in the shade of Roppe 150 Dark Gray or similar.
The Contractor shall be limited to (3) colors as manufactured by Mohawk or approved equal. Ensure cove base covers any damage when removed. Rubber cove base shall be cut to length of wall.
Butt joints shall be kept to a minimum. No vinyl cove base will be accepted. Adhesives for wall base application shall be provided by the Contractor and applied per manufacturer installation guidelines.

2.3 THRESHOLDS/TRANSITION STRIPS: Thresholds\transition strips shall be used on all changes in flooring. Flooring transitions shall be of the same manufacturer and color of LVP/T and Carpet Tile provided and shall be fastened to the subfloor in accordance with manufacturer recommendations. Contractor shall provide T-molding where flooring levels are of similar heights, and reducers where flooring levels differ. Installations of transitions shall be in accordance with manufacturer recommendations.
2.4 FLOOR LEVELER: The Contractor shall provide and apply an approved floor leveling compound to achieve a smooth level plane across the existing floor and ensuring that all gaps and voids are filled. The Contractor shall follow the manufacturer’s installation/application recommendations.
PART 3.0 – EXECUTION
3.1 GENERAL: All work shall be performed as shown and in accordance with the manufacturer’s diagrams and instructions, unless otherwise specified. The Contractor shall field verify all dimensions and site conditions. Price increase adjustments to the original contract price will not be issued because the Contractor was not aware of existing conditions. The Contractor shall provide all labor, materials, signage, tools and equipment required to perform all dismantling, repairs and installation as listed in this statement of work. The Contractor shall perform work in accordance with BUILDING PHASING PLAN. The Contracting Officer shall approve work performed prior to continuation of subsequent phases.
3.2 DEMOLITION: The Contractor shall demolish and dispose of the existing carpet, cove base, and transition strips for B3226, B3303, and B430 as depicted in the drawings. The Contractor shall recycle or divert construction wastes from landfill disposal to the maximum extent practicable.
The Contractor shall track recycling and waste disposal and submit the report on the provided Construction Waste Management Form for Government Approval at the end of the project, and prior to final acceptance of the work. The Contractor shall take all necessary precautions to prevent damage to property to remain in place. Any damage to the aforementioned shall be repaired or replaced by the Contractor at no additional cost to the Government.
3.3 INSTALLATION: All work shall be done with the building occupied and the work area unoccupied. The Contractor shall coordinate with the Contract Inspector prior to start of work. The Contractor shall move all furnishings to each building’s respective designated storage areas inside of the facility or outside of the facility in a temporary portable storage container provided by the Contractor. The Contractor shall provide written notification to the Contracting Officer 7- calendar days lead time prior to starting each phase. The Contractor shall not begin work until all materials and necessary equipment and are on site and approved. The Contractor shall be responsible for the moving and protection of all furniture and other items, with the exception of electronics, training equipment, and personal items. Furniture includes, but is not limited to, office desks, conference tables, chairs, bookcases, filing cabinets, cubicles, refrigerators, etc. The Contractor alongside the Contracting Officer’s Designated Representative shall inventory all room furniture and other items and make note of any dents, large scratches, missing hardware, and other existing defects. The Contractor shall protect and clean all items. Damage caused to furniture or other Government property as a result of Contractor operations shall be repaired or replaced at no additional cost to the Government. The Contractor shall replace or repair all furniture to its original location unless directed
SOW: Page 13 of 18 otherwise by the Contracting Officer’s Designated Representative. The Contractor shall not move/return any furnishings until after inspection of each completed area according to the phasing plan. Prior to installation of new flooring, the Contractor shall remove existing flooring to include carpet and luxury vinyl tile/plank. The Contractor shall provide floor preparation to the existing surface to include scraping and the removal of excess glue and material. The Contractor shall be required to use the entrances/exits indicated by the Contracting Officer’s Designated Representative. The Contractor shall provide and install a self-leveling topcoat and floor leveler to existing concrete floor as per the manufacturer’s installation recommendations. The Contractor shall ensure that all gaps and voids have been filled. The Contractor shall ensure that a smooth and flat surface has been achieved.
The Contractor shall provide and install new carpet flooring, LVP/T flooring, thresholds/transition strips, and rubber cove base. The Contractor shall coordinate with the Contract Inspector prior to start of work.
3.4 LUXURY VINYL PLANK (LVP/T) FOR BUILDING 3226/3303:
Luxury Vinyl Plank (LVP) and coordinating thresholds/transitions shall be provided in accordance with the approved installation instructions of the manufacturer. Tile lines and joints shall be kept square, symmetrical, tight, and even. Floor shall be in a true, level plane, except where indicated as sloped. Edge tile width shall vary as necessary to maintain full-size tiles in the field, but no edge tile shall be less than one-half the field tile size, except where this is impossible because of irregular shaped rooms. Flooring shall be cut to, and fitted around, all permanent fixtures, built-in furniture and cabinets, pipes, and outlets. Edge tile shall be cut, fitted, and scribed to walls and partitions after field flooring has been applied. Edge strips shall be provided where required. Cove base and edge strips shall be secured with adhesive per manufacturer recommendations. Floor leveler should be used where needed.
Noise shall be kept to an absolute minimum. If new floor covering interferes with operation of existing doors, the Contractor shall cut door bottoms to provide minimum adequate clearance for new floor coverings as specified. Door bottoms shall be cut straight and square. Wrap door bottom with masking tape, prior to cutting, in order to circumvent splintering; seal wood door bottoms with wood sealer.
Doors shall be able to operate freely without hindrance after installation of new tile and or carpet as specified. Reinstall doors to good working condition. Doors that are damaged shall be replaced or repaired to match existing undamaged condition at no additional cost to the Government. New doors shall be of like quality and shall be finished to match existing doors. The amount of dust, resulting from the project, shall be controlled to prevent the spread of dust to occupied portions of the building and to avoid creation of nuisance in the surrounding area. Any unnecessary damage to the buildings resulting from the Contractor’s operation shall be repaired at the expense of the Contractor and to the satisfaction of the Government. Existing painted surfaces which are damaged or altered by work under this contract shall be properly patched and prepped and be given a minimum of two coats of paint to match adjacent surfaces. The Contractor shall fully restore the contract work area site to its original condition. The restoration shall include cleaning, mopping, and maintenance as required to return site to its original condition as determined by the Contracting Officer.
3.5 CARPET FOR BUILDINGS 3226/430:
Carpet and coordinating thresholds/transitions shall be provided in accordance with the approved installation instructions of the manufacturer. Carpet and transition strips shall be adhered using industry standard adhesive. Cut outs, at door jambs, floor utilities, columns, and ducts shall be nearly cut and fitted securely. Seams at doorways shall be located parallel to and centered directly under doors. Seams shall be uniform, unnoticeable, and treated with a seam adhesive. The Contractor shall terminate carpet at door thresholds with transition strips that match existing thresholds if needed. The Contractor shall not make any alterations to the building itself throughout the duration of this project. Doors shall be able to operate freely without hindrance after installation of new carpet as specified. Reinstall doors to
SOW: Page 14 of 18 good working condition. Doors that are damaged during operations shall be replaced or repaired to match existing undamaged condition at no additional cost to the Government. New doors shall be of like quality and shall be finished to match existing doors. The amount of dust, resulting from the project, shall be controlled to prevent the spread of dust to occupied portions of the building and to avoid creation of nuisance in the surrounding area. Noise shall be kept to the absolute minimum. Any unnecessary damage to the buildings resulting from the Contractor’s operation shall be repaired at the expense of the Contractor and to the satisfaction of the Government. The Contractor shall fully restore the contract work area site to its original condition. The restoration shall include cleaning vacuuming, shampoo cleaning, and maintenance as required to return site to its original condition as determined by the Contracting Officer.
3.6 INSPECTION REQUESTS: Contractor shall provide the Government a minimum 24 hours written notice prior to the scheduled inspection. Any work that covers or inhibits the ability of a government inspector from completing his QA inspections under standard construction conditions is subject to removal and replacement at the Contractor’s expense.
3.6.1 QUALITY ASSURANCE: In all work performed, it is the Contractor’s responsibility to perform proper QC inspections throughout the project to ensure a quality product. The Government inspector is responsible for adequate Quality Assurance (QA) for the work accomplished. It is not the role of the Government to perform the Contractor’s QC for the project, especially during inspections. All work regarding meetings and inspections shall be performed by the Contractor in accordance with the attached Project Documents.
3.6.2 PRE-FINAL INSPECTIONS: The Contractor shall conduct a pre-final walk-through inspection and publish the pre-final inspection findings in a pre-final inspection report. The Contractor shall certify that 100% of the Contractor’s QC punch list is completed prior to requests for a Government pre-final inspection, with their completed QC punch list attached. The Contractor shall conduct a pre-final walk-through inspection with a Government representative and publish the inspection findings in a Government pre-final inspection report. Contractor shall comply with all applicable provisions listed in the attached Project Documents regarding pre-final inspection requirements. Pre-final inspection report shall include a list of Contractor warranty and manufacturer warranty books/certificates from newly installed equipment and materials. The Contractor shall be responsible for completing and submitting to the Government the Project Closeout Checklist during this Pre-Final Inspection.
3.6.3 FINAL INSPECTIONS: At the contract final inspection, there will be a joint Government- Contractor inspection to include operational testing of all fire detection, plumbing, communications, electrical, mechanical devices, and equipment to ensure all facility systems are functioning and operable. Under no circumstances will Goodfellow AFB occupy or operate in any building areas under construction without consent from the Contractor or in emergency situations.
The Contractor shall certify that 100% of the pre-final punch list is completed prior to requests for a Government final inspection. The Contractor shall conduct a final inspection and publish the findings in a final inspection report (Punch List) in accordance with the format specified in the attached Project Documents. The inspection shall concentrate on the items identified at the pre-
SOW: Page 15 of 18 final inspection and recorded in the pre-final report. The final inspection report shall (1) certify that all items of the design have been implemented and that the construction is complete; (2) include a record of as-built drawings and specifications verifying that all development standards have been met; (3) schedule to complete the punch list; (4) One year Contractor warranty and manufacturer warranty books/certificates from newly installed equipment and materials.
3.6.4 MANUFACTURER’S CATALOG DATA: Data composed of catalog cuts, brochures, circulars, specifications and product data, and printed information in sufficient detail and scope to verify compliance with requirements of the contract documents.
3.6.5 SAMPLES: The Contractor shall submit physical samples of the carpet, Luxury Vinyl Flooring, rubber wall base, and thresholds stated in paragraph 2.2 PRODUCT DATA or equal for Contracting Officer (CO) approval. The Contractor shall submit a sample in each color of the product. The Contractor shall turn over any unused flooring to the Government prior to final inspection.
3.6.6 MANUFACTURERS WARRANTY: The Contractor shall provide at minimum a one (1) year Construction warranty for all work conducted on this project.
The Contractor shall identify all items being installed that are guaranteed or under warranty for more than a one-year period and provide validated copies of the manufacturer’s warranty to the Government. All warranty information shall be filed by the Contractor in the name of Goodfellow AFB. All warranties shall be identified by product with a listing of the name and address of the company and the expiration date of the guarantee or warranty.
3.6.7 OPERATIONS AND MAINTENANCE MANUALS: The Contractor shall provide system O&M manuals and parts list to the Government, and train maintenance personnel in system operations prior to delivery and Government acceptance, as well as manufacturers’ point of contacts (POCs) for troubleshooting, repair and consulting questions. All warranty information shall be provided by the Contractor as described in the attached Project Documents.
3.7 CONTRACTOR QUALITY CONTROL DAILY REPORTS: The Contractor shall establish and submit to the Contracting Officer a written daily quality control report to assure compliance of this project package. Any work found not to comply with the contract shall be promptly removed, replaced, or corrected in an approved manner. Written Daily Quality Control Report shall be submitted at the end of each work period, to include, but not be limited to the following:
a) Number and skill level workers and start and end time of various tasks.
b) Materials delivered to job site.
c) Work accomplished or in progress.
d) Remarks or special instructions on delays and safety.
PART 4.0 – SPECIAL CONDITIONS & PERMITS

4.1 BASE CIVIL ENGINEERING WORK CLEARANCE REQUEST: The Contractor shall obtain and process AF Form 103 for approval prior to commencement of work for this project. The Contractor shall always have the completed form on site.
4.1.1 PROCESSING AF FORM 103: Due to the requirement for multiple agencies to coordinate on these requests, expect 7 days for paperwork processing. Contractor requests should be submitted at the earliest possible date to preclude delays.
4.2 UTILITY OUTAGES: When a utility outage is necessary to perform the contract work in an occupied facility, regardless of whether the work area is occupied, the outage shall be performed by the Contractor during non-duty hours at no additional cost to the Government, unless otherwise approved by the Contracting Officer. The Contractor shall notify the Contract Inspector of outage requirements to include buildings affected; length of outage; and reasons for outage. The Contractor must allow affected occupants a minimum of two – (2) weeks’ notice prior to outage.
The Contractor is also required to provide the Contracting Office a written notification of the requested outage.
4.2.1 ACCIDENTAL UTILITY DISRUPTIONS: In the event of accidental disruption of any utility, the Contractor shall immediately notify the Contracting officer of the unplanned outage.
After reporting it to the Contracting Officer, call Civil Engineering (325-654-3436) to inform them of an outage with what type of utility you may have encountered.
The Contractor shall immediately take every reasonable step to repair the damage in a manner acceptable to the Government and shall restore the utility to full use as soon as practicable.
4.2.2 LIMITS OF GOVERNMENT UTILITY INFORMATION: The Government does not have, nor will it provide, record drawings of Frontier telephone cable plant and Suddenlink cable. The Contractor shall call 17 CS/SCMP at 325-654-3010 a minimum of five (5) calendar days in advance to have underground communications cable routes marked. In addition, the Contractor shall also notify 1-800-DIG-TESS. In the event the Contractor identifies utilities in the contract work area, which interfere with the newly proposed construction, the Contracting Officer shall be immediately notified, and the Government will take necessary corrective action at no cost to the Contractor. The Contractor shall furnish to the Contracting Officer as-built drawings clearly identifying the exact location of all utilities identified in the work area prior to project final inspection.
4.3 STREET CLOSINGS: When road closures are required, written notification must be made to the Contracting Officer a minimum of 7 calendar days in advance. One lane of traffic shall be maintained at all times unless otherwise approved in writing by the Contracting Officer. The Contractor shall notify the Security Forces at (325)-654-3504 and Base Fire Protection at (325)- 654-3532 three days prior to any closures. Personnel exposed to a traffic environment during hours of darkness, periods of reduced visibility, or as part of construction or maintenance activities, shall use reflective vest/accessories. Ft. Lancaster is expected to be closed during the demolition of B443. Contractor shall submit notification as indicated above and shall not exceed (60) days of closure to this road.

4.4 BASE FIRE REGULATIONS: The Contractor shall comply with Base Fire Regulations as set forth in the latest edition of GAFB Instruction 32-2001, titled “Base Fire Protection Program”. The Contractor shall use no explosives or fire in performing the work. All work shall be in strict compliance with all National Fire Codes.
4.5 LOCKOUT/TAGOUT, HAZARDOUS ENERGY CONTROL: In addition to the requirements in OSHA Std. 1910.147 (General Industry), if a Contractor needs to lock or tag something out, the Contractor will ensure that affected employees are notified before and after the locks and tags are used.
PART 5.0 - SITE MAINTENANCE
5.1 PROTECT SURROUNDINGS: The Contractor shall protect adjacent property, buildings, and their contents free of dust, dirt, or other materials from construction activities. Work areas shall be maintained in a neat, clean, safe condition and shall, at a minimum, be cleaned at the end of each shift. All streets and roadways in/or adjacent to the site shall always remain free of project generated trash and debris. Parking on turf is not permitted without written approval.
5.1.1 GOVERNMENT STORAGE: There are no Government furnished covered or secure storage areas. The Contractor shall provide all temporary storage containers to store all furnishings from B3226, B3303, and B430, and must be laid down outside of the building.
5.1.2 CONTRACTOR STORAGE AND DELIVERIES: All equipment and materials delivered shall be secured with protection from the weather, high winds, humidity, temperature variation, dirt, and any other contaminants. The project documents will designate Contractor storage and laydown yards. All project storage areas shall be kept free of debris, leaks, stains, or splashes and kept in a neat, clean, and safe condition. Any contamination of the storage area by a hazardous substance shall be immediately remediated by the Contractor, in accordance with the contract above at no additional expense to the Government. Upon completion of the work, the Contractor shall leave the work site and storage area(s) in a clean, neat, and workmanlike condition satisfactory to the Contracting Officer.
5.2 SITE CLEANLINESS: The Contractor shall collect all trash, debris, refuse, garbage, etc., that is generated and place it in appropriate disposal containers with lids on a periodic basis or as directed by the Contracting Officer’s Representative. Disposal shall be outside the limits of Government property. Disposal shall be by sanitary landfill or other approved methods and shall conform to all local, state, and federal guidelines, criteria, and regulations. All areas shall be cleaned daily and as required to maintain a professional appearance at all times. All existing building interiors affected by the work performed shall be fully protected by the Contractor up until completion. Restoration of the original ground contours is required unless otherwise directed by the Contracting Officer’s Representative.
PART 6.0 - CONSERVATION
6.1 UTILITIES CONSERVATION: The Contractor shall use good judgment in the conservation of Government utilities. Prevailing energy conservation practices shall be adhered to and enforced
SOW: Page 18 of 18 by the Contractor. The Contractor shall instruct employees in utilities conservation practices. For this contract, utilities, such as water and electricity, will be furnished by the government at no cost to the Contractor.
The Contractor shall not adjust mechanical equipment controls for heating, ventilation, and air conditioning systems unless dictated in the project documents. Water faucets or valves shall be turned off after the required usage has been accomplished.
PART 7.0 – RESPONSIBILITY
7.1 CONTRACTOR RESPONSIBILITY: The above Parts 1 through 6 do not in any way limit the responsibility of the Contractor to perform all work and furnish all plant, labor, and materials required by this Statement of Work.
PART 8.0 - COMPLETION OF WORK
8.1 OPERATIONAL SYSTEMS: The Contractor shall insure that work for this project is performed in accordance with the criteria herein and that all equipment and systems shall be fully operational at the completion of work for this project.
