Attachment 1 – Statement of Work

STATEMENT OF WORK

DELIVERY OF SHOWER TRAILERS

AT THE 322 TRS & 324 TRS

21 February 2018

1. SCOPE

This Performance Work Statement (PWS) covers requirements for the delivery of shower trailers at the 322nd Training Squadron, located at Bldg. 9110, 1315 Truemper St. JBSA Lackland, TX 78236, as well as the 324th Training Squadron, located at Bldg. 10416, 1315 Truemper St. JBSA Lackland, TX.

2. REQUIREMENTS

2.1.1. General Requirements

The contractor shall perform all actions outlined in the purchase contract, involving delivery of the shower trailers to 322nd TRS and 324th TRS. This includes drop off of trailers to the designated staging areas, as outlined in the 322/324 TRS siting proposal to 37 TRW. Furthermore, the Contractor shall remove all/all packaging materials from the facility/trailers and JBSA Lackland, upon completion of the project. See Appendix 1 to Statement of Work for specifications.

2.1.2. Installation

The contractor shall position the shower trailers at the designated staging areas (see Attachment 1) as follows:

322 TRS: Site 1

324 TRS: Site 2

The contractor shall furnish all tools, equipment materials, and manpower to accomplish the installation of the shower trailers. The contractor shall also remove all packaging materials from JBSA Lackland TX and shall be responsible for any refuse containers needed for trash and packing materials. Last, the contractor shall be responsible for returning the facility to its original state prior to installation and repairing any damage caused by the contractor’s personnel or equipment. Contractor is not required to complete any infrastructure support (i.e. plumbing, electrical, waste), the Government will be responsible for the installation of each trailer.

2.2. Delivery

Interaction between the contractor and their personnel with Basic Military Training (BMT) students (‘Trainees’ / ‘Airmen’) is prohibited. The contractor shall be responsible for off-loading items from delivery vehicles and shall provide any additional equipment to accomplish the task.

2.3. Site Program Manager/Point of Contact (POC)

The Contractor shall designate an on-site project manager as the Site POC for the 322 TRS and 324 TRS individual projects.

The Site POC or alternate(s) shall be available during normal duty hours (0730-1630hrs), until project completion. The Site POC shall be the interface for all work site communications with the Government, including quality, safety, and discrepancy matters.

2.4. Entry and Vehicle Control Requirements

Contractor shall comply with AFFARS Clause 5352.242-9000, “Contractor access to Air Force installations” which is hereby incorporated by reference if not otherwise included. Contractor personnel shall comply with all on-base traffic regulations.

Parking will be located on the East Parking Lot of B9110, off of Carswell Ave.

2.5. Testing

The Contractor shall conduct an operational check of all equipment and connections to ensure proper function of the furniture and will make any necessary adjustments/repairs.

2.6. Hours of Operation

The Contractor shall have access to the site from 0730 – 1630 Monday through Friday (excluding weekends (Saturday and Sunday) and federal holidays). The Contractor shall report to the site Facility Manager at the beginning of each day prior to the start of any work and at the end of each day prior to departing to discuss progress. The Facility Manager for this project is TSgt Jonathan French or TSgt Frank Zapata, who can be reached at 210-671-4205 / 4207

Anticipated observed holidays during period of performance:

28 May 18 (Monday) – Memorial Day

04 July 18 (Wednesday) – Independence Day

2.7. Warranty

Manufacturer’s warranties for individual pieces shall be provided along with contact information and operation manuals if any.

2.8. Site Tour

Upon request, a tour of the facility can be conducted to accommodate delivery and installation planning.

3. ACCEPTANCE

Upon completion of the work, the units shall be subject to a final acceptance test performed by the Contracting Officer (Mr.

Robert De La Rosa) and the Facility Managers of B9110 (TSgt Jonathan French / TSgt Frank Zapata) and B10416 (TSgt Brandyn Walter / TSgt Lukas Bartels / TSgt Richard Crotchie) or their designee.
