About this file
This is an Exhibit H Required Affidavit document for RFP #110118, an OCORA Campground solicitation issued by a Kentucky state or local agency. The affidavit serves as a mandatory submission requirement that all bidders and contractors must read, sign, notarize, and return with their bid proposal. The document establishes compliance requirements and declarations that bidders must make under penalty of perjury as a condition of contract award consideration. No specific products, services, quantities, response dates, bidder meetings, site visits, or contract term details are included in this affidavit document, as it functions solely as a certification and attestation requirement rather than a project specification document.
The affidavit requires bidders to affirm compliance with Kentucky campaign finance laws (KRS 45A.110 and KRS 45A.115), disclosure requirements under KRS 45A.485, Kentucky Department of Revenue sales and use tax registration requirements, and state tax payment obligations. For non-bid contracts specifically, additional affirmations are required regarding contributions to gubernatorial and candidate campaigns under KRS 121.056, KRS 121.330(1)-(4), with limits on direct contributions and solicitation amounts. The document requires a notarized signature from a duly authorized representative and explicitly states that inaccurate disclosure may result in contract termination, fund repayment, and other legal remedies. No pricing, set-asides, funding sources, or incumbent information is addressed within this affidavit document.
