PERFORMANCE WORK STATEMENT (PWS)

MEDICAL READINESS TRAINING CENTER (MRTC)

MEDICAL MATERIEL TECHNICIAN

JBSA-CAMP BULLIS, SAN ANTONIO, TEXAS

1. AGENCY BACKGROUND

The United States Air Force Medical Service (AFMS) requires military trained personnel capable of rapid planning, coordination, and execution of a broad array of combat medical tasks as part of their organizational mission. The Medical Readiness Training Center (MRTC) is located at Joint Base San

Antonio (JBSA)-Camp Bullis in San Antonio Texas. The MRTC courses taught at JBSA-Camp Bullis include:

1.1 Expeditionary Medical Support (EMEDS): Designed to support Air Expeditionary Forces (AEFs) during major combat, contingency, Humanitarian Assistance (HA), and Disaster Relief (DR), Defense Support of Civil Authorities (DSCA), and stability operations.

1.2 Ground Surgical Team (GST): Trains small teams to operate in austere environments.

1.3 Aeromedical Evacuation & Patient Staging Course (AEPSC): Provides staging capability for patients. Interfaces between Medical Treatment Facilities (MTF) and Air Evacuation (AE). The goal is to keep attached MTF beds clear. The focus is on movement/treatment of patients before/during AE transport.

1.4 Expeditionary Medical Readiness Course (EMRC): Basic level, initial medical field training for all

3-level students about to enter tech school.

1.5 Patient Decontamination (DECON) Training: An exercise for patient decontamination in peace-time or war-time setting.

2. DESCRIPTION OF SERVICES

This is a non-personal service requirement that requires one (1) Medical Materiel Technician (MMT) to work as a full-time employee in the simulated contingent warehouse environment alongside of active duty military personnel. The MMT provides logistical support to twenty-three (23) training buildings and manages $60 million in equipment and supplies.

3. SCOPE

The contractor shall provide services required to perform under this contract. Contractor care shall cover the same range of services provided in a civilian medical warehouse facility.

The Contractor personnel will support the internal and external customers' medical materiel needs, ensuring the integrity of the inventory system-Defense Medical Logistics Standard Support (DMLSS), and aiding all logistical support functions to AFMS personnel assigned to EMEDS, GST, DECON, AEPSC, and

EMRC courses.

4. PERIOD OF PERFORMANCE

The period of performance (PoP) shall be for one (1) base year of twelve (12) months.

5. REQUIRED SUBJECT KNOWLEDGE AND CONCEPTS

The MMT must be proficient in general warehousing tasks, customer support, requisition, shipping, receiving, inventory management, assembling medical kits, Quality Assurance (QA) and cargo preparation and deployment. To include medical controlled items stored in a secure location, operation of various materials handling equipment and utilizing/ updating records and maintaining adequate inventory levels of each training building on the compound.

The work environment involves moderate risks or discomforts which require special safety precautions, such as working around moving warehouse equipment, carts, or machines. Contractor personnel must be comfortable working around with Live Patient Models (LPMs). Currently, swine are utilized as the LPMs but the type of animal utilized may be subject to change depending on written protocol.

5.1 Physical Requirement and Work Environment: Job requires physical capabilities due to conditions of simulated contingent training environment.

5.1.1 Ability to work in a non-climate controlled environment.

5.1.2 Must wear appropriate safety clothing and shoes. This includes a need for a safety harness, safety glasses, lift belts and steel toe shoes etc.

5.1.3 Ability to sit, bend and stand for extended periods of time.

5.1.4 Ability to walk over rough, uneven, or rocky surfaces; recurring bending, crouching, stooping, stretching, reaching; or similar activities.

5.1.5 Ability to frequently lift up to 50lbs. from ground to waist level.

5.1.6 Ability to pushing loaded carts of medical supplies (weight in excess of 100lbs).

5.1.7 Work requires stocking supplies in the primary inventory area as well as numerous secondary inventory points throughout the training compound.

5.1.8 Work requires maintaining a clean environment while performing tasks that can produce dust or other infectious pollutants while transitioning between environments.

5.2 Administrative Duties- Perform work in a wide range of systematized logistical operations, such as performing recordkeeping functions in inventory, storage, cataloging, receipts and control processes.

5.2.1 Operate computer terminals to perform records search, data input, and data corrections.

Search catalogs, records, or other data sources to obtain or correct stock numbers or to find authorized substitutes for stock items.

5.2.2 Maintain an accurate accounting and reporting system for non-expendable property and perform standardized property management control processes.

5.2.3 Monitoring the paperwork and records associated with receiving, storing, issuing, replenishing, or inventorying commonly used administrative supplies and equipment.

5.3 Inventory Management- Monitor and evaluate inventory. Properly process gains and losses, manage operating inventory, perform counts, research discrepancies, process adjustments, and perform transaction history inquiries. Attention to detail necessary to adhere to quality standards

5.3.1 Proficient in Defense Medical Logistics Support System (DMLSS).

5.3.2 Demonstrate knowledge of properly handling controlled medical items according to the

Drug Enforcement Administration (DEA) requirements.

5.3.3 Monitors expiration dates, rotates stock to limit outdates and properly dispose materials according to OSHA standards.

5.3.4 Delivers supplies in a timely manner and ensures supplies and material delivered accurately and in proper working order.

5.3.5 Continuously identify supplies and equipment not utilized to be turned into Defense

Reutilization Management Office (DRMO).

5.4 Communication- Effectively communicate in person or via electronic devices (telephone or email) with co-workers, customers, and the general public.

5.4.1 Ability to communications using ground radio systems, commercial phones, satellite communication system and local area networks.

5.4.2 Demonstrate ability to properly manage customer accounts and articulate discrepancies when they arise.

5.4.3 Ability to take direction and adapt to changing situations/job duties.

5.4.4 Demonstrate ability to recognize combat stress and control processes.

5.4.5 Ability to coordinate activities closely with others in stressful situations.

5.4.6 Summarize basic soldiering skills including grid coordinate plotting, and camouflage of medical assets, defense of medical assets.

5.4.7 In addition, duties shall include but are not limited to: working independently, coordinating logistics activities with other base organizations, units and government agencies, ensuring the overall readiness and availability of equipment/supplies to include general medical materiel management and warehouse activities.

5.5 Equipment- Demonstrate safe usage and proficiency of all equipment listed in the allowance standards in accordance with (IAW) Air Force Occupational Safety and Health & Occupational

Safety Health Association (AFOSH/OSHA) standards, and manufacturer’s guidelines and operator’s manuals.

5.5.1 Required to maintain a valid state driver’s license.

5.5.2 Must follow safety procedures and regulations, including wearing special protective gear and disposing of materials as regulated by law.

5.5.3 Physically moves materials and uses heavy equipment to move materials such as maneuvering a manual pallet jack and operating a 6k and 13k powered forklift both inside and outside of facilities. Forklift operator certification required.

5.5.4 Identify steps involved with basic medical logistics in deployed location (i.e.

requisitioning, accountability, and proper utilization of supplies/equipment).

5.5.5 Demonstrate ability and knowledge of acquisition and equipment management.

5.5.6 Monitor and manage vehicles for MRTC to include maintenance and program management.

5.5.7 Familiarization of Hazard Material (HAZMAT) policies and procedures.

5.5.8 Perform casualty movement using North Atlantic Treaty Organization (NATO) litters and gurneys; safe loading and unloading procedures for vehicles in the deployed location to include Modular Ambulance, Highly Mobile Multipurpose Vehicle (HMMWV).

5.5.9 Demonstrate Knowledge of Operational Security (OPSEC), Communication Security

(COMSEC), and Emissions Countermeasures (EMSEC).

5.6 Education: Shall have a High School Diploma or a General Education Development (GED) and prior experience as a medical materiel technician in a contingency warehouse setting for at least one year.

5.7 Certifications: Forklift operator certification required. Must obtain an on-the-job (OJT) training within one (1) month of hire date. OJT will be provided by government personnel.

5.8 Experience: Prior medical logistics experience is required and deployment experience is preferred.

5.9 Changes in personnel: Requests for changes in personnel shall be submitted to the COR and

Contract Officer (CO) at least fifteen (15) calendar days prior to making any permanent substitutions.

6. ADDITIONAL QUALIFICATION

6.1 Employees (General): The Government has the right to restrict the performance under this contract of any contract employee, or prospective contract employee, who is identified as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population.

6.2 Government Operated Vehicle (GOV) License: Contractors will receive training to be issued a

GOV license. Vehicles include but are not limited to: ambulance, flatbed truck, High Mobility

Multipurpose Wheeled Vehicle (HMMWV), Light Medium Tactical Vehicle (LMTV). The training and test to obtain a GOV license will be provided by the Government at no extra expense to the Contractor.

7. GOVERNMENT FURNISHED PROPERTY, EQUIPMENT, AND SERVICES

The Government will provide use of all equipment, computers, simulation models, live patient models, medical consumables, individual protective equipment and transportation within the MRTC training compound. The Contractor shall keep Government furnished supplies, equipment, and work areas in a safe, orderly and clean condition. The Government may provide voluntary specialty training to the contractor.

Additionally, the Government will provide local telephone service, Class-A telephone lines and defense switching network lines limited to matters related to the performance of this contract. Personal long distance calls are not authorized.

8. TRAVEL

The contractor shall be required to travel between all Joint Base San Antonio installations to attend meetings, conferences, and training IAW the mission of the MRTC. Contractor Personnel must have a valid

U.S. driver’s license. The government shall provide an initial training to drive a government vehicle.

9. GENERAL INFORMATION

9.1 Quality Control. The contractor shall be responsible for quality control for all work accomplished during the performance of the task. The contractor shall provide and maintain a Quality Control

Plan (QCP) which ensures the requirements in this PWS are met. The QCP will be considered acceptable and approved unless the contractor is notified by the CO prior to award. The QCP shall be included in the technical proposal for evaluation and acceptance. As a minimum, the Contractor shall develop quality control procedures that address the areas identified in Section 13, Service

Summary.

9.2 Quality Assurance. The government will periodically evaluate the contractor’s performance by appointing a COR’s to monitor performance to ensure services are received. The COR will evaluate the Contractor’s performance in accordance with the Quality Assurance Surveillance Plan (QASP) for this requirement.

9.3 Government Remedies. The CO shall follow the requirements of Federal Acquisitions Regulation

(FAR) 52.212-4, Contract Terms and Conditions for Commercial Items (May 1997), for contractor’s failure to correct non-conforming services.

9.4 Hours of Operation. The Contractor shall provide qualified and trained personnel to cover the required hours of operation. The MMT will work Monday to Friday from 0730 to 1630. Any deviation to these days and times will be only as authorized by the COR or CO. The tasks to recruit, qualify, and place the MMT are not severable; therefore, the time to recruit, qualify, and place the

MMT starts from the first day of the PoP. The Contractor shall have all employees ready to report by fifteen (15) calendar days after contract award. If this date falls on a weekend or holiday, it will move to the next business day. The Contractor shall coordinate with the COR for all in-processing actions.

9.5 Place of Performance: The work to be performed under this contract will be performed at JBSA-

Camp Bullis San Antonio, Texas.

9.5 Recognized Holidays: Contractor personnel will not be required to work on recognized holidays.

Federal law (5 U.S.C. 6103) establishes the public holidays listed in these pages for Federal employees. Please note that most Federal employees work on a Monday through Friday schedule.

For these employees, when a holiday falls on a non-workday -- Saturday or Sunday -- the holiday usually is observed on Monday (if the holiday falls on Sunday) or Friday (if the holiday falls on

Saturday).

9.6 Federal Holiday List (10 calendar days)

9.7 Absences. The Contractor shall provide replacement personnel within one (1) working day to ensure that absences do not interrupt service performance. If the Contractor does not provide replacement personnel, the government will take action IAW FAR Clause 52.212-4, Contract

Terms and Conditions – Commercial Items.

9.8 Replacement Personnel. The Contractor shall be responsible for administering the absences for individuals filling the required positions. The Contractor shall have adequate Medical Materiel

Technician available in such instances so a lapse in coverage does not occur. All work schedule changes shall be submitted to the Quality Assurance Personnel (QAP) at least 48 hours in advance.

9.9 If the Contractor is prevented from performing due to acts beyond the Contractor's control including emergency base closure IAW FAR Clause 52.212-4(f) Contract Terms and Conditions-

Commercial Items- Excusable delays, and or FAR Clause 52.249-8 Default (Fixed-Price Supply and Service) as appropriate will be applied to determine if the non-performance is excusable. In

P a g e 7 | 12 the event of an emergency base closure due to weather or other contingency situation the CO will notify the Contractor of any changes in performance requirements. The Contractor shall be responsible for instructing contracted personnel concerning their duty status. Under no circumstances will the Contractor be paid for services not rendered.

9.9 Services performed under this contract have been determined not to be essential for performance during crisis declared by the National Command Authority.

9.10 The Government will restrict the performance on this contract by any individual who is identified as a potential threat to the health, safety, security, general well-being, or operational mission.

9.11 The Contractor shall not use Government facilities or other Government property for personal use or other business not related to this contract.

10. CONTRACTOR REPRESENTATIVE

The Contractor shall identify the name and telephone number of the Contractor’s point of contact to the

CO in writing prior to beginning performance. The Contractor shall identify in the designation letter any limitations on its representative's authority to act on behalf of the Contractor. The Contractor shall provide an updated designation letter whenever any changes occur. The Contractor representative may be required to meet with the Government during the performance of this contract at the request of the CO.

10.1 The Contractor Representative shall respond to a request to meet with the CO within twenty-four

(24) hours of notification under routine circumstances.

10.2 Contractor shall not respond to any media inquiries nor provide interviews, comments, or any other responses to the media regarding any subject related to this contract. All inquiries or complaints from the media or other sources shall be immediately relayed to the COR.

10.3 The Contractor shall ensure that its employees conduct themselves in a professional manner while on the installation and refrain from disruptive, offensive, or otherwise improper behavior that undermines order and discipline. The CO may direct the Contractor to remove from performance of this contract on this installation any Contractor employee engaging in such misconduct.

10.4 Smoking in Air Force (AF) Facilities. Contractors are advised that the AF has placed restrictions on the smoking of tobacco products in AF facilities. IAW AFI 40-102, Tobacco Free Living, outlines the procedures used by the commander to control smoking in our facilities. Contractor employees and visitors are subject to the same restrictions as government personnel. Smoking is permitted only in designated smoking areas.

10.5 Joint Base San Antonio (JBSA) Traffic Code. All applicable Traffic Codes will be enforced and apply to all personnel operating vehicles on a JBSA installation. The governing instruction for all

Traffic Codes is IAW AFI 31-218 Motor Vehicle Traffic Supervision, and AFMAN 31-116 Air

Force Motor Vehicle Traffic Supervision.

10.6 Cellular Phone Usage. Contractors are advised that DoD Instruction (DoDI) 6055.04 prohibits cellular telephone usage when approaching, entering or exiting any installation gate. Cellular phone usage by drivers on installations is prohibited in moving vehicles unless used with a hands-free

P a g e 8 | 12 device. The installation Security Forces Squadron (SFS) is strictly enforcing this regulation. Those found in violation of this regulation are subject to be ticketed.

10.7 Post Award Conference/Periodic Progress Meetings: The Contractor agrees to attend any post award conference convened by the contracting activity or contract administration office IAW FAR

Subpart 42.5, Postaward Orientation. The CO, COR, and other Government personnel, as appropriate, may meet periodically with the Contractor to review the Contractor's performance. At these meetings the CO will apprise the Contractor of how the government views the Contractor's performance and the Contractor will apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the government.

10.8 Contracting Officer Representative: The COR will be identified by separate letter. The COR monitors all technical aspects of the contract and assists in contract administration The COR is authorized to perform the following functions: assure that the Contractor performs the technical requirements of the contract: perform inspections necessary in connection with contract performance: maintain written and oral communications with the Contractor concerning technical aspects of the contract: issue written interpretations of technical requirements, including

Government drawings, designs, specifications: monitor Contractor's performance and notifies both the CO and Contractor of any deficiencies; coordinate availability of government furnished property, and provide site entry of Contractor personnel. A letter of designation issued to the COR, a copy of which is sent to the Contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates or changes in delivery dates. The COR is not authorized to change any of the terms and conditions of the resulting order.

11. SECURITY:

11.1 The Contractor shall comply with IAW AFI 71-101, Volume 1, Criminal Investigations Program

(Chapter 2, paragraph 2.7) and AFI 71-101 Volume-2, Protective Service Matters, (Chapter 1, paragraph 1.2). Contractor shall report to Security Forces any information or circumstances which may pose a threat to Department of Defense (DoD) or Contractor, resources, or DoD information.

11.2 Installation Access. Criminal History Check will be conducted on all prime/sub-contractor employees requiring base access. The Contractor shall provide the CO and the Information

Protection Office a current list of employees needing access. The list shall include employee's name, date of birth, state driver's license/state ID number and state of issue. Notifications of

Contractor employee additions and deletions shall be provided with the same information listed above and within 3 working days. The Government will notify the Contractor that installation access passes are available for those employees clearing the criminal history check. The duration of any pass issued will not exceed the duration of the contract.

11.2.1 National Agency Check with Inquiries (NACI) will be IAW Homeland Security Presidential

Directive 12 (HSPD-12).

11.3 Freedom of Information. The Contractor shall not respond to any Freedom of Information Act request or release any information in response to a Freedom of Information request. Any request for information received by the Contractor under the Freedom of Information Act will be referred to the CO.

11.4 Physical Security. The Contractor shall comply with Force Protection Condition (FPCON) procedures, Random Antiterrorism Measures (RAMS) and local search/identification requirements. The Contractor shall safeguard all government property, including controlled forms, provided for Contractor use. At the close of each work period, government training equipment, ground aerospace vehicles, facilities, support equipment, and other valuable materials shall be secured.

11.5 Key Control: The Contractor shall establish and implement methods of making sure all keys/key cards issued to the Contractor by the Government are not lost or misplaced and are not used by unauthorized persons. NOTE: All references to keys include key cards. No keys issued to the

Contractor by the Government shall be duplicated. The Contractor shall develop procedures covering key control that shall be included in the Quality Control Plan. Such procedures shall include turn-in of any issued keys by personnel who no longer require access to locked areas. The

Contractor shall immediately report any occurrences of lost or duplicate keys/key cards to the CO.

11.5.1 In the event keys, other than master keys, are lost or duplicated, the Contractor shall, upon direction of the CO, re-key or replace the affected lock or locks; however, the Government, at its option, may replace the affected lock or locks or perform re-keying. When the replacement of locks or re-keying is performed by the Government, the total cost of re-keying or the replacement of the lock or locks shall be deducted from the monthly payment due the Contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the Government and the total cost deducted from the monthly payment due the Contractor.

11.5.2 The Contractor shall prohibit the use of Government issued keys/key cards by any persons other than the Contractor’s employees. The Contractor shall prohibit the opening of locked areas by

Contractor employees to permit entrance of persons other than Contractor employees engaged in the performance of assigned work in those areas, or personnel authorized entrance by the CO.

11.5.3 Lock Combinations: The Contractor shall establish and implement methods of ensuring that all lock combinations are not revealed to unauthorized persons. The Contractor shall ensure that lock combinations are changed when personnel having access to the combinations no longer have a need to know such combinations. These procedures shall be included in the Contractor’s Quality Control

Plan.

11.6 Unescorted Entry to Restricted/Controlled Areas. If this contract requires unescorted entry to controlled or restricted areas, the Contractor shall comply with DOD 5200.2-R, Appendix 1, and

IAW AFI 31-501, Personnel Security Program Management (Paragraph 3.24). If the performance of this contract requires unescorted entry to a restricted/controlled area, personnel must have a favorably adjudicated NACI investigation. All Contractor employees affected by this requirement must have completed an acceptable submission that complies with all directions for completion, of the investigation request through the government security office within thirty (30) calendar days of

Contract Award or Notice of Award. To begin this process, all affected Contractor employees must complete within fifteen (15) calendar days after Contract Award or Notice of Award Standard Form

85P worksheet that can be downloaded from http://www.opm.gov/forms/pdf_fill/SF85P.pdf. The

Contractor shall notify the COR or Government Inspector when the worksheet is complete. The affected employee will then be scheduled by the government for two or more appointments to complete the security package. P a g e 10 | 12 disqualifying information is discovered or developed, the government reserves the right to deny entry to restricted/controlled areas. In this instance the CO will notify the Contractor of the denial.

That individual will not be allowed to perform duties requiring access to restricted/controlled areas.

Upon receipt of a favorable investigation results and authorization by the appropriate commander, the Contractor member will receive appropriate entry credentials for access to restricted/controlled areas, unless disqualifying information is subsequently discovered. In this instance access will be revoked. The NACI adjudication process normally takes two (2) to five (5) months after submission of the package.

11.7 Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately-owned vehicle while on JBSA.

12. CONTRACTOR MANPOWER REPORTING APPLICATION (CMRA): The Contractor shall report ALL Contractor labor hours (including Sub-Contractor labor hours) required for performance of services provided under this contract via a secure data collection site. The Contractor is required to completely fill in all required data fields at http://www.ecmra.mil.

12.1. Reporting inputs will be for the labor executed during the PoP for each Government fiscal year

(FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk.

12.2. Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and Contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the Contractor name and contract number associated with the data.

12.3. User Manual: Data for Air Force service requirements must be input at the Air Force CMRA link.
