About this file
This is a Vendor Questionnaire template designed for prospective offerors responding to the Mobile MRI Lease solicitation for the Department of Veterans Affairs Veterans Health Administration, Veterans Integrated Service Network 17.
The questionnaire serves as an evaluation tool to assess vendor qualifications and capabilities for providing mobile MRI equipment with technician services at the Central Texas Veterans Health Care System Austin VA Outpatient Clinic (7901 Metropolis Drive, Austin, TX 78744-3111). The solicitation (36C25726Q0525) is an RFQ issued as a combined synopsis/solicitation for commercial products and services, set aside exclusively for Service-Disabled Veteran-Owned small business concerns. The requirement encompasses 12 months of mobile MRI service with technician support during the base period (September 30, 2026 through September 29, 2027), with four additional one-year option periods available. Offerors must complete the vendor questionnaire as part of their proposal submission to demonstrate their ability to meet the technical, operational, and service requirements outlined in the attached Statement of Work. The questionnaire likely requests information regarding vendor experience, staffing qualifications, equipment specifications, service delivery capabilities, and other relevant factors that will be used in evaluating proposals for contract award.
