REQUEST FOR QUOTE (RFQ)

MEDICAL MATERIEL TECHNICIAN

JOINT BASE SAN ANTONIO (JBSA) CAMP BULLIS, TX

1. Solicitation Number: FA3016-20-U-0043 is a solicitation, issued in accordance with (IAW) Federal Acquisition Regulation (FAR) Subpart 12.6 Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR Part 13 Simplified Acquisition Procedures, and supplemented with additional information included in this notice. This solicitation incorporates provisions and clauses in effect through:

This purchase is restricted to Service Disabled Veteran Owned Small Business (SDVOSB), the North American Industry Size Classification Industry System (NAICS) is 561210 Facilities Support Services. Small business size standard: $41.50M

2. Requirement. IAW the Performance Work Statement, (PWS) (Atch03), the 502d Air Base Wing through the 502d Contracting Squadron, Joint Base San Antonio (JBSA) Randolph has a commercial requirement for non-personal services for one (1) Medical Materiel Technicians

(MMT).

3. Period/Place of Performance. Non-personal services are required for a period of twelve months (12) at the Medical Readiness Training Center, 23400 Marne Road, and Bldg. 6350 JBSA Camp Bullis 78257.

4. Mandatory Registration. To be eligible for award, registration with the System for Award Management (SAM) must be current. There are no exceptions. To register or apply please visit the SAM website at http://www.sam.gov. If you are not registered, you may request an application at

(866) 606-8220 between the hours of 6 am – 8 pm (Eastern Time). SAM registration must be completed prior to award.

5. Schedule. The quoter must provide a firm fixed price quote for the requirements herein. The quote shall include a unit, extended price, and total prices for the contract line item numbers (CLIN), see attachment 02.

6. Solicitation Questions. All questions must be in writing via e-mail to Carmen Aguirre carmen.aguirre.1@us.af.mil and Teresa Calavera teresa.calavera@us.af.mil. Reference, “Solicitation FA3016-20-U-0043, Medical Materiel Technician” in the email subject line. Answers to questions will be made accessible to all potential quoters.

7. Instructions to Quoters. Addendum to FAR Provision 52.212-1, Instructions to Quoters

– Commercial applies to this acquisition. As prescribed in FAR 12.301(b)(1), the following addendum is incorporated to this solicitation and hereby amends any language herein:

a. Quotes must be valid for 60 days after closing date of RFQ.

b. Email quotes, attachments, and all other correspondence as listed in this solicitation to both points of contact (POCs).

c. Late responses will be processed IAW FAR 52.212-1(f) Instructions to Offerors -- Commercial Items, late submissions, modifications, revisions, and withdrawals of offers.

d. Interchanges may be conducted with one (1), some, or all quoters as the Government is not required to conduct interchanges with any and/or all quoters responding to this RFQ.

8. Special Notes and Instructions. Only quotes submitted by Service Disabled Veteran Owned Small Business (SDVOSB) will be accepted. Quoters are cautioned that clauses and provisions within this RFQ may include blocks that must be completed prior to submission of the quote.

Addendum to FAR 52.212-1 Instructions to Offerors -- Commercial Items (Oct 2018) is incorporated by reference. This addendum changes terminology in FAR Provision 52.212-1 from "offerors" to "quoters" and from "offers" to "quotes"

The following paragraphs of this provision have been tailored.

52.212-1(b) Submission of quotes: Quotes may be submitted on vendor's stationary and in accordance with industry standard.

The quoter is required to agree with all terms, conditions, and provisions included in the solicitation.

Quotes that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.

52.212-1(c) Period for acceptance of quotes. The quoter agrees to hold the prices in its quote firm for sixty (60) calendar days from the date specified for receipt of quotes.

52.212-1(f) Late submissions, modifications, revisions, and withdrawals of quotes. Quoters are responsible for submitting quotes, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in the solicitation by the time specified in the solicitation.

The following paragraphs of this provision are not applicable.

The following paragraphs are added to this provision.

(m) The following factors shall be used to evaluate quotes. Quotations should contain the quoter’s best terms on price and technical capability.

(1) Price. To be awarded a purchase order, the total price must be found to be fair and reasonable.

(2) Management Staffing Plan.

Evaluation -- Commercial Items: The provision at FAR 52.212-2, Evaluation – Commercial Items, is not used in this solicitation. Pursuant to FAR 12.602, contract award will be made using Simplified Acquisition Procedures in accordance with FAR 13.106. Quoter's submissions will be evaluated as outlined below:

1. The government will award a purchase order resulting from this solicitation to the responsible quoter whose quote conforming to the solicitation represents the best value to the government, price and other factors considered.

2. The government is not assigning a relative importance to each evaluation factor and intends to conduct a comparative evaluation of quotes in accordance with FAR 13.106- 2(b)(3). All timely quotes received will be evaluated. Award decision will be based on an overall subjective evaluation of the quotes. The quoter that provides the best solution to meet the requirements of the PWS that is found to be fair and reasonably priced, and is found responsible will be awarded the purchase order.

3. The following factor will be used to evaluate quotes.

A. Price. To be awarded a purchase order, the total evaluated price must be found to be fair and reasonable. Adequate competition is anticipated so quoters are encouraged to quote their best pricing with available discounts.

B. Management Staffing Plan. Quoters are required to provide a management/staffing plan addressing how they will manage personnel to meet the requirements of the contract.

This plan must include recruitment, hiring and development strategies for maintaining 100% coverage. IT should also state how personnel would meet and/or exceed the required skills and qualifications noted in the PWS. An "acceptable" or "unacceptable" rating will be used to evaluate the quoter's management staffing plan. See Table 1 for the rating descriptions. The technical factor and all sub factors must be rated "acceptable" to be eligible for award. The technical plan shall address the following sub factors:

Rating Description Acceptable Quote meets the minimum requirements of the solicitation.

Unacceptable Quote does not meet the minimum requirements of the solicitation.

4. Once all quotes are evaluated on the above factors, the government evaluators will consider all ratings and determine which quote will provide the best value to the government in fulfilling this need.

5. Interchanges. The government reserves the right to hold interchanges if, during the evaluation, it is determined to be in the best interest of the government. Interchanges are fluid interaction(s) between the contracting officer and the vendor that may address any aspect of the quote and may/may not be documented in real time. Interchanges may be conducted with one, some, or all, as the government is not required to conduct interchanges with any and/or all quoters.

6. A vendor must be determined to be responsible by the contracting officer in order to be eligible for award. Responsibility is described in Federal Acquisition Regulation (FAR) Subpart 9.1., "Responsible Prospective Contractors." Part of the determination address financial capability. Please provide the names, addresses, and points of contact for all financial institution organizations utilized to determine if the vendor has adequate financial resources to perform the contact or the ability to obtain them to include written authority to contact the provided financial institution.

9. Attachment 1 contains FAR/DFARS provisions and clauses are applicable to this Solicitation. (For full text references, www.acquisition.gov). Include FAR Provision 52.212-3, Offeror Representations and Certifications -- Commercial Items, with your quote.

FAR Provisions 52.212-1 and 52.212-2 addenda are included. FAR Clauses 52-212-4 and 52.212-5 are included. FAR 52.212-4 contains an addendum in the list of clauses and provisions. The clause at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items, and the associated addendums found within this solicitation, are applicable to this acquisition.
