About this file

This document is an Itemized Service Budget (ISB) form and instruction guide for a state and local contract award issued by the Arizona Department of Economic Security (ADES) for Housing Support Services targeting homeless youth and families. The contract period extends through June 30, 2026, and covers two service codes: YFS (Youth/Families Emergency Shelter) and YFR (Youth/Families Rapid Rehousing). The organization executing the contract is identified as TBD, with a contract number also listed as TBD.

The ISB template establishes budget categories for allowable costs across both service delivery areas, including: total personnel and employee-related expenses for direct services; shelter diversion services; essential client services such as case management, childcare, education, and employment assistance; emergency shelter operation expenses; housing vouchers for rapid rehousing assistance (limited to 12 months and compliant with HUD Fair Market Rent standards); rapid rehousing stabilization and financial assistance including security deposits, utility payments, and moving expenses; and indirect costs using either a 15% de minimis rate or federally negotiated rates. The form requires contractors to identify all revenue sources beyond ADES funding and to separate total service costs from ADES-specific costs. All budget line items are currently listed at $0.00, indicating this is a blank template awaiting completion by the service provider. The contract requires that all costs be allowable, allocable, reasonable, and eligible for service delivery as defined in the Scope of Work.

YFSYFR ISB Instructions An Itemized Service Budget (ISB) for the contract period for State Fiscal Year 2026, as follows:

* Utilize the ISB form to identify Total Service Costs and ADES Costs allocated to the Service Delivery Area. Total Service Cost column is to be utilized to account for all expenses involved in carrying out the service. ADES Cost column is to be utilized to account for only the expenses that an Offeror will be charging to ADES to carry out the service.

* Ensure that costs are allowable, allocable, reasonable, and eligibile for the delivery of the service(s) (See Scope of Work, Section 8 Payment for Services);

* Place a zero (0) in the column(s) for any budget category where no costs are anticipated;

* Breakout the costs under each budget category;

* Identify all revenue sources (Offeror is to list out all the revenue sources include ADES and the amounts they are receiving from each source to provide this service); and

* Utilize the information in the following table to determine appropriate costs for each budget category.
