About this file
This is a Pre-Operational Inspection Form for the Metro Freeway Service Patrol (FSP) program, a service operated by Metro. The form establishes mandatory vehicle and personnel inspection requirements that must be completed and verified prior to the start of each FSP shift. The inspection checklist comprises 38 items covering driver documentation (valid California Driver License, FSP Identification Card, DMV Medical Card, and tow truck certificate), vehicle registration and insurance, operational equipment (FSP radio, GPS, shop radio, and public address system), safety equipment (fire extinguisher, absorbent materials, fuel and water containers), vehicle condition (cleanliness, brakes, tires, lights, and body integrity), and contractually required tools and accessories. The contractor or contractor's representative must initial the form only after confirming that the driver and vehicle meet all listed requirements and FSP contract specifications. The form is valid for designated dates and must remain in the vehicle during the valid period, with separate columns provided for Monday through Friday inspections.
Drivers and vehicles that fail to meet all requirements specified on the inspection form and within the FSP contract are prohibited from providing service. The form identifies that this inspection protocol is effective as of October 2025 and applies to all contractors and subcontractors operating vehicles in the Metro FSP program. Compliance with this pre-operational inspection is a contractual obligation, and the form serves as documentation of adherence to Metro's operational and safety standards for the Freeway Service Patrol service delivery.
