About this file
This is an Exhibit J Site Visit document from the Los Angeles County Metropolitan Transportation Authority (LACMTA) outlining inspection procedures for tow and flatbed vehicle service providers responding to a Request for Proposal. The site visit consists of two mandatory components: vehicle inspection and facility inspection. An unscheduled site visit will be conducted by LACMTA representatives at the business address listed in the proposer's proposal. The vehicle inspection component involves the California Highway Patrol (CHP) performing a Level I Safety Inspection on one randomly selected light or medium duty tow or flatbed vehicle from the proposer's fleet inventory. All vehicles on the fleet inventory list are eligible for inspection, though waivers may be requested for vehicles that have been sold, possess Non-Operation documentation from the DMV dated prior to RFP submission, or have collision damage that would prevent inspection passage.
The CHP inspection evaluates 34 specific items against California Vehicle Code sections, including tire tread depth, fuel caps, fire extinguishers, signage, vehicle registration, license plates, lighting systems, brakes, windshields, mirrors, reflectors, turn signals, and wheel lift and sling assemblies. A vehicle fails inspection if it violates more than two items or fails any items marked as "Unsafe Vehicle" hazards, which result in automatic failure. The facility inspection component assesses the proposer's office capabilities, dispatch systems, filing systems, and operating yard. Both inspections are documented with detailed forms requiring signatures from CHP officers and towing company representatives, along with date, time, and observational notes.

EXHIBIT J
SITE VISIT
1.0 INSPECTION PROCEDURES
An unscheduled site visit will be conducted by representatives of the LACMTA. The site visit will be to the business address listed in the Proposal Letter, Exhibit 5. The site visit consists of two components: 1) Vehicle Inspection, and 2) Facility Inspection
1.1 VEHICLE INSPECTION PROCESS
The California Highway Patrol (CHP) shall perform the Level I Safety Inspection on one vehicle randomly selected by the proposer or authorized representative from the proposer’s light/medium duty tow/flatbed vehicle fleet inventory list submitted as part of the proposal package. LACMTA representatives will verify that all light and medium duty tow and flatbed trucks in the proposer’s fleet are included on this list. If the vehicle selected is not on site at the time of the selection, the truck shall be ordered to return to or be brought to the facility. Any vehicle on the inventory list will be eligible for inspection, however, waivers may be requested for vehicles that have been sold, vehicles with Non-Operation documentation from the DMV dated prior to the RFP submittal date, and any vehicle that would not pass inspection specifically due to collision damage or mechanical repair. The proposer must notify Metro if any of the vehicles on the Vehicle Fleet Inventory list have been sold or inoperable where the damage to the vehicle will not allow the vehicle to pass inspection. CHP will verify the damage and determine if the vehicle can be inspected.
All items listed in Table 1 shall be inspected. Proposer or their representatives shall not place items in or on the truck once the LACMTA representatives arrive. Each item listed in Table 1 is equal to one violation with the possible exception of the rear taillight. For example, if the right side taillight is out, the right turn signal, four way flasher and right side brake light may all be affected by one broken bulb. In this instance, the tow company would be cited for one violation rather than four violations. Also, if any of the Unsafe Vehicle items are checked, the vehicle immediately fails the inspection.
