About this file
The document is a Compost, Mulch, and Renewable Natural Gas Procurement Reporting Form from the County of San Mateo, California, designed for contractors, vendors, and county employees to report their quarterly procurement of compost, mulch, and renewable natural gas (RNG). The form must be submitted at the beginning of each quarter with details on procurement activities from the previous quarter, including product name, quantity, date of procurement, supplier information, product use details, and application location. Vendors are required to email the completed form and corresponding invoices to SUST_SB1383Compliance@smcgov.org.
The form provides a standardized template with columns for tracking procurement specifics, such as the unit of measurement (tons, diesel gallon equivalent), quantity procured, product supplier contact information, and specific use cases. The example entries show procurement of 50 diesel gallon equivalents of renewable natural gas for transportation fleet use and 2 tons of compost applied in landscape installations at the County Center in Redwood City. The form emphasizes accurate and complete reporting, with a certification statement requiring the vendor or county employee to verify the information's correctness.
