The form requires vendors to make representations regarding Covered Telecommunications Equipment and Services under Section 889 of the NDAA. The form includes Yes/No checkboxes for each representation and requires signature by the merchant's authorized representative with the authority to bind the company. Vendors are directed to review the full Section 889 provision and clause in the Federal Acquisition Regulation (FAR) at Acquisition.gov before completing the certification and to return the completed form to their Contract Specialist.
