DESCRIPTION OF SERVICES/ GENERAL INFORMATION

1. Introduction

1.1. Scope. The contractor shall provide all personnel, equipment, tools, materials, supervision, transportation and parts, to provide Dry Ice Block Delivery for Joint Base Andrews, MD.

1.2. Services Description

1.2.1. Contractor Requirements

1.2.1.1. Whole dry ice blocks wrapped so as to keep blocks separate and to keep blocks from freezing together / 50 lbs. per block / 1,000 lbs. per units ordered per delivery. Delivered in sufficient containers to sustain the integrity of the product. Estimated order average / week is 3,000-5,000 lbs.

1.2.1.1.1. Minimum order / delivery shall be 1,000 lbs. No

contract defined maximum. Dry Ice shall be ordered in units of 1,000 lbs. (1,000; 2,000; 3,000; etc.).

1.2.1.1.2. Contractor shall be notified of amount of dry ice

needed by the government for the following week’s delivery NTL COB Thursday of the preceding week.

Notice shall be via email so both parties have records of the transaction.

1.2.1.1.3. If no delivery is needed the following week, contractor shall be notified by the standard set in 1.2.1.1.2.

1.2.1.2. Containers shall be left to hold the dry ice blocks till the next delivery in which they will be switched out with stocked replacements.

1.3. General Information

1.3.1. Hours of Operation

1.3.1.1. Delivery every Monday to be delivered between the hours of 0730-1630.

1.3.1.2. Personnel who deliver the product will go to the Pearl Harbor Gate and contact TSgt Goosby or TSgt Cone @ (301-981-5522/3340) or Aircraft Services Dispatch (301-

981-2666) upon arrival at the gate.

1.3.1.3. Personnel will be escorted from the Pearl Harbor gate to the delivery destination, which is Bldg. 1900.

1.3.2. Base passes. The contractor shall obtain base identification and vehicle passes for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract.

Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation. The contractor shall submit a written request on company letterhead to the DD577 Holder for the 89 APS listing the following:

contract number, location of work site, start and stop dates and names of employees and subcontractor employees needing access to the base.

The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The DD577 holder will complete a short-term contractor access worksheet and will endorse the request, then forward it to the issuing base pass and registration office or security police for processing. When reporting to the registration office, the authorized individual(s) should provide a valid driver's license, current vehicle registration, valid vehicle insurance and social security number to obtain a vehicle pass.

During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.

1.3.3. Period of Performance: Proposed Agreement effective start date is 06 Apr 20 through 05 Apr 24.

1.3.4. Security Requirements. All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the Government installation, shall abide by all security regulations of the installation. The contractor shall sign an agreement stipulating the security requirements of this contract.

1.3.4.1. Security Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DOD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.

1.3.5. Performance of services during crisis declared by the Commander.

In the event the contractor is unable to perform his operations due to a peacetime emergency and or wartime contingency disturbance etc., neither the Government nor the contractor shall take action for noncompliance with contract requirements.
