Past Performance Questionnaire

The 338th Enterprise Sourcing Squadron is in the process of competitively selecting a source for Candidate Development Support Services (CDSS) III. Service Provider selected will be required to regionally deliver Special Warfare/Combat Support (SW/CS) professionally and technically experienced personnel to inspire SW/CS candidates through mentorship and a structured development process. Contracted personnel shall provide Air Force Specialty Code (AFSC) insight and job knowledge to better inform the candidate on their Air Force career choice. Contractor shall provide coaching on nutrition, strength/conditioning training, and mental resiliency as well as set and monitor targeted fitness goals/expectations for the candidates. The candidate development program is aimed to both regionally enhance first-contact with accessions as well as mentally/physically prepare individual candidates for the enduring demands of the SW/CS initial skills training.

One of the considerations in proposal evaluation is the verification of the Offeror's past performance on contracts or other work efforts which reflect the Offeror's ability to perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the Offeror, for the evaluation of the Offeror's performance on those contracts or work efforts.

Our areas of interest regarding the Offeror are summarized in the enclosed questionnaire. In order to meet the acquisition milestones, we request your written response no later than 10 calendar days after your receipt of this letter. This schedule will allow us sufficient time to analyze the data.

To assist you in preparing your response and expediting your reply, the questionnaire may be filled out electronically and e-mailed to daniel.carrillo.4@us.af.mil. Your completed questionnaire will become a part of the official Source Selection records.

Your assistance is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.

Exceptional (E)

Very Good (VG)

During the contract period, contractor performance is meeting (or met) all contract requirements and consistently exceeding (or exceeded) some. Some minor problems encountered. Contractor took timely corrective action.

Satisfactory (S)

During the contract period, contractor performance is meeting (or met) all contract requirements. For any problems encountered, contractor took effective corrective action.

Marginal (M)

During the contract period, contractor performance is not meeting (or did not meet) all contract requirements. For problems encountered, corrective action appeared only marginally effective, not effective, or not fully implemented. Customer involvement was required.

Unsatisfactory (U)

During the contract period, contractor performance is failing (or failed) to meet most contract requirements. Serious problems encountered. Corrective actions were either ineffective or non-existent. Extensive Customer oversight and involvement was required.

Not Applicable (N/A)
Unable to provide a rating. Contract did not include performance for this aspect. Do not know.
