SECTION I
1.0 DESCRIPTION OF SERVICES
1.1 General. This is a non-personnel services contract to design and construct load bank manifolds for avionics testing equipment at Building 1030, Edwards Air Force Base, California. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government.
1.1.1 Overview Description of Services/Introduction. The Contractor shall provide all management, personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to design and construct load bank manifolds for avionics testing equipment at Building 1030 as defined in this Statement of Work except for those items specified as Government furnished property and services. The Contractor shall perform to the standards in this contract. This includes the planning, coordination, and surveillance of the activities necessary to ensure disciplined work performance and timely resources application to accomplish all tasking under the contract. The Contractor shall be responsible for maintaining communication with the Contracting Officer (CO) and the government customer, and to immediately notify both the CO and the customer of any problems that would prevent timely performance of this contract. The Contractor is responsible for and required to implement, and maintain management control systems necessary to plan, organize, direct, and control all activities under this contract.
1.2 Background. The 412 EWG Site Support Division requires liquid cooling (PAO, EGW and Coolanol) aircraft devices built to be connected in the Anechoic Chamber to simulate aircraft pressure drops while operating existing liquid cooling systems. This will negate the need to move existing system cooling carts into the Chamber to perform various operational and maintenance procedures.
1.3 Inherently Governmental Function. The Contractor shall not perform Inherently Governmental functions as defined in FAR Subpart 7.5 in relationship to this SOW.
1.4 Period of Performance. 1 Aug 2018 - 30 Oct 2018 (90 days)
1.5 Place of Performance. The contractor shall perform the services in the Statement of Work at Building 1030, Edwards Air Force Base, California.
1.6 Specific Tasks.
1.6.1 General Description.
1.6.1.1 Contractor shall build three independent devices to indicate pressure and flow of PAO, EGW and
Coolanol fluid inside of the Anechoic Chamber on the turntable. The devices shall be constructed to be lightweight as possible to allow system transport to be hand carried by personnel as needed for utilization. Site survey will be required for the contractor to validate and verify the exact quick disconnects to be installed, address build design concept and integration components required to interface with the existing Simatec PLC.
1.6.2 Build Requirements
1.6.2.1 Contractor shall build each of the fluid devices to indicate pressure and flow digitally, connect with the applicable fluid quick disconnect (QD), have adjustable pressure drop and isolation, and integrate with the existing Simatec PLC.

1.6.2.2 Below is a generic list of probable required components.

1.7 Minimum Qualifications.
Construction of the required load bank manifolds must be performed by personnel with at least five years of experience performing maintenance and design of similar hydraulic operated systems. An understanding and experience performing work on similar systems is required as these units must operate seamlessly with highly unique pieces of precision test equipment. Failure to correctly build and test this system without negatively impacting the units to which they will be connected could result in USAF inability to support national test requirements.
1.8 Deliverables.
1.8.1 Below is a list of deliverables under this contract.

1.8.2 Data. The Government has unlimited rights to all deliverables of this contract.
1.8.3 Inspection. Deliverables will be inspected by the Government upon completion of work, prior to final acceptance. Progress meetings will be held as required.
1.9 Small Business Subcontracting Program (Small Business Utilization) When subcontracting opportunities exist, any Contractor receiving a contract award valued for more than the simplified acquisition threshold, shall submit a subcontracting plan for small business, small disadvantaged businesses, HUBZones, women-owned small businesses, service disabled veteran owned small businesses, veteran-owned small businesses, Historically Black Colleges and Universities and Minority Institutions, and Alaska Native Corporations and Indian Tribes. (A subcontracting plan is not required if there are no subcontracting opportunities, for contracts awarded to a small business, for personal services contracts or contracts performed entirely outside the US and outlying areas.) The plan shall contain goals for each of the categories, as well as 10 other elements identified (11 total) in FAR 19.704 and FAR Clause 52.219-9. This rating element assesses the contractor's performance in meeting the requirements of the plan and achievements against his negotiated goals. The contractor may use one of three types of plans: Subcontracting plan, commercial plan (for commercial items), and the DoD comprehensive (test) plan. (See FAR 19.704, 52.219-9 for additional guidance.) An Individual Subcontracting Report (ISR) shall be submitted semi-annually during contract performance for the periods ending March 31 and September 30. Reports are due 30 days after the close of each
Attachment 1 Statement of Work reporting period, unless otherwise directed by the Contracting Officer. A report is also required for each contract within 30 days of contract completion. Summary Subcontract Reports (SSR), including those submitted for comprehensive subcontracting plans under the DoD Small Business Subcontracting Plan (Test Program), shall be submitted semi-annually for the six months ending March 31 and the twelve months ending September 30.
Reports are due 30 days after the close of each reporting period. For commercial plans, a Summary Subcontract Report (SSR) is due 30 days after the end of the Government's fiscal year. For the CPARS rating, the Government will use the most recent ISR and/or SSR that the Contractor submitted and will not delay the evaluation if the rating period is different than the subcontracting reporting period.

SECTION II
2.0 GOVERNMENT FURNISHED PROPERTY, EQUIPMENT, AND SERVICES
2.1. Government Furnished Services. The Government will provide the following (as required)
2.1.1 Emergency Medical Services. Edwards AFB will provide emergency medical services for Contractor personnel in performance of services under this contract base IAW Air Force Instruction (AFI) 41-115 Chapter 1.
The Contractor shall reimburse the Government for treatment provided at the current reimbursement rate for non-authorized user IAW AFH 41-114, paragraph 42. The Contractor shall provide, to the Edwards AFB Medical Group, the name of a physician and/or hospital to which injured personnel can be transferred for further treatment and care after receiving initial emergency treatment on base. All mishaps will be promptly reported to the base safety office through the organization’s Safety Officer.
2.1.2 Security Police and Fire Protection. Phone number for Security Forces Squadron, 661-277-3340;
Emergencies (Fire/Security Forces/Medical), 911.
2.1.3 Utilities. The Government will provide all electricity, water and sewage at no additional cost to the Contractor. The Contractor shall instruct employees in utilities conservation practices. The Contractor shall be responsible for operating under conditions that preclude the waste of utilities, which include turning off the water faucets or valves after using the required amount to accomplish requirements under this SOW.
2.2 Government Furnished Materials. The Government will provide the following information: access to relevant Government organizations, information and documentation, manuals, texts, briefs, and associated materials, as required.

SECTION III
3.0 GENERAL INFORMATION
3.1. Hours of Operation. The services required under this contract will primarily be completed between the following hours: 7:30AM – 4:30PM, Monday thru Friday except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. Any additional work requiring an extended duty day or weekend shifts shall be coordinated through the government for approval no later than 24 hours in advance. For other than firm fixed price contracts, the Contractor will not be reimbursed when the Government facility is closed for the above reasons. On occasion, the contractor will be required to work on weekends and Federal Holidays. These instances will be worked out in advance between the Government and the Contractor. The Contractor must at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS when the Government facility is not closed for the above reasons.
3.1.1 Recognized Holidays. Contractor is not required/permitted to perform services on the following holidays.

*If the holiday falls on a Saturday, it will be observed on the preceding Friday. If the holiday falls on Sunday, it will be observed on the following Monday.
3.1.2 Work Schedules, Leave, and Absences. The contractor shall provide the government with a performance schedule for the project. Any changes in the schedule shall be coordinated with the government in advance.
3.2. Contract Personnel
3.2.1 Alcohol/Drug Use. The consumption of alcoholic beverages or illegal drugs by Contractor personnel, while on duty, is strictly forbidden. The Contractor shall immediately remove any employee who is under the influence of alcohol or drugs.
3.2.2 Appearance. Contractor personnel shall present a neat appearance. Contractor’s uniform must be easily recognized. This shall be accomplished by wearing distinctive clothing bearing the name of the company and by wearing badges that contain the company name and employee name.
3.2.3 Certifications. Contractor employees shall have current and valid professional certifications and licenses required to perform the work in this SOW, prior to contract award. All certification and licensing requirements by Federal, State, and local agencies shall be complied with at the Contractor's expense.
3.2.4 Contractor Qualifications. The Contractor shall utilize employees possessing adequate training, skills, and knowledge to perform the requirements of this contract.
3.2.5 Privately Owned Vehicles. Each Contractor employee driving a privately owned vehicle on the installation must carry minimum liability insurance as required by the State. Contractor employees shall adhere to all posted speed limits and shall not use hand-held cellular telephones while driving on the installation for phone calls or texting. Use of seatbelts is mandatory. Any Contractor employee cited for driving under the influence will have their driving privileges immediately revoked for a period to be determined.

3.3 Safety.
It is the contractor's responsibility to ensure its employees and managers have a comprehensive understanding of and full compliance with OSHA requirements. Detailed information is available on the OSHA website at http://www.osha.gov. It is the contractor's sole responsibility for compliance with OSHA, Public Law 91-596. The contractor shall comply with all federal, state and local laws, and Air Force Instructions for the protection of their employees while working on an Air Force Installation or property. The protection of contractor employees is exclusively the obligation of the contractor. Contractor shall manage all work areas to ensure the safety of building occupants, base personnel, or visitors in or near the areas. Contractor must ensure that personnel are properly trained on the equipment they are tasked to operate. The contractor shall develop site specific mishap reporting procedures which result in damage to government property or injury to Air Force personnel. These procedures are to be included in the safety and health plan for review. In accordance with AFI 91-202, AFMC Supplement, Safety Program, the contractor shall submit a Safety and Health Plan to Contracting for review and acceptance by the safety office prior to contract award.
The contractor shall perform work in a safe manner as required by OSHA Safety and Health Requirements.
Provide a verbal report to the CO as soon as possible of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a completed copy of required Accident Investigation Reports to the CO within five calendar days of each occurrence.
The contractor shall submit a Safety and Health Plan that describes the capabilities and processes to ensure compliance with safety and health requirements during the performance of the work described in the solicitation.
The Contractor must submit a Safety and Health Plan and corresponding site safety checklist to the Contracting Officer (CO) 10 days after contract award. The Contractor’s plan shall include appropriate measures to ensure the Contractor reacts promptly to investigate, correct and track alleged safety & health violations and/or uncontrolled hazards in contractor work areas.
The plan shall:
a. Demonstrate a management commitment to employee safety and health;
b. Identify the application of the safety and health plan to subcontractors;
c. Identify the roles and responsibilities of the following individuals:
Management;
Supervisors;
Employees;
Safety Coordinator;
d. Identify applicable safety rules and regulations;
e. Include a worksite hazard analysis to include base-line hazard identification and required control measures;
f. Include a job site analysis to include hazards of tasks required to control measures;
g. Identify employee safety and health training requirements and the documentation process;
h. Include a workplace inspection frequency, to include identifying the individual conducting the inspections;
i. Include employee hazard reporting procedures;
j. Identify individual(s) responsible for corrective action hazards;
k. Identify first aid/injury procedures;
l. Identify procedures for accident investigation and reporting;
m. Identify emergency response procedures; and
n. Identify the process for tracking controlled hazards in contractor work areas

3.3.1. Safety Equipment. The Contractor shall furnish personal safety equipment for its employees in performance of this contract. Safety equipment includes shoes or boots, eyewear, respirators, and gloves, etc.
3.3.2. Safety Threats. The Contractor shall not employ persons for work on this contract if such employees are identified to the Contractor by the CO as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population.
3.4. Conservation of Utilities. The Contractor shall instruct employees in utilities conservation practices. The Contractor shall be responsible for operating under conditions that preclude the waste of utilities.
3.5. Contractor Furnished Property and Services. Except for those items or services specifically stated to be Government-furnished in Section III and any appendixes, the Contractor shall furnish everything required to perform this contract in accordance with all of its terms.
3.6 Environmental Protection. The Contractor shall comply with all applicable Federal, state and local laws, regulations and standards that are or may become applicable to Contractor’s activities in performance of this
SOW.
3.6.1 Hazardous Material Identification and Material Safety Data. (a) Hazardous material, as used in this clause, includes any material defined as hazardous under the latest version of Federal Standard No. 313 (including revisions adopted during the term of the contract). (b) The offeror must list any hazardous material, as defined in paragraph (a) of this clause, to be delivered under this contract. The hazardous material shall be properly identified and include any applicable identification number, such as National Stock Number or Special Item Number. This information shall also be included on the Material Safety Data Sheet submitted under this contract.
3.6.1.2 Prior to use, the Contractor shall provide the Base Hazardous Material Pharmacy, through the Contracting Officer, a list of all hazardous materials the Contractor brings onto Government property and shall submit and AF Form 3952 along with a Safety Data Sheet (SDS) for each Hazardous Chemical listed in OSHA Hazard Communication Standard 29 CFR 1910.1200. The purpose of the inventory is to provide information for Emergency Planning and Community Right-to-Know Act (EPCRA) reporting.
3.6.1.3 Any products that meet the criteria of “Hazardous Waste” must be bar coded and tracked until permanently removed from Government property. Criteria for identifying hazardous waste are contained in Subpart C of 40 CFR Part 261. NOTE: Products containing chlorofluorocarbons (CFC’s) or ozone depleting substances shall not be allowed or approved for use.
3.6.2 Compliance with Environmental Management Systems. The Contractor’s work under this contract shall conform with all operational controls identified in the applicable agency or facility Environmental Management Systems and provide monitoring and measurement information necessary for the Government to address environmental performance relative to the goals of the Environmental Management Systems.
3.6.3 Health and Safety on Government Installations.
(a) In performing work under this contract on a Government installation, the contractor shall: (1) Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and Government personnel performing or in any way coming in contact with the performance of this contract; and (2) Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes.
(b) The contracting officer may, by written order, direct Air Force Occupational Safety and Health (AFOSH) Standards and/or health/safety standards as may be required in the performance of this contract and any
Attachment 1 Statement of Work adjustments resulting from such direction will be in accordance with the Changes clause of this contract. (c) Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract.
3.6.4 Permits. The Contractor shall be solely responsible for obtaining at its cost and expense any environmental permits required for its operations under the Contract, independent of any existing permits held by the Government. Any and all environmental permits required for any of the Contractor’s operations or activities would be subject to prior concurrence of Government. The Contractor acknowledges that the Government will not consent to being named a secondary discharger or co-permittee for any operations or activities of the Contractor under the Contract. In the event the Government is named as a secondary discharger or co-permittee for any activity or operation of the Contract, the Government shall have the right to take reasonable actions necessary to prevent, suspend, or terminate such activity or operation, including terminating this Contract, without liability or penalty.
3.6.5 Right to Inspect. The Government’s rights under this Contract specifically include the right for Government officials to inspect upon reasonable notice the work site for compliance with environmental, safety and occupational health laws and regulations, whether or not Government is responsible for enforcing them. Such inspections are without prejudice to the right of duly constituted enforcement officials to make such inspections.
The Government normally will give the Contractor twenty-four (24) hours prior notice of its intention to inspect, unless it determines the entry is required for safety, environmental, operations or security purposes.
3.6.6 Non-compliance. If the Contractor is found to not be in compliance with any of these regulations, there may be cause for the facility to shut down until Contractor, when within the Contractor’s control or the Government’s, has remedied the problem. Any sanitary deficiencies shall require immediate correction.
3.6.7 Spill Response. The Contractor is responsible to report and promptly clean up all spills in a manner consistent with current environmental regulations. In the event that it is necessary to utilize Government material, equipment, or personnel to clean up a Contractor caused spill, the Contractor shall be required to reimburse the Government for all associated costs.
3.7 Audit. This contract may be subject to audit by any and all cognizant Government agencies. The Contractor shall make available all records, which it maintains with respect to this contract, to such agencies.
3.8 Miscellaneous Paragraphs
3.8.1 Official Correspondence. The Government singularly identifies each project with a contract number at time of award. The contract number is a unique identifier to purposely and permanently represent an awarded project. The Government issued contract number is to be referenced on all official communication starting upon notice of award.
3.8.2 Post Award Conference. The Contractor agrees to attend any post award conference convened by the Contracting Officer IAW FAR Part 42.5. These meetings shall be at no additional cost to the Government.
3.8.3 Records. All records created and received by the Contractor in the performance of this contract shall be maintained and readily accessible. Records shall remain the property of the Government.
3.8.4 Insurance. The contractor shall obtain the necessary insurance as required by FAR part 28.3. The minimum amounts of insurance will be maintained IAW FAR 28.307-2.

3.8.5 Fire Prevention. The Contractor shall take all necessary precautions to prevent fires in the units and Contractor-furnished and Government-furnished facilities while performing any work under this contract. In the event fires do occur, the Contractor shall immediately notify the Base Fire Department at 911. The Contractor shall then notify the requiring activity and the CO. The Contractor shall brief and document its employees monthly in fire safety issues and procedures in reporting fires.
3.8.6 Inclement Weather. If the base closes due to inclement weather, the Contractor will be released, by the installation commander, with the general population of the base at no decrement to contract price. The Contractor will follow the local procedures for reporting in inclement weather as advertised on local television and radio stations.

SECTION IV
4.0 SECURITY REQUIREMENTS
4.1 All contractor employees will be subject to a criminal history background check IAW AFMAN 31-113, Installation Perimeter Access Control. Vehicle registration, proof of insurance and a valid driver's license must be presented for vehicles.
4.2 The contractor shall comply with all base security regulations. Additionally, the contractor shall comply with actions dictated by force protection condition changes/measures.
4.3 The contractor shall report suspicious activity to Security Forces at 661-277-3340 in a timely manner.
4.4 Contractor will be offered Level I - Antiterrorism Awareness Training by the government. This training provides contractor employees with the requisite knowledge necessary to remain vigilant for possible terrorist threats.
Coordination for training should be done by the requiring unit after contract award.
4.5 The Contractor shall be responsible for safeguarding all government property, equipment and materials provided for Contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.
4.6 The Contractor and its employees shall comply with all base traffic regulations.
4.7 Contractor employees are prohibited from possessing weapons, firearms or ammunition, on themselves or within Contractor-owned or privately owned vehicle while on Edwards AFB."
4.8 Controlled Area Badges. The Contractor shall obtain a Contractor Badge for all personnel from the 412th Electronic Warfare Group for entrance to restricted areas prior to commencing work under this contract. If in the event a “free zone” is not established prior to contract execution, contractor personnel will require government escorts to be provided by the 412th Electronic Warfare Group.

SECTION V
APPENDIX 1
DEFINITIONS
Arrival Services. Services performed to prepare for and properly park transient aircraft in an assigned parking area.
Contract Administrator (CA). The individual within the contracting office who performs the day-to-day administration of the contract. The contract administrator may also be the contracting officer.
Contracting Officer (CO). The duly appointed Government agent authorized to award or administer contracts.
The contracting officer is the only person authorized to contractually obligate the Government.
Corrective Action Report (CAR). Used to document unacceptable performance by the Contractor.
Defect. Any non-conformance with requirements specified in the contract.
Defective Service. A service output that does not meet the standard of performance specified in the contract for that service.
Fair Wear and Tear. The deterioration of items attributed to normal usage.
Government Furnished Property (GFP). Facilities, equipment, tools, supplies, parts, or any other items furnished for the concessionaire's use by the Government.
Major discrepancy. A major finding is defined as a condition that would endanger personnel, jeopardize equipment or system reliability, affect safety of flight, or warrant discontinuing the process or equipment operation.
Minor discrepancy. A minor finding is defined as an unsatisfactory condition that requires repair or correction, but does not endanger personnel, affect safety of flight, jeopardize equipment reliability, or warrant discontinuing a process or equipment operation.
Performance Assessment. A process that measures success towards achieving defined performance objectives or goals defined within the performance thresholds in the services summary or the process of assessing progress towards achieving the objectives/goals developed in a performance plan or partnering agreement.
Performance Indicator. A measurable characteristic of an output of a work process. Generally synonymous with attribute.
Performance Management. The use of performance measurement information to effect positive change in organizational culture, systems, and processes, by helping to set agreed upon performance goals, allocating and prioritizing resources, informing managers to either confirm or change current policy or program directions to meet those goals, and sharing results of performance in pursuing those goals.
Performance Objective. The outcome associated with successful contract performance in a specific area. This is a critical success factor in achieving the organization’s mission, vision and strategy which, if not achieved, would likely result in a significant decrease in customer satisfaction or risk mission failure. Obtaining multi-
Attachment 1 Statement of Work services/sub-services performed at a certain measurable standard and consistently ensures success in achieving the objectives critical to the mission.
Performance Threshold. The minimum performance level of a performance objective required by the Government.
Quality Assurance (QA). Those actions taken by the Government to assure services meet the requirements of the Statement of Work (SOW).
Contracting Officer Representative (QA or COR). Individual who monitors a Contractor on a daily basis and who is involved in every aspect of a contract to ensure the Contractor is in compliance with that contract.
Quality Assurance Program Coordinator (CORC). The individual designated by the contracting squadron commander to manage the installation QA program.
Quality Control (QC). Those actions taken by a Concessionaire to control the performance of services so that they meet the requirements of the SOW.
Standard.
