For
CAGE AND RACK WASHER
59TH MEDICAL WING, JOINT BASE SAN ANTONIO, TEXAS
(ONE-TIME PURCHASE REQUEST)

STATEMENT OF WORK
For
CAGE AND RACK WASHER
SCOPE OF WORK: The contractor shall provide one (1) Cage and Rack Washer unit (Girton Model 80-0 or Equivalent). The contractor shall provide all management, personnel, licenses, tools, supplies, equipment, materials, transportation, shipping, labor and any other incidentals required to provide and install a Full Turnkey solution in accordance with Attachment 1 below (Salient Characteristics and Safety Features) for the 59th Clinical Investigations & Research Support 1100 Wilford Hall Loop, Bldg. 4430 JBSA-Lackland AFB, TX 78236. The hours of operation are Monday through Friday from 07:30 A.M. - 05:00 P.M.
1. DESCRIPTION OF REQUIREMENT.
1.1. Installation. The contractor will adhere to all regulations affiliated to maintain commercial and healthcare building code compliance standards that fall under National Fire Protection Association (NFPA), Occupational Safety and Health Administration (OSHA) while working on the premise. Installation of the washer shall conform to all manufacturer specifications along with the same practicing standards by the contractor to include any commercial regulations governed by Local/State Law for each utility identified within the requirement.
1.1.2. Installation will include the removal of the existing washer, all packing, shipping material, removal of any waste, and any byproducts or debris created from any actions generated during installation. The Government will be responsible for salvaging after contractor removes the old washer.
1.1.3. The contractor shall have the proper personnel, to include but not be limited to electrician and engineering specialists, on site for the installation due to the high voltage output during the disconnection and reconnection to main the power supply line(s).
1.1.4. All equipment, components and any incidentals will be supplied by the contractor to include any associated mechanical and electromechanical parts and supplies at no additional cost to the government.
1.1.5. At no point during the project shall the contractor adjust, reconfigure, or alter the interior of any walls, ceilings, or flooring for the installation. Any pipes, cable, wiring, etc. shall be replaced at Contractor expense using the current fixtures, openings, pathways through walls/floors to support any steam, water, electrical, or heat output but no alternations shall be made for which do not already exist with the facility. Refer to Attachment 2 below for photos of current location conditions. Any alterations provided by the Contractor shall be at no additional cost to the Government. Cage and rack washer shall be a Turn-key solution; this is mandatory.
1.1.6. The contractor shall clean-up the designated work area and dispose of any byproducts from the installation, to include any debris from any work performed within the vicinity.
1.1.7. Government provided badges (refer to paragraph 3) shall be worn on clothing above the belt-line while of the premise. All badges will be turned into the Facility Management Office once the contractor has completed all tasks outlined below:
1.1.7.1. Contractor shall ensure the unit has and can maintain a heavy-duty hydro spray wash that will thoroughly and efficiently clean at a large capacity.
1.1.7.2. Perform a final clean-up of any remaining packing, shipping material. All contractor generated debris shall be removed by the contractor and disposed of off the premises.
1.1.7.3. Contractor is responsible for providing relevant Government Personnel all Operator and Maintenance Training for end users and maintenance personnel assigned.
1.1.7.4. Following contractor(s) completion of all work outlined, the project lead will coordinate a walkthrough inspection with a representative from Facility Management the Clinical Engineering Flight and the end user to ensure no malfunctions/errors exist.
1.1.7.4.1. Any malfunctions that occur will be corrected immediately upon discovery at no additional cost to the government if the reason for the malfunction was due to improper installation or contractor negligence.
2. WARRANTY. The contractor shall include and provide technical and site support for one full year to cover any defective parts and replacement. Warranty shall also cover all labor necessary to provide this warranty at no additional cost to the government. The Government will not accept any refurbished parts or replacement parts/pieces; all items provided by the contractor or any contractor’s subcontractors shall be new off the shelf products/items and the responsibility of the contract to provide under this warranty.
3. CONTRACTOR PERSONNEL. All contractor personnel will report to and sign-in at Keys & Badges, 1100 Wilford Hall Loop, Bldg 4554, Room# 0J047, located at the ground level to receive a temporary badge prior to beginning any work. Badges will be worn above the belt-line for the duration of the installation.
3.1. Bass Passes. Contractor personnel shall obtain base passes for entry onto Joint Base San Antonio -Lackland AFB in compliance with AFI 10-245, Antiterrorism (AT), 21 September 2012.
3.1.1. Base passes can only be issued for a maximum of 180 days and must be renewed every 180 days thereafter.
3.1.2. A minimum of 15 days’ notice is required to obtain base passes. The contractor must submit the following pertinent information to the 59th Clinical Engineering Flight point of contact no later than 15 calendar days before expected work date or pass expiration date to Mrs.

*NOTE. All Contractor Personnel must have a valid driver’s license and have on their person when retrieving the base pass at JBSA – Lackland AFB’s Visitor’s Center located at the Luke gate entrance. Failure to do so will result in delay or denial to enter the premises.
3.1.3. Contractor personnel are not authorized entry to Joint Base San Antonio – Lackland AFB if they have been convicted of any of the following offenses:
a. Illegal sale, distribution, or manufacture of explosives or weapons.
b. Arson
c. Importation or manufacture of a controlled substance.
d. Extortion
e. Bribery
f. Kidnapping or hostage taking
g. Sexual assault
h. Armed Robbery
i. Illegal possession or use of an explosive or weapon
j. Distribution or intent to distribute a controlled substance.
k. Conspiracy or attempt to commit any of the above offenses.
l. Two or more felony convictions of any type
m. or have any outstanding warrants for their arrest.
4. ACCIDENT REPORTING. The contractor shall report all occupational exposures and all accidents while on performing the requested services while on 59th Medical Wing compound resulting in death, trauma, or occupational disease to the Government point of contact. All accidents shall be reported immediately. The Government point of contact may require the contractor to complete certain forms to document exposures and/or accidents. The contractor shall complete these forms promptly, at no additional cost to the government.
4.1.1. Emergency Health Care. The Medial Treatment Facility (MTF) will provide emergency health care for contractor providers for injuries incurred while on duty in the MTF. These services will be billed to the contractor at the current full reimbursement rate.
4.1.2. Traffic Laws. The contractor shall comply with the installation traffic code as specified in Texas State Law and Air Force Instruction (AFI) 31-218_IP, Motor Vehicle Traffic Supervision, and may be subject to search prior to authorization to proceed through gate.
4.1.3. Weapons, Firearms, and Ammunition. Contractor personnel are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately-owned vehicle while on JBSA – Lackland AFB.
4.1.4. Smoking in Air Education and Training Command (AETC) Facilities. Contractors are advised that the (AETC) Commander has placed restrictions on the smoking of tobacco products in AETC facilities. AFI 40-102, Tobacco Use in the Air Force (current edition), outlines the procedures used by the commander to control smoking in our facilities. Contractor personnel and visitors are subject to the same restrictions as government personnel. Smoking is permitted only in designated smoking areas. The hospital campus is a 100% smoke free zone.
4.1.5. Physical Security. The contractor shall be responsible for safeguarding all government property provided for contractor use. At the end of each work period, all government equipment and materials will be secured.
5. GENERAL DEFINITIONS. The following terms shall have the meaning set forth in accordance with the terms of this SOW.
5.1. COMMANDER. MTF Commander, Flight Commander or other leader or a designated representative, e.g., Department Chairperson of the activity designated in this contract.
5.2. CONTRACTOR. The offeror identified in block 15A of Standard Form 33. The contractor is an independent contractor and responsible for its own liability.
5.3. MEDICAL TREATMENT FACILITY (MTF). Air Force hospitals or clinics, including all activities providing outpatient and/or in-patient healthcare services for authorized personnel.

SALIENT CHARACTERISTICS & SAFETY FEATURES

Brand Name: Girton Model 80-0 Cage and Rack Washer
Any item other than the brand name item shall meet all the below Salient Characteristics and Safety Features to be considered a Technically Acceptable equivalent item (refer to RFQ, clause 52.212-2). Any item, other than the brand name item, unable to meet the below aspects shall be considered Technically Unacceptable.
Salient Characteristics:
a. Washer must fit into existing pit dimensions, no modifications to existing pit or floor will be allowed, and there will be no other facility alterations approved for this requirement.
Item shall be a turn-key solution.
b. Washer must be provided with an oscillating header system that provides full coverage;
the use of rotary spray arms, pivoting spray arms, or alternating side-to-side spray devices shall not be used. Contractor shall ensure the unit has and can maintain a heavy-duty hydro spray wash that will thoroughly and efficiently clean at a large capacity.
c. Sump area of washer must feature an automatic flush to remove debris from sides of the sump tank area.
d. The interior of wash cabinet must employ full-length panic cables on both sides of the wash chamber; this safety device immediately disables the washer if cable is pulled.
e. The washer must employ stationery jets beneath the floor grid to wash the underside of racks.
f. Circulating pump shall be vertical mounted and total draining.
g. Horizontal pumps that retain fluids in the pump head shall not be utilized; this eliminates water carry over between cycles, and cross contamination.
h. All interior flexible connections used in the water supply shall be sanitary food grade with no threads; this eliminates areas where cross contamination can accumulate.
i. All steam, condensate and water piping, both internal and external must be stainless steel schedule 40 pipe; no carbon steel or copper shall be used which is susceptible to corrosion.
j. A built-in service diagnostic program, accessible by service code, shall be included/provided and displayed to permit system calibration and verification of satisfactory component operation.
k. The unit must be provided with a door on the discharge (clean) end for pass-thru operation, complete with safety switch and lights indicating washer is in operation or the cycle is complete.
l. A 16" x 18" (minimal size, a larger widow is acceptable) safety glass viewing window must be supplied in each door for visual inspection.
m. Compartment size door to door dimension: 48" wide x 88" high x 100" long (variation of
1.0 inch).
n. Any item must include a 1-year warranty from date of complete installation for all defective parts and shall include all associated costs for replacement and labor.
Safety Features
a. The opening of the door will stop the operation of the machine. Door must then be closed and the start button depressed to continue operation.
b. Door is equipped with spring loaded, explosion relief type latches that open readily when pushed from inside the cabinet. The use of emergency exit hardware from within the wash chamber shall not be employed.
c. Dual stainless steel safety cables are installed inside the wash compartment on both sides of the wash area. The cables are installed approximately 3' above the floor and run the entire length of the cabinet. If the cable is pulled, the machine will immediately cease all operations. To restart operations, the power must first be turned on again and the start button depressed.
