FAMILY HEALTH CLINIC APPOINTMET ALGORITHM
How long has your current condition been going on?
Clarify the reason for Appt: What is the reason for your appointment? Examples: Illness, injury, etc.
Hello and thank you for calling the 6th Medical Group answering service. My name is __________. Are you calling to book an appointment or to leave a telephone consult for your PCM team?
Emergency Guidelines – Direct patient to nearest ER:
Loss of LIFE, LIMB, or EYE SIGHT Patients with a fever – Babies under 3 months with a fever (100.4 deg. Fahrenheit or higher) Animal Scratches & Bites Alcohol or Medication overdoses Severe Bleeding (Trauma/injury, Rectum, Vaginal or Urine), Vomiting of blood Any bones that is protruding from the skin Seizures of loss of consciousness in the last 24 hours Auto accident related injuries that have occurred the same day Head or neck injuries in the last 24 hours New onset of chest pain, severe difficulty breathing, severe abdominal pain, SEVERE/SUDDEN (<4 hours) numbness/weakness in limbs If a patient has any of the above, and they refuse the ER – inform them their only option is to report to the ER
Established patient. Seen previously within the facility and has a PCM assigned. FUTURE appointment
1. Book appointment with PCM
2. If no appointment, book within PCM team base on patient availability
3. If no appointment, book clinic-wide If no appointment, place a high priority t-con to the Family Health assigned PCM with subject “SYMPT” with the following format:
PCM name:
Call back number:
pts symptoms:
“Sir/Ma’am, your message will be forwarded to the PCM team, you should receive a call back within 2 hours. Thank you” Book at 24 hour symptom appointment
1. With PCM
2. If no appointment, within PCM team
3. If no appointment, book clinic-wide Greater than 2 weeks? Months? Years?
Less than 2 weeks?
If patient unable to agree with an appointment date or time, please place a t-con to the Family Health assigned PCM with subject “Appt” with the following format:
PCM name:
Call back number:
State appt was offered but pt declined “Sir/Ma’am, your message will be forwarded to the PCM team, they should returned you call within 3 business days”
