About this file

This document is an exhibit defining mandatory data entry fields for the CDEC Salesforce Database used by the Protective Services, Safety and Family Preservation (PSSF) Program. The exhibit outlines ten distinct data collection categories required for case management and service delivery tracking. Required fields include household account information (account name, phone number, county, number of adults and children, housing situation, and addresses), contact records for primary caregivers and children (first name, last name, gender, birthdate, race/ethnicity, Hispanic/Latinx status, tribal member status, and primary spoken language), and new PSSF client case documentation (case start date, record type, status, household, site, county, and supervisor designation). Additional mandatory data entry includes Data System Consent Forms, CFSA 2.0 Assessments for families receiving intensive case management or six or more hours of services within three months, PSSF Services tracking (service provided, start date, service category, individual receiving service, and associated activities), and Goal Setting documentation with goal names and topics.

The exhibit emphasizes that while certain fields are marked as mandatory in the system and highlighted in the PSSF Salesforce User Guide, best practice involves completing as much information as possible for each entry. Demographic fields offer options to decline or select "unknown" as responses. The document notes that field designations and mandatory requirements may change with system updates. Documentation requirements include maintaining backup copies of signed Data System Consent Forms and completing all required sections of Parts A and B in CFSA 2.0 Assessments. Optional data entry categories include Resource Referrals, Flex Funding, and Client Case Closure procedures, which require closure of all active services before changing case status to closed.
