STATEMENT OF WORK

SHADE STRUCTURE FABRIC CANOPY REPLACEMENT BPA

37 TRAINING WING

ADDENDUM 1 DATED JUNE 18, 2024

1. BACKGROUND: 37 TRW Commander has directed the establishment of a Blanket Purchase Agreement (BPA) to provide timely replacement of shade structures fabric canopy covers located at locations throughout Joint Base San Antonio (JBSA) Lackland, Chapman Training Annex and Camp Bullis (see attachment 3).

2. GENERAL DESCRIPTION: Contractor shall provide all materials, supplies, supervision, personnel, equipment, tools, and services necessary to replace fabric canopy covers at locations listed on attachment 3.

3. SCOPE OF WORK: The scope of work under this BPA includes the provisions of replacement of shade structure fabric canopy covers to include all ancillary fasteners and related items to manufacturer’s specifications, requirements, and recommendations for each call. The services may include, but are not limited to the following:

3.1. Requirement will consist of actions, services, and duties to complete the removal of damaged canopies and replaced with new canopies. The damaged canopies will be disposed of off the installation by the contractor. (Refer to SOW, Paragraph 11)

3.2. Contractor shall be responsible for conducting a site survey prior to call submittal to determine conditions which may affect final quoted price.

3.3. During site visit, the contractor shall identify any obstructions that may hinder, delay, or obstruct project or project location including but not limited to trees, powerlines, ground leveling, etc.

3.4. If a welding is required, it shall be incumbent upon the contractor to attain all applicable permits.

3.5. Post warning signs of construction in the area. Signs must be in plain view for pedestrian/vehicle traffic to see. (Refer to SOW, Paragraph 5)

3.6. Replace dimensioned canopies outlined in attachment 3.

3.7. Canopy minimum specifications:

• Canopy shall be beige or desert tan in color (same as existing color)

• The canopy shall be reinforced throughout to ensure strength in welded connections/joints.

• The structure and fabric shall be engineered and reinforced to withstand 125 mile per hour winds.

• Canopy shall be constructed with coated polyester UV and flame resistant, water-resistant fabric.

• All cabling shall be corrosion resistant and provide sufficient strength to provide increased tension and security between the canopy and support structure.

• All hardware shall be galvanized and/or be rust/corrosion resistant.

• Fabric shall be sewn with UV resistant thread.

• shade cover fabric must be wrought tight enough to prevent water from pooling atop the shade cover at any point.

• Canopy framing shall be touched-up on visible rusted or bare areas with existing color paint (desert tan) as needed with rust resistant primer and two coats exterior paint.

• Ensure all steel framing is touched-up (finished) with existing color (desert tan) paint with a 2.5 to 3.5 mil thick UV inhibited weather resistant powder coat.

3.8. Routine coordination conferences may be scheduled by the Contracting Office throughout the life of this contract. Coordination conferences may be held to discuss contract administration, contractor quality control, safety, phasing, scheduling, and other aspects relating to this requirement. The contractor shall be present for meetings when requested.

3.9. Contractor shall immediately (within 24 hours) notify 37 TRS for any SOW clarifications regarding a call that is placed.

3.10. Canopies owned by the 37 Wing within the JBSA location can be added into the BPA.

For structures with different dimensions not currently listed require the contractor to develop a price which the government will evaluate to be added to the price list.

4. QUALITY ASSURANCE: Quality assurance is an integral part of this BPA. The contractor shall ensure that all replacement services are carried out to the highest quality standards to meet manufacture’s specifications, requirements, and recommendations. The following quality assurance measures shall be implemented by the Contractor:

4.1. Standards and Specifications: The contractor shall adhere to industry best practices, manufacturer’s specifications, requirements., and recommendations. Materials used for replacement shall IAW the SOW and Attachment 3 measurements.

4.2. Inspection and Testing: Before commencing to replacing parts, the contractor shall conduct a thorough inspection of the fabric canopy cover and fasteners to ensure all damaged areas are identified and reported/verified to Government POC. Additionally, the contractor shall perform any necessary testing to verify the suitability of canopies' structural integrity.

4.3. Qualified Personnel: All replacement work shall be carried out by qualified and experienced personnel per manufacturer’s specifications, requirements and recommendations to meet warranty, safety and liability conditions. The contractor shall ensure their staff has the necessary expertise, training, and certifications to perform the required tasks effectively and safely to meet manufacture’s specifications, requirements, and recommendations. The contractor will not use military or civilian personnel to complete requirements. Contractor will use their own designated/qualified personnel to complete requirements.

4.4. Quality Control and Assurance Checks: Throughout the replacement process, the contractor shall implement quality control checks to verify that each step is completed correctly. This includes inspection of progress, validate workmanship, and address any issues promptly.

5. COMPLIANCE, SAFETY, AND STORAGE: The contractor shall strictly adhere to all relevant safety regulations and guidelines during the fabric canopy cover replacement process. The safety of personnel and the public shall be of utmost importance, and the contractor shall take necessary precautions to prevent accidents or incidents.

5.1. The contractor shall furnish and maintain adequate barricades, and warning signs, on the job site. The contractor shall clean up debris daily. The construction site shall always be kept clean, orderly, and safe for workers.

5.2. Contractor shall not work nor store materials in a manner that would hinder or otherwise block pedestrian/vehicle traffic in the immediate area particularly student access to/from and use of the adjacent shade cover.

5.3. Contractor is responsible for coordinating with government POC in regards to obstructions underneath canpoies that prevent contractor from providing the requirement.

5.4. The contractor is responsible to store and safeguard additional products used to replace shade structure fabric canopy covers to prevent delays.

5.5. The Contractor will not use government facilities to store materials/equipment.

6. WARRANTY AND GUARANTEE: The contractor shall provide a 1-year standard warranty or guarantee for the replacement services performed. The warranty period should be clearly specified in the BPA and cover any defects or issues arising from the replacement work during the specified timeframe.

7. INSTALLATION: The contractor will provide replacement of fabric shade canopy covers and strapping/anchoring materials in regard to attachment 3. The contractor will be responsible for any damages incurred during replacement of fabric shade canopy covers. Once the replacement and installation begin, the contractor will install all equipment in accordance with the manufacturer’s specifications, requirements, and recommendations, Federal, State, County and local codes and regulation ordinances, as well as IBC and specific industry and

The Contractor is responsible for ensuring all removed materials will be disposed of in a timely manner by the contractor IAW applicable Federal, State, local, and host military installation directives at no additional project cost to Government.

8. SITE PROGRAM MANAGER/POINT OF CONTACT (POC): The Contractor shall designate an on-site project manager as the Site POC for their individual projects. The Site POC or alternate(s) shall be available during normal duty hours and workweek (i.e., Mon – Fri) until project completion approval by Government.

9. PERFORMANCE LOCATION: Project work is to be performed at various locations at JBSA- Lackland Chapman Training Annex, and Camp Bullis. The areas have adjacent parking and accessibility in the surrounding areas. The contractor shall coordinate with the Government representative to gain base, facility and site access to the fabric shade canopies.

See attachment 3 for the building/location along with sizing of the fabric shade canopy covers.

10. WORK HOURS AND SPECIAL CONDITIONS: The contractor have access to the site and may work between the hours of 0600 and 1800 LT, Monday through Friday, exclusive of Federal Holidays. At the option of the government and with approval of the Contracting Officer, work may be permitted on Federal Holidays, weekends, or after 1800 LT at no additional cost to the government.

10.1. When the contractor desires to work other than standard times, a written request shall be submitted to the Government Acceptor for approval. Written approval from the Contracting Officer must be received before proceeding with the work. The request shall be submitted at least five (5) workdays prior to the proposed performance of work, and it shall indicate the day/days, time/amount of work, and reason why performance is necessary during nonstandard hours.

10.2. The contractor personnel shall not report to Government facilities to work nor remain at the work locations any time the Government is unexpectedly required to close their offices.

The contractor shall not be compensated for these Government closures. The contractor is responsible for all notifications of their contractor staff during times of closure.

11. SITE MAINTENANCE AND CLEAN UP: All shipping and packaging materials shall be removed by the contractor personnel. Contractor shall ensure all leftover wrappings, boxes, paper, and any other related refuse is picked up and properly disposed of off-site daily and by Government’s demand basis, as necessary. All debris, trash and refuse shall be removed from the site. At completion of project, area shall be left clean and in a final professional appearance.

All contractor generated refuse and waste shall be stored in a waste container provided by the contractor and hauled from the project site to a disposal area to be selected by the contractor and shall be located outside the physical boundaries of the installation. It is the contractor’s responsibility to tarp and or protect their dumpsters etc., at no additional cost to the Government. Recycle of project and packing materials is required. The contractor shall protect adjacent property, offices and their contents from dust, dirt, or other materials. Refuse disposal shall comply with the current JBSA-Lackland disposal environmental requirements. On-site dumpsters furnished by the contractor shall be coordinated with the Government 37 TRSS personnel.

12. OCCUPATIONAL SAFETY AND HEALTH: The Contractor shall establish a safety and occupational health program consistent with requirements of the Occupational Safety and Health Administration (OSHA), Title 29 CFR. The Contractor shall also provide Personnel Protective Equipment (PPE) and training consistent with applicable standards to contractor employees. The Contractor shall comply with all national and local fire codes and policies as needed to protect USAF resources (facilities, equipment and USAF personnel). The Contractor shall immediately report all mishaps involving serious personnel injuries, fatalities, and/or Government property/equipment damage occurring on the job to the responsible Safety Office and the Government representative(s). The Contractor shall fully cooperate with any Air Force (AF) led safety investigation and shall not disturb the mishap scene until proper officials authorize movement.

13. DELIVERY: Freight for the contractor.

13.1. Entrance to installation/base and facility will be coordinated through the Government representative(s).

13.1.1. Contractor shall be responsible for determination of the appropriate delivery method and handling, based on site conditions to include the existence loading docks and equipment at any given location.

13.1.2. The contractor shall be responsible for following all safety and security guidelines within the area in which work is being performed, to include securing any contractor owned tools or equipment, and any on site storage being utilized.

13.1.3. The contractor shall prevent its personnel from entering any area other than the designated work area to complete the project herein.

13.1.4. The contractor shall maintain a means of egress within all designated work areas to comply with fire codes.

14. QUALITY ASSURANCE /INSPECTION/ACCEPTANCE: The Government representative(s) shall inspect all work performed by the Contractor. The Government representative(s) will document all Quality Assurance inspections. Validation between the Contractor and the Government representative(s) that all products have been installed according to manufacturer’s guidelines and customer specifications. A pre-final and final punch list walkthrough inspection (s) shall be coordinated with the contractor’s authorized representative and Government Inspector/Acceptor.

14.1. The contractor shall, without additional charge or cost to Government, replace work found by the Government not to conform to contract requirements, unless in the public interest the Government consents to accept the work with an appropriate adjustment in contract price. The Contractor shall promptly segregate and remove rejected material from the premises.

14.2. Unless otherwise specified in the contract, the Government shall accept, after completion and inspection of all work required by the contract or that portion of the work the CO determines can be accepted separately. The contractor shall notify the Government representative(s), will conduct a final inspection with the contractor, to determine if the project can be accepted or if corrections are required to be completed prior to final acceptance. Acceptance shall be final and conclusive except for latent defects, fraud, gross mistakes amounting to fraud, or the Government’s rights under any warranty or guarantee.

15. RECORDS MANAGEMENT: The contractor shall manage all data created for Government use or legally controlled by the Government, in support of the functional activity or required by AF publication, IAW with the AF Records Management procedures in Air Force Instruction (AFI) 33-322, Air Force Manual (AFMAN) 33-363 and Air Force Instruction (AFI) 33-364 (or their subsequent replacements).

16. SECURITY REQUIREMENTS, CLEARANCE and BADGES: All installation personnel assigned to this requirement shall be a United States citizen, shall have a Local Area Check (LAC) conducted by the contractor and shall identify possible security threats to the installation Security Forces. The contractor shall provide the Government with the names, social security numbers, driver’s license numbers and state of issue, date of birth, and country of birth of personnel performing on this task before they will be allowed on the installation.

This information shall be provided at a minimum of 72 hours prior to performance start date for the Government representative(s) to coordinate installation access for contract personnel.

Contract personnel not meeting the minimal standards will not be issued an installation pass and will not be allowed access to the installation.

16.1. Personnel employed by the Contractor and/or their subcontractors must get a pass for their privately owned vehicles at the installation Visitor Reception Center, with proof of following:

16.1.2. Current License Plates

16.1.3. Current State Inspection Sticker (If required)

16.1.4. Valid State Driver License

16.1.5. A phone number for sponsor on base

16.2. Vehicles owned by the contractor with the company name permanently printed on them are not required to obtain a pass if a current work order is presented at the time of entry, however, current liability insurance, state inspection sticker and registration is required. The person driving the vehicle must have a valid operator license for the type of vehicle.

16.3. Upon completion or termination of the agreement or expiration of the identification passes, the contractor shall ensure that all base identification passes issued to contractor employees are returned to the issuing office. If a contractor employee has been terminated, the pass will need to be retrieved and returned to Security Forces so that employee does not have base access. If the pass is not retrieved, then SF will need to be notified so base access is not allowed. Upon completion of the contract return all issued installation access passes to the issuing authority. Failure to comply with these requirements may result in withholding of final payment.

16.4. The Contractor is required to provide identification badges for its employees. All contractor personnel shall wear these badges while on duty on the Government site.

Badges are required to identify the individual, company name, and be clearly and distinctly marked as contractor. Size, color, style, etc. are to be mutually agreed upon by contractor and Government POC.

17. COMPLIANCE WITH LAWS AND REGULATIONS: The service provider shall be knowledgeable of and shall comply with all applicable federal, state, and local laws, permits, Department of Defense (DoD), AF, and base requirements and instructions required for the performance of the duties in this SOW. The service provider shall ensure policies and procedures are established that protect the safety and welfare of customers, employees, and the community to minimize or eliminate safety or environmental pollution risks.

18. ORDERING PROCESS:

18.1. Request Initiation: Whenever replacements are required Authorized BPA Caller shall submit a call to the contractor. The request shall include details of the damaged shade structure fabric canopy cover size, location, and any other relevant information.

18.2. Authorized BPA Callers are allowed to place up to $250,000.00 per authorized call.

The contractor can only accept calls placed by callers designated on the Authorized Caller’s List. All other personnel, attempting to make calls shall be designated to the Customer POC for further assistance.

18.3. Work Execution: Upon receiving approval by Government, the contractor shall
