About this file
This document is an income verification calculation worksheet developed by the Arizona Department of Economic Security (DES) for use in the Rapid Rehousing (RRH) program. The worksheet serves as a standardized tool for DES Youth & Families Rapid Rehousing Program providers to determine client income eligibility and document financial circumstances for clients receiving rapid rehousing assistance.
The worksheet establishes that household income must not exceed 50% of the Area Median Income (AMI) for the client's geographic area to maintain eligibility for the RRH program. Income eligibility is determined using HUD income limits in effect at the time of verification, with calculations derived from third-party documentation or client self-certification. The document includes a Declaration of No Income section requiring clients to attest under penalty of perjury that they have no income from sources including employment, unemployment benefits, Social Security, disability benefits, spousal or child support, or self-employment. Clients must provide written verification from income sources if they answer affirmatively to questions about receiving financial assistance for bills, food, utilities, transportation, vehicles, credit obligations, or phone services. Income calculations multiply gross amounts by pay period frequency to determine annual income, with total household income compared against 50% AMI thresholds. Income certification must be completed after financial assistance has been provided for one year, with providers required to use the HUD Exchange Income Calculator and print calculations for client files.
