ADDENDUMS
ADDENDUM TO FAR 52.212-1 INSTRUCTION TO OFFERORS--COMMERCIAL ITEMS. The following paragraphs of this provision are tailored as follows:
(b) Submission of Offers: Offerors are responsible for submitting one original, hard copy proposal to include all items requested in this paragraph, and as stated in the solicitation. Proposals may be submitted electronically by emailing to: brooke.geigenmiller@us.af.mil and aaron.pearson@us.af.mil. Proposals must be signed/dated in blocks 30a-30c of the solicitation with the company's name/address/phone number and cage code provided in block 17a. Proposals must include completed copies of the SF1449 to include all provisions and clauses, as well as completed copies of all attachments identified in the solicitation. All pages requiring fill-in information must be returned with the offeror's proposal and must be received by the date/time provided in solicitation block 8, page 1, unless otherwise changed via solicitation amendment. Solicitation amendments must be acknowledged in writing or the proposal may be considered non-responsive. It is the offeror's responsibility to check www.
SAM.gov for amendments issued against this solicitation and respond accordingly.
Contractors shall submit questions pertaining to the solicitation package to the CA no later than 08 September 2022, 12:00 PM Central.
(b)(6) Price The offeror shall submit proposed pricing for all contract line item numbers (CLINs) identified in the solicitation. See Standard Form (SF) 1449 Continuation Sheet Schedule.
(b)(10) Past Performance Information (PPI)
a. Provide a list of no more than three (3) of the most relevant contracts performed for Federal agencies or commercial customers within the last three (3) years.
Recent contracts are defined as those on which some portion of performance occurred during the last three (3) years from date of receipt of proposal. Relevant performance includes present/past performance effort involving similar scope and magnitude of effort of this solicitation.
The offeror shall submit Attachment 3, Past Performance List of References, with your proposal for contracts that you consider relevant. The offeror shall ensure that the references listed in Attachment 3 are provided the Past Performance Questionnaire (PPQ), Attachment 4 for completion. If a CPAR is available for your references, annotate on Attachment 3 and no PPQ will be required.
Questionnaires shall be sent by the reference completing them directly to the Government Contract Specialist, Brooke Geigenmiller via e-mail brooke.geigenmiller@us.af.mil. This office strongly recommends all offerors request their references to submit their past performance questionnaires immediately to the Contract Specialist listed above.
b. The evaluation of PPI will take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to this solicitation. The type of past performance information (PPI) identified under Attachment 3 will determine which of the remaining paragraphs apply.
(1) For PPI where the offeror was the prime contractor; comply with paragraph c.
(2) For PPI where the offeror was a subcontractor; comply with paragraphs c. & d.
(3) For PPI where the offeror is using a subcontractor's PPI; comply with paragraph c. & e.
(4) For PPI where the offeror is part of a teaming arrangement; comply with paragraph f.
(5) For PPI where the offeror key personnel; comply with paragraph g.

e. If using PPI from subcontractors as part of list in Attachment 3, in addition to information requested in paragraph b. above, furnish the following information:
(i) What function(s)/task(s) did the subcontractor perform on the Atch 3 contract?
(ii) What function(s)/task(s) will the subcontractor perform on this contract?
(iii) What percentage of the total contract value/annual value did the subcontractor perform on the Atch 3 contract?

f. If a teaming arrangement or JV is proposed, the offerors shall provide complete information as to the arrangement, including any relevant and recent past/present performance information on previous teaming or JVs with same partner. In addition to information requested in paragraph (c) above for each team/JV member, furnish the following information:
(i) What function(s)/task(s) will each team or JV member perform on this contract?
(ii) What approximate percentage of the total contract value/annual value will each team or JV member perform on this contract?
g. If using PPI from key personnel as part of list in Attachment 3, in addition to information requested in paragraph c. above, furnish the following information:
(i) What function(s)/task(s) did the key personnel perform on the Atch 3 contract?
(ii) What function(s)/task(s) would the key personnel perform on this contract?
(iii) What percentage of the total contract value/annual value did the key personnel perform on the Atch 3 contract?

(c) Period of Acceptance of offers. The offeror agrees to hold the prices in its proposal firm for 90 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation.

ADDENDUM TO FAR 52.237-1 SITE VISIT
Site Visit Information An organized site visit has been set for Wednesday, 17 August 2022 at 8:45 AM CT. In order to gain access to the base to attend the site visit you must have a valid driver's license/identification card, valid registration for any vehicle you will be operating, and proof of insurance. A background check will be conducted on all individuals requesting a pass. Attendees are encouraged to begin the pass process at the Visitors Control Center (VCC) located at the Division Street Gate, 3300 C Street BLDG 6470, entrance to Keesler AFB at least 2 hours before the site visit scheduled on Wednesday, 17 August 2022 at 8:45 AM CT. Please arrive at the Contracting Office no earlier than 8:45 AM CT and no later than 9:00 AM CT. We will be departing around 9:15 AM CT, to

All site visit participants MUST have a base pass in order to attend. Unless you already have base access (i.e.
retired military ID card or have a current badge/pass due to an ongoing contract held on Keesler AFB), a pass must be obtained from the Keesler AFB Visitor Center.
Visitors Center hours of operation are Monday - Thursday 0600 - 1700. The Visitor Center is normally the least busy between the hours of 0600 - 0900. Please plan to get your base pass early, as we do not want to delay the site visit start time due to waiting on participants to get their pass. Participants will meet at the 81st Contracting Squadron, Bldg 4605 and will be transported to Bldg 233 on Wednesday, 17 August 2022 at 8:45 AM CT.
Participation is limited to NO MORE THAN 2 representatives from a company. An email containing the full name (as it appears on their identification card) of each site visit attendee must be sent to brooke.

Questions will not be entertained at the site visit. This visit provides interested parties the opportunity to inspect the site and formulate any questions. All questions shall be submitted in writing by email to brooke.
