About this file

This document is a Price Schedule Exhibit D for a food service contract (SWIFT Event No. G0210-2000017183) involving the Bemidji Community Behavioral Health Hospital in Minnesota. The contract is for food service at one facility located at 800 Bemidji Ave N in Bemidji, with a required meal service capacity of 16 meals per mealtime. The pricing document outlines detailed nutritional requirements for different diet types, including regular diets (minimum 2200 calories), modified diets (with multiple specialized diet options), and double portion diets (3000-3200 calories).

The pricing schedule requires vendors to submit costs for various meal types, including regular, modified, and double portion diets, with specific pricing for breakfast, lunch, dinner, and three daily snacks. Vendors must provide emergency food supply pricing and a minimum service fee to cover fixed costs. Nutritional specifications are precise, including fat content limitations, fiber requirements, sodium levels, and dietary variety provisions. The document emphasizes that pricing should account for food preparation, packaging, delivery, serving equipment cleaning, and prepackaged food item costs. Responders are not mandated to offer pricing for all line items but are encouraged to provide comprehensive pricing across locations and meal types.
