PERFORMANCE WORK STATEMENT

FOR

EXERCISE EQUIPMENT MAINTENANCE & REPAIR

SERVICES

MAIN BASE

STATEMENT OF WORK

FOR

EXERCISE EQUIPMENT REPAIR SERVICES

AT EDWARDS AFB CALIFORNIA

1. DESCRIPTION OF SERVICES. The Contractor shall provide all management, tools, supplies, equipment and labor necessary to maintain and repair Government owned exercise equipment located in Buildings 2200, 2209 and 2501. Service maintenance and repair tasks shall be in accordance with the procedures recommended by the respective equipment manufacturer specifications.

1.2. MONTHLY PREVENTATIVE MAINTENANCE. The Contractor shall ensure equipment listed in Exhibits 1 and 2 are maintained according to the manufacturer’s recommendations by performing preventative maintenance tasks by the 15th of each month. The Contractor shall ensure all equipment operates safely, and is clean and free of dust and grease build-up. The Contractor shall complete preventative tasks and return equipment to good working condition, clean and free of defects. The Contractor shall minimize disruptions and inconveniences to users. All replacement parts will be new or reconditioned and have a ninety-day warranty. All reconditioned parts meet manufacturer’s specifications for reconditioning.

Monthly Preventative Maintenance shall be in accordance with the manufacturer recommendations to include the following:

1) Check and ensure all movable parts are in proper working order.

2) Check all electrical components for function.

3) Check and clean all parts.

4) Inspect and adjust belts where applicable.

5) Lubricate as necessary.

6) Adjust and set according to manufacturer’s specification.

7) Check for worn parts, pulleys, cables, etc.

8) Check for rips or tears in upholstery and annotate as needed

1.3. REPAIRS. Repair work requests will occur when equipment malfunctions or fails to operate properly. The Contractor will be informed of the equipment malfunction and shall come prepared to correct the deficiency. The Contractor shall provide a price proposal quote to the Project Manager prior to proceeding with any repair work request.

The Contractor shall return equipment to operational condition within 72 hours after negotiation or unless otherwise approved by the Government to extend the time allotted.

The Contractor shall notify the Project Manager when any electronic components must be ordered and repair delays are expected. Any repair cost over $3,000 for parts and labor requires approval from the Contracting Officer.

1.3.1 Repair Work Request Procedures:

The following procedures shall apply for all repair Work Request pertaining to this contract:

(1). The Project Manager shall contact the contractor of the equipment malfunction.

(2) The Contractor shall submit a price proposal to the Project Manager. All work requests submitted with a total estimated value of less than $3000 will be sent to Project Manager for review and approval prior to performing repair work. All Repair Work Request of $3000 or more will be sent to both the Project Manager and Contracting Officer for review and approval. Approval will be done in writing. All oral approvals will be followed up in writing.

(3) The Project Manager shall:

(a) Promptly review the Repair Work Request for technical acceptability and reasonableness of the price.

(b) Verify that the proposed work is required, appropriate, and is not covered under the Preventive Maintenance contract line item(s).

(c) Verify that the funding is available for the Repair Work Request.

(4) The Project Manager shall inform and coordinate with the Contracting Officer in a timely manner on all requests over $3000.

1.4. PARTS. Contractor shall maintain a 10% bench stock of spare parts for all cardiovascular machines listed in Exhibits 1, 2 and 3. The bench stock of repair parts will be for all moving parts, not electronic components.

1.5. AUTHORIZED PERSONNEL. The individuals authorized to place service calls on behalf of the Government are located at 412 FSS/FSVS and AFTC/PZIOA. The contractor who receives the award will be provided with the information.

1.6. VERIFICATION PROCEDURES. Contractor shall upon entering any listed facility notify fitness staff and sign-in using the government’s contractor log book. All work started or completed must be listed in the Government’s contractor log book with description of services rendered. Upon completion of work the Contractor shall notify the Government representative that all work is complete. The Government representative shall inspect the repairs and sign off on the work order. Failure to obtain a signature may be grounds for rejection of the invoice.

2. GOVERNMENT FURNISHED SERVICES. Water and electricity will be furnished to the Contractor by the Government from existing system outlets at no cost to the Contractor.

3. GENERAL INFORMATION.

3.1. HOURS OF OPERATION. Normal duty hours for Edwards AFB are 7:30 A.M. to 4:30 P.M. Monday through Friday, except Federal Holidays and Installation Family Days. The Contractor may work, with prior AFTC/PZIOA approval, extended hours to ensure timely completion of work at no additional cost to the Government.

3.2. SECURITY REQUIREMENTS. All Contractor personnel shall obtain personal identification card (CAC) IAW Homeland Security Directives #12 and base regulations.

These forms may be obtained at the Security Forces, Building 3000. The Contractor shall turn in personnel identification card of any employee who is terminated, quit, or otherwise left employment of the contract on and upon completion/termination of the contract. All Contractor employees shall abide by all installation security regulations and procedures.

3.3. SAFETY. The Contractor shall comply with all fire and safety regulations at Edwards Air Force Base, in addition to Federal OSHA and Cal-OSH.

3.4. DAMAGES. Any Government or privately owned property damaged by the Contractor shall be replaced or repaired immediately by the Contractor at no expense to the Government.

3.5. EXCESS MATERIAL. All excess materials generated by the Contractor shall be removed from the job site daily and disposed of by the Contractor.

3.6. PROPERTY CONTROL. The Contractor shall obtain approval by the Government Quality Assurance personnel or the Contracting Officer before removing any item listed on Exhibits 1 or 2 from the existing location.

4. SERVICE SUMMARY.

Perform Preventative Maintenance 1.2. 90% completed week prior to scheduled deadline, the 15th of each month. Other 10% within 48 hours of 15th of each month

Perform Repair Work Request 1.3. Meet manufacturer’s guidelines 90% of the time;

Other 10%, not to exceed two verified customer complaints.

Weight Equipment Functionality 1.2, 1.3 95% of weight equipment will be operational during the month.

Cardiovascular Machines Functionality

1.2, 1.3 95% of cardio equipment will be operational during the month.

5. SECURITY

5.1. All contractor employees will be subject to a criminal history background check IAW AFMAN 31-113, Installation Perimeter Access Control.

Vehicle registration, proof of insurance and a valid driver's license must be presented for vehicles.

5.2. The contractor shall comply with all base security regulations.

Additionally, the contractor shall comply with actions dictated by force protection condition changes/measures.

5.3. The contractor shall report suspicious activity to base Security Forces at 661-277-3340 in a timely manner.

5.4. Contractor will be offered Level I - Antiterrorism Awareness Training by the government. This training provides contractor employees with the requisite knowledge necessary to remain vigilant for possible terrorist threats. Coordination for training should be done by the requiring unit after contract award.

5.5. The Contractor shall be responsible for safeguarding all government property, equipment and materials provided for Contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.

5.6. The Contractor and its employees shall comply with all base traffic regulations.

5.7. Contractor employees are prohibited from possessing weapons, firearms or ammunition, on themselves or within Contractor-owned or privately owned vehicle while on Edwards AFB.

6. QUALITY ASSURANCE.

6.1. QUALITY ASSURANCE. The contractor shall be responsible for quality assurance. The assigned government inspector(s) shall evaluate and approved all work before and after completion.
