About this file
This document is a Past Performance Questionnaire for a Department of Veterans Affairs (VA) lease solicitation, Lease # 36C25625R0143. The form is designed to collect past performance information from potential lessors (landlords) by gathering references and evaluating their historical performance across eight key areas, including tenant improvement build-out, maintenance responsiveness, safety and security issue handling, building cleanliness, and overall lease performance. The evaluation uses a pass/fail scoring system where all questions must be rated "Acceptable" to receive a passing score, with a special provision that if a small business receives a fail rating, the Contracting Officer will seek a Certificate of Competency from the Small Business Administration. Potential offerors must complete Section 1, and if they have relevant past performance, must forward the questionnaire to three references who will complete Sections 2 and 3 detailing their experience with the lessor.

Introduction:
This questionnaire is designed to collect past performance information on Offerors (Landlord/ Lessors) competing for award of this lease. Instructions to the Offeror:
Please complete Section 1 on the following page. If Offeror checks the “no relevant past performance” box, then do not proceed any further in filling out the form and email (preferred), or mail the questionnaire back to the Contract Specialists. References must be current or former tenants of the Offeror. Once the respondents complete the remainder of the form, it must be returned to the Offeror and submitted as part of the Offeror’s proposal contents.
