About this file
This is a Summary of Work Amendment for a renovation project at Eglin Air Force Base to remove carpet and install vinyl flooring in rooms 111 and 112 of Building 351. The contractor must remove two layers of water and mold-damaged carpet (approximately 1500 SF) and install new Symbiotic 5.0 vinyl planks (6" x 48") in light gray or light brown with wood-grain pattern, along with dark grey cove base that coordinates with the vinyl planks.
The project requires the contractor to provide all materials, equipment, labor and disposal services, with dumpsters permitted on-site. All work must comply with NFPA and NEC requirements, UFC 3-600-01 standards, and NFPA 101 Chapter 10 for interior finishes. Special instructions include coordinating base passes/security access, providing adequate ventilation during work, submitting warranty and maintenance documentation before payment, and contacting 96th Civil Engineering if significant mold is discovered. An asbestos survey has been completed with no asbestos found. The project includes no civil, mechanical (HVAC), electrical, or communications work.
