1. DESCRIPTION OF SERVICES. The contractor shall provide all labor, personnel, equipment, tools, material, supervision and other items and services necessary to perform inspections, load testing, maintenance, evaluation and certification for one mobile crane (Appendix 3) all non-mobile cranes/hoists, bridges and monorails (see Appendices 1, 2, 4, 5 & 6) at Patrick AFB (PAFB) in accordance with (IAW) all local, state, and federal laws, base policies, Air Force Instructions and the requirements of this Performance Work Statement.
1.1 Principle Features. Contractor must provide all equipment, labor and preventive maintenance materials such as oils, lubricants and grease, to perform recurring maintenance.
Inspections shall include annual weight capacity test (Critical Load Cranes/Hoist only), preventive maintenance, major and minor repairs of all systems. The contractor shall provide annual Load Test Certificates”, “Crane Inspection and Maintenance Reports”, and Non Destructive Test Reports” for critical load Cranes/Hoist hooks. Contractor personnel shall be certified to inspect all hoists, cranes, bridges and monorails IAW OSHA, the Crane Manufacturers Association of America (CMAA 74), ASME/ANSI B30 Cranes, Hoists and Lifting Devices, ASME/ANSI B30.10 Hooks, and AFI 91-203 Chapter 35. Maintenance will include testing, cleaning, greasing, lubrication, and adjustment of motors, bearings, rollers, switches, etc. Maintenance will also include tightening/replacing nuts, bolts, screws, etc., inspect/repair/replace cables, wire rope, chains, clutches, brakes (discs), and end stops, repair/replace all electrical components to include motors, disconnects, loose wire, strobe lights, horns, lights, fuses, relays and bad connections. Preventive maintenance shall be performed as prescribed by the manufacturer as detailed in the owner's manual. All equipment used for repair or replacement of parts for recurring maintenance shall be supplied by the contractor (see paragraph 5).
1.2 Inspection, Maintenance, Repair, and Certification of Cranes / Hoists. All non-mobile cranes/hoists shall be inspected, maintained, repaired, and certified IAW 29 CFR 1910.179. The contractor shall also adhere to AFI 91-203. The initial inspection shall be a complete inspection for all cranes and hoists. Inspection frequencies for cranes/hoists are to be completed as to which category they fall into:
Category 1: Critical load Cranes and Hoists require monthly and annual inspections, annual NDI on the lower hook and annual weight load test and certification.
Category 4: Non Critical load Cranes and Hoists require annual and semi-annual inspections.
Prior to the scheduled inspection, repairs and/or maintenance, the contractor shall report to the 45 CEOE office located at 1060 South Patrick Drive, Bldg. 1060, PAFB, FL and sign into the Cranes/Hoists logbook for this contract. After inspections are performed, the contractor shall report to Bldg. 1060 and sign out of the logbook. Failure to sign in/out of this logbook could result in non-payment of services.
The government reserves the right to add or remove equipment during the life of this contract.
This will be accomplished via a change modification to the contract.
1.2.1 Catalog of Non-Mobile/Mobile Cranes/Hoists. The contractor shall prepare and submit to 45 CEOE a catalog of all cranes/hoists that have been inspected. The catalog shall include unit number, make, type (category), serial number, model number, and tonnage, including expiration date of inspection. At the end of each fiscal year, the catalog shall be updated and submitted no later than the first two weeks of the following fiscal year to the Contracting Officer (CO).
1.2.2. Catalog of Non-Mobile/Mobile Cranes and Hoists after Repair and Recertification.
All cranes and hoists that are repaired and recertified shall be inspected by the contractor and the catalog shall be updated with the repair description, repair date, recertification date and inspection date.
1.2.3. Load Limit Test. The contractor shall be responsible for providing weights for all weight load tests.
1.2.3.1. Cranes and Hoists. The contractor shall stencil the weight capacity of the cranes/hoists on the railing if this weight is not currently stenciled on railing or required by the referenced directives.
1.2.3.2. Mobile Cranes. The Mobile Crane shall be inspected and weight load tested annually.
Civil Engineering Heavy Repair/ Structures Section (CEOH) will provide an operator during the mobile crane weight load testing. The Mobile Crane weight load testing will be conducted during normal duty hours at Facility 697 using standard weights.
1.2.4. Load Test Schedule.
1.2.4.1. Non-Mobile Cranes/Hoists. All inspections, repairs and certification shall be accomplished prior to the expiration date on current Load Test Certificates. Annual certifications shall begin and be completed prior to the previous expiration date. Current expiration dates will be provided by the government IAW AFI 91-203.
1.2.4.2. Mobile Crane. The 30 Ton Mobile Crane shall be weight load tested and certified prior to the expiration date on current Load Test Certificates.
1.2.5. Reload Testing. In the event of unscheduled repairs requiring an additional load test certification (not including annual testing and certification) the Contracting Officer CO may consent to an additional cost to load test the crane(s) in order for it to be recertified IAW AFI 91- 203.
1.2.6. Parts and Repairs for Non-Mobile Cranes and Hoists Only. The contractor shall notify 45 CES and the CO or their representative within ten (10) workdays of any required repairs. All repairs must have prior CO approval. The contractor shall provide, in writing, a cost proposal to include a description of suggested repairs, a list of all parts to include manufacturer name, part numbers, serial numbers, cost, labor costs, and method of repair.
1.2.6.1. Once approved by the CO, the contractor shall make all repairs in order for the crane/hoist to be considered fully operational. Repair costs shall not exceed 50% of the crane/hoist replacement value. The contractor shall furnish parts specified by the manufacturer or of equal or better quality.
1.2.6.2. If repairs are disapproved by the Government, the cranes/hoist will be removed from service and "red tagged" by the contractor. The government will furnish red tags to lock out disabled equipment.
1.2.7. Reports and Certificates. Upon completion of inspection and/or certification of equipment, the contractor shall tag the equipment with the new date of certification within 48 hours of completing such work. Monthly Inspection reports shall be distributed to the Contracting Officer or their representative on a monthly basis. A copy of the Annual and Semi-Annual Inspection Reports and Load Test Certification will be distributed upon completion of said inspections to the using Agent, 45 CEOE, and the Contracting Office (45CONS) within 15 business days of completion.
1.3. Repair Service Calls For Non-Mobile Cranes/Hoists Only.
1.3.1. Routine Calls. 45 CEOE or the CO will notify the Contractor of needed repairs. The Contractor shall respond within 48 hours by providing all necessary personnel, tools, and equipment on site necessary to repair the crane or hoist and return it to serviceable and safe operation. If, for any reason, the Contractor is unable to immediately affect repairs, the Contractor shall make diligent efforts to return the crane or hoist to service as soon as possible. The Contractor shall survey the repair, and provide the CO with an estimated completion time and cost of repairs, not covered in the basic service contract. The Contractor shall commence repair work only after notification from the CO. Downtime of the crane/hoist will be kept to an absolute minimum. The Contractor must notify the customer and the CO of all projected downtime and estimated time for repair. Should removal of the equipment from the work site be required for repair, the Contractor shall ensure that adequate facility access is maintained until the original component is reinstalled. This may involve coordination with the facility manager. The Contractor shall prepare and submit a written report within five (5) work days after the repairs are completed. The report shall include the date and time of the service call, the location of the crane or hoist, the repairs performed costs of repairs, and the name of the technician performing the repairs
1.3.2. Emergency Service Calls for Non-Mobile Cranes/Hoists. In the event an emergency repair is necessary, the contractor must be available within 4 hours of being called. The time allotted for an emergency repair is 72 hours. Prior to emergency repairs, the contractor shall report to the 45 CEOE office located at 1060 South Patrick Drive, Bldg. 1060, PAFB, FL and sign into the Cranes/Hoists logbook for this contract. After emergency services are performed, the contractor shall report to Bldg. 1060 and sign out of the logbook. Failure to sign in/out of this logbook could result in non-payment of services.
The Contractor shall prepare and submit a written report within five (5) work days after the repairs are completed. The report shall include the date and time of the service call, the unit number of the crane or hoist, the repairs performed costs of repairs and the name of the technician performing the repairs.
1.4. Safety and Health Standards. The contractor shall be required to furnish signs, which will be placed when work is being performed, stating "Hoist being serviced - DO NOT USE" and "Overhead Work". The signs will be removed immediately after work is accomplished. The contractor shall also establish "clear zones" by barricading or roping off the area prior to performing hazardous/overhead work IAW AFI 91-203. When a Facility has more than one hoist, only one hoist will be serviced at a time, unless approved by the 45 CEOE office.
1.5. Proposed Schedules. For each fiscal year of this contract, the contractor shall submit proposed schedules for inspections and certifications to the CO and 45 CEOE not later than the one week after contract award. Coordination from each facility manager maybe required.
1.5.1. Contractor shall be responsible for contacting appropriate personnel at each listed facility to schedule hoist inspection, maintenance and load test at least 24 hours prior to arrival.
2. SERVICE SUMMARY (SS).
2.1 The evaluation results will be based on end user satisfaction. All services performed must meet the prescribed standards stated in the Service Summary (SS) to be regarded as acceptable.
Services considered unacceptable shall be re-performed at no additional charge to the Government.
Performance Objective PWS Para Performance Thresholds
Perform Maintenance. Provide Preventative maintenance service and inspections to all non-mobile cranes/hoists listed in Appendix 1, 2 and 4. Provide Annual Load Test and Non-Destructive Inspection of lower hooks for all cranes/hoists listed in Appendix 1, 2, 3, and 4 of Category 1.
IAW AFI 91-203 table 35-25

100% of all CAT-1 cranes and hoists must be operational and in good working condition.
Perform Inspection. Perform Annual and Semi-annual inspection on all Category 4. Crane/ hoists in Appendix 5 and 6. IAW 91-203 (Paragraph 35.1.3.3)
1.2
90% of all CAT-4 cranes and hoists must be operational and in good working condition.
Repair Service Calls. Repairs are made in a timely manner; Customers are informed of expected repair time.
Response and repair is made within the specified time.
1.3.1
Repairs are made within the specified repair time 100% of the time for CAT-1 and 90% of the time for CAT-4.
Emergency Calls. Repairs are made in a timely manner; Customers are informed of expected repair time.
Response and repairs should be made within the specified time.
1.3.2
Contractor responds to Emergency Calls within 4 hours after notification 100% of the time for all cranes.
Submit Maintenance and Repair Reports.
Provide and maintain accurate accountability of all preventative maintenance, repairs, inspections and load tests (if applicable) for all equipment listed in Appendices, 2, 3, 4 5 and 6.
1.2.6 1.3
All monthly maintenance reports shall be submitted to the CO no later than the 5th day of each month. Routine Service and Emergency Service reports are due to the CO Offices within 5 workdays of work completion.
3. SERVICES.
3.1. Utilities. The Government will provide electricity, water, and sewage service used in job areas if available. Hot water is generally not available in most facilities.
3.1.1. Emergency Services. The Government will provide police and fire protection. For all Security and Fire Emergencies call 911. When calling 911 from a cellphone, specify location and nature of emergency.
3.1.2. Emergency Medical Treatment. In case of an emergency medical situation, the contractor shall call 911 for assistance. When calling 911 from a cellphone, specify location and nature of emergency.
4. GENERAL INFORMATION:
4.1. HOURS OF OPERATION: The Government reserves the right to establish specific days and times for performance of any work under this contract.
4.1.1. Normal Hours: The Contractor shall perform required services under this contract during normal duty hours, 7:00 AM. - 3:45 PM, Monday through Friday. The Contractor may be required to coordinate with facility managers for specific work times.
4.1.2 Recognized Holidays*. The contractor is not required to provide service on the following days: New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Days, Thanksgiving Day, and Christmas Day. If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on a Sunday, it is observed on Monday. *Note: Any of the above holidays falling on a Saturday will be observed on the preceding Friday. Holidays falling on a Sunday will be observed on the following Monday.
4.2 Contractor Personnel. The contractor shall provide a point of contact (POC), to include a local or toll-free phone number, for all contractual matters during all hours of operation.
4.2.1. Authorized Personnel. The contractor shall provide a roster of contractor personnel authorized to work on the installation to the Contracting Officer not later than seven (7) days prior to the contract start date. This roster shall show employee names, social security number (SSAN), position title, security clearance, and proof of U.S. Citizenship. If changes occur, an updated roster shall be submitted to the Contracting Officer.
4.2.2 Appearance. Contractor personnel shall present a neat appearance and be easily recognized at all times through the use of clearly visible shirts and/or ball caps bearing the name of the organization performing this PWS. The compliance of contractor with this requirement shall be monitored by the contractor and the government will evaluate as a Quality Control Program issue.
4.2.3 Language. The contract manager and employees must be able to read, write, speak, and understand English.
4.2.4 Qualifications. The contractor shall ensure employees have verifiable professional certifications/experience prior to commencement of work on the contract.
4.2.4.1 Special Qualifications. Contractor personnel shall be certified by appropriate federal and state regulatory agencies to meet federal and local certification requirements in maintenance of cranes and hoists.
4.2.5 Employment Restrictions. The Contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest nor shall the Contractor employ any person who is an employee of the Department of Defense, either military or civilian, unless such person seeks and receives approval IAW DOD 5500.7-R, Sections 2-206 and 2-303. In addition, the Contractor shall not employ any person who is an employee of the Department of Defense if such employment would be contrary to the policies contained in AFI 64-106, paragraph 3.
4.2.5.1 The Government has the right to restrict the employment under the contract of any Contractor employee, or prospective Contractor employee, who is identified as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population.
4.3. Employees Work Location. The Contractor’s employees shall be restricted to those areas necessary to perform work under this contract as well as direct routes to and from the work site as approved by the Contracting Officer. Variations shall be approved by the CO prior to or before any deviation is made by the contractor.
4.4. Records. For each crane/hoist serviced, the contractor shall be responsible for creating, maintaining, providing the original record, or a reproducible copy of any such record of all repairs accomplished along with the certification of the crane/hoist to 45 CES and the CO.
4.5. Quality Control (QC). The contractor shall develop and maintain a control (QC) Plan to ensure maintenance and repair services and load testing services are performed in accordance with OSHA, CMAA, ASME/ANSI, NFPA 10, NFPA 17, NFPA 17A and other applicable industry standards or codes. One (1) copy of the Contractor's QC Plan shall be provided to the Contracting Officer and 45 CEOE not later than 15 days after contract award date. Updated copies shall be provided as changes occur. On-site records of all inspections conducted by the contractor and corrective actions taken shall be made available to the Government during the term of the contract. Inspections shall be performed and recorded monthly and shall be available to the government upon request. The QC Inspection Report shall be provided to 45 CES/CEOE and the CO on the same day that inspections are completed.
4.5.1 Records of inspections shall be kept and made available to the government throughout the contract performance period and for the period after contract completion until final settlement of any claims under this contract.
4.6 Quality Assurance (QA): A government representative from 45CES/CEOE will evaluate the contractor’s performance and adherence to standards. The 45CES/CEOE will obtain results of inspections, noting the facility number, date and time of inspection. If inspection reveals unacceptable performance, the CO shall be notified immediately. The CO will then notify the contract manager and require re-performance of the service (referred to as “call back”). The contractor shall be allotted time as agreed to by all parties, but no longer than 24 hours, to return to Patrick AFB and accomplish corrective call-back work.
4.6.1. Performance Evaluation Meetings. The Contract Specialist (CS) may be required to meet at least weekly with 45CES/CEOE office during the first month of the contract. Meetings will be as often as necessary thereafter as determined by the CO.
4.6.2 Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the CO.
4.6.3 The services to be performed by the contractor during the period of this contract shall at all times and places be subject to review by the CO or authorized representative(s).
4.7 Security Requirements. The Contractor and all employees shall comply with all Patrick Air Force Base security requirements in the performance of all work under this PWS. Security and base access requirements are contained in AFFARS 5352.242-9000, Contractor Access to Air Force Installations and 45 SW 31-1, Integrated Defense Plan.
4.7.1 ID Passes and Badges. COs are authorized to act as badge authorities on behalf of the contracted agencies for which they are responsible and may designate up to a maximum of two company personnel to act as Contractor Badge Authorities. If this action is deemed necessary by the CO, the request process outlined in 45 SW IDP 31-1 Appendix 4, Annex C, para 3.e. will be adhered to. Contractor Badge Authorities appointed by COs will only be allowed “sponsorship” not “escort” privileges. The contractor is responsible to obtain such clearances and/or passes as required for his/her employees to enter Patrick AFB, FL prior to the start of work (instructions will be provided at time of pre-performance conference). Applications shall be coordinated through the CO or Contractor Badge Authority. Badge requests must be in writing and include the visitor’s full name, Social Security number, driver’s license number with state of issuance, date of birth, company/unit, days and hours of access and the sponsoring agency. The request must be signed or endorsed by the sponsor or contractor badge authority. The request should be provided to the appropriate Visitor Control Center (VCC) 72 hours before the visit and should also be hand carried by the visitor when reporting to the VCC. At the time of application, Security Forces will conduct a background check on employees who will be required to provide two forms of identification of which one must be an authorized government issued photo identification card.
4.7.2 The prime contractor shall be responsible for all passes issued to his/her company to include subcontractor’s employees under this contract. Upon completion or termination of a contract or expiration of the identification badges/passes, the sponsoring agency will ensure that all base identification badges/passes issued to any employees or badged representatives are collected and returned to the issuing VCC within 24 hrs. Contractors are also required to return all installation access credentials for any employee no longer requiring access (e.g. termination, resignation etc.).
Prior to submitting an invoice for final payment, the contractor will obtain a clearance certification from the issuing office which states all base identification badges/passes have been turned in and/or accounted for and submitted to the responsible CO. Badged individuals may not transfer existing badges to a follow-on contract.
4.7.1.1 Sign in Procedures. During the course of this contract, there may be facilities or areas where the contractor employees will be required to sign in when they begin work and sign out when complete. The contractor is obligated to comply with these security requirements.
4.7.1.2 Reporting Lost Badges. Lost badges must be reported within 24 hours. The Contractor shall submit a letter to the Security Forces, through the Contracting Officer, explaining the details of the circumstances.
4.7.2 Contractor Vehicles. All vehicles will be in good operable condition, registered, licensed, insured, and meet all local, state, and federal safety requirements. The company name will be prominently displayed upon each vehicle and upon all equipment used in the performance of this
PWS.
4.7.3 Drivers License. Contractor personnel driving motor vehicles on Patrick AFB shall be in possession of a valid and current state driver’s license for the type of vehicle operated.
4.8. Project Safety. The Contractor shall initiate and maintain programs to comply with the provisions of the Occupational Safety and Health Standards Act (OSHA) concerning entry requirements in confined spaces and handling potential hazardous substances. All work shall be accomplished IAW all federal, state and local safety laws or instructions and any other measures as deemed applicable by the Base Ground Safety Office (45 SW/SEG, Phone: 321-494-7233).
4.8.1. Voluntary Protection Program: The contractor shall;
Perform work in a safe manner as required by OSHA Safety and Health Requirements.
Provide a verbal report to the CO as soon as possible of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact.
Provide a completed copy of required Accident Investigation Reports to the CO within five calendar days of each occurrence.

4.8.2. Safety. The contractor shall provide and place signage as needed to safely cordon areas and clear zones where cranes and hoists are being serviced. These clear zones shall be completely void of people and equipment not directly involved in service of the cranes and hoists.
During inspection, service, maintenance, or repairs, each affected crane and hoist shall be suitably identified as being out of service. This placard placement shall also include switches, controls, breakers, etc.
4.9. Environmental. The contractor is responsible for complying with all federal, state, Air Force, and local rules and regulations, all applicable environmental regulatory permits and all current 45 SW Management/Operating Plans to ensure environmental compliance at all times.
The contractor shall demonstrate compliance with all changes to rules and regulations during the contract period. The contractor shall complete all appropriate training necessary to meet the requirements of all environmental programs. The Air Force will provide web-based environmental training not specific to Federal and or State license/certification requirements. The contractor shall be responsible for ensuring all employees obtain and maintain necessary certifications and training. The 45 SW Environmental Offices is the single point of contact with all regulatory agencies concerning Air Force issues such as: regulatory interpretation, compliance reporting, inspections, and spills or releases. The contractor shall attend and provide necessary coordination and technical support for regulatory and Air Force inspections/audits. The contractor shall perform pre-inspections of contractor operated sites/systems prior to scheduled regulatory inspections.
4.9.1. Hazardous and Controlled Waste Management. The contractor shall;
Manage all hazardous, non-hazardous, industrial and universal wastes in accordance with all applicable federal, state, local and Air Force policies, regulations and laws.
Manage their hazardous waste accumulation sites, and ensure that all new and existing waste streams have been identified, in accordance with the 45 SW Hazardous Waste Management Plan, and all applicable policies, regulations, and laws.
Notify 45 SW Environmental Office of all changes in hazardous waste activities including, but not limited to, waste stream process modifications, the location or relocation of accumulation sites, and names of accumulation site managers and alternates.
Ensure that all employees who are responsible for hazardous waste management activities receive annual hazardous waste training and, where applicable, have job descriptions that meet the requirements of 40 CFR 265.16 and AFI 32-7042, paragraphs 2.6 and 1.10.1.11 respectively.
All hazmat employees (this includes all employees associated with hazardous waste) must receive the appropriate level of DOT training.
4.9.2. Petroleum Tanks/Systems and Spill Prevention Control and Countermeasures (SPCC). The contractor shall;
Ensure that any spill/leak/release of petroleum product is reported in accordance with (IAW) Patrick AFB Spill Prevention Control and Countermeasures (SPCC) Plan, Patrick AFB Facility Response Plan (FRP), 45 SW Hazardous Materials Emergency Response Plan.
Maintain all associated documentation on-site and in a current status for review during regulatory and Air Force inspections; all petroleum containers 55 gal or greater are subject to SPCC requirements.
Manage and inspect all applicable petroleum storage containers directly associated to contract operations IAW the SPCC requirements specified in 40 CFR 112, Patrick AFB SPCC Plan, Patrick AFB FRP and the 45 SW Hazardous Materials Emergency Response Plan. Applicable Contractor personnel shall meet all SPCC training requirements.
4.9.3. Air Program
The contractor shall manage, store and track hazardous materials to avoid exceeding the Risk Management Program threshold quantities identified in the 40 CFR-Part 68. The contractor shall track all chemicals IAW AFI 32-7086 and track all Ozone Depleting Substance (ODS) to ensure conformance with AFI Manual 32-7089.
4.9.4. Environmental Release Notification, Response and Clean-Up.
The contractor shall follow all spill notification and response procedures established in the Patrick AFB SPCC Plan, Patrick AFB FRP and 45 SW Hazardous Materials Emergency Response Plan.
The contractor shall discontinue use of all leaking/damaged equipment and if it is contractor equipment, remove it from the site until it is repaired.
If the Contractor spills or releases any HAZMAT or other substance into the environment, the Contractor shall immediately notify the CO/QA and appropriate emergency responders. The Contractor is responsible for all costs associated with clean-up and restoration, including any applicable fines and/or penalties. The Contractor shall maintain a spill plan as required by federal, state, and local laws and regulations.
4.9.5. Hazardous Material Management.
The contractor shall comply with, but not limited to: AFI 32-7086, Hazardous Materials Management, February 2015 and the 45th HAZMAT Tracking Procedures for Construction and Service Contractors. No hazardous materials are allowed to be brought on to any 45 SW installation without any prior approval. The contractor shall contact the 45 CES/CEIE Hazardous Materials Program Manager at 494-9268 or 494-9259 to review hazardous materials being brought on to 45 SW facilities.
The contractor shall;
Maintain copies of the Safety Data Sheets (SDS) for hazardous materials on site at all times and provide them to the Contracting Officer at the end of the project.
Manage Class I and II Ozone Depleting Substance (ODS) requirements in accordance with AFI 32-7086, and the 45 SW ODS Management Plan.
4.9.6. Pollution Prevention. The contractor shall participate in the 45 SW Recycling Program.
The program recycles all paper; cardboard, plastic bottles and aluminum drink containers, used oil filters, and toner cartridges. All materials recycled will be reported to the Recycling Contracting Officer Rep (COR) or 45 CES/CEIE.
The contractor shall ensure;
Recycled content products, energy star products, water efficient and bio-based products are considered in all purchases.
Where applicable, use environmentally safe products in the course of completion of their service.
4.9.7. Green Procurement Program. The contractor shall comply with the 45 SW Affirmative Procurement (AP) Plan 32-7080, Executive Order 13148, the Environmental Protection Agency’s Comprehensive Procurement Guidelines (http://www.epa.gov/cpg/products.htm). The contractor shall provide the minimum percentage of recovered materials to ensure compliance with the affirmative procurement requirements by considering the use of recovered content materials to the maximum extent practicable.
5. WARRANTIES/GUARANTEES.
5.1. The Contractor shall warranty all materials and workmanship done under this contract for a period of one year. The Contractor shall repair or replace, at his own expense, any materials that become defective during the warranty period as a result of imperfect material or workmanship.
5.2. All warranties shall become valid and operative upon the date of Government acceptance, as established by the Contracting Officer. Date of turn-on of mechanical equipment, electrical systems and sub-systems shall precede the date of Government acceptance in order for testing, balancing, interfacing, check-out and final finishing to proceed. Warranties shall not apply where damage is the result of abuse or neglect by the Government.
6. CONTRACTOR MANPOWER REPORTING APPLICATION. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Cranes and Hoists via a secure data collection site.
The contractor is required to completely fill in all required data fields using the following web address: https://afcmra.hqda.pentagon.mil/. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2019. 7. PUBLICATIONS/FORMS. The following instructions and guidance listed in Table 1 below shall be followed in the performance of this contract. Revisions to listed publications may be issued during the life of the contract. The contractor shall immediately implement those changes which result in a decrease or no change in the contract price. Prior to implementing any such revision that will result in an increase in contract price; the contractor shall submit to the CO a price proposal and obtain prior approval from the CO. Said price proposal shall be submitted within 30 calendar days from the date the contractor receives notice of the revision giving rise to the increase in cost of performance.
