About this file

Document Summary

This document is an income verification calculation worksheet issued by the Arizona Department of Economic Security (DES) for use in the Rapid Rehousing (RRH) program. The worksheet is designed to determine client income eligibility for state and local rapid rehousing assistance and includes two primary components: a Declaration of No Income form and an RRH Income Verification Worksheet for Youth & Families. The form serves as an official assessment tool for DES Youth & Families Rapid Rehousing Program providers to evaluate household financial circumstances and document income status.

The worksheet establishes that household income must be at or below 50% of the Area Median Income (AMI) for the applicable geographic area to qualify for assistance. Income eligibility determinations use HUD income limits in effect at the time of verification rather than limits corresponding to the grant year under which funds were awarded. The income calculation process requires providers to document all sources of income including employment, government benefits, assets, and third-party support, utilizing either the HUD CPD Income Calculator or self-certification forms. Income certification must be completed after one year of financial assistance provision, with total annual income calculated by multiplying gross amounts by pay period frequency and summing household member contributions. The document includes detailed questioning to verify zero-income status and requires written verification from income sources when clients report receiving assistance, establishing accountability measures for accurate income reporting and program compliance.
