About this file
Exhibit B is a Logic Model document associated with the City of Phoenix Human Services Department's Youth Crime Prevention Program RFP (HSD-RFP-26-0074). The program targets juvenile crime reduction across four Phoenix neighborhoods—Maryvale, South Phoenix, Alhambra, and Central City—through a three-year professional services contract commencing January 1, 2026. The initiative will provide intervention services for youth aged 12-17, encompassing violence prevention, substance abuse prevention, after-school enrichment, youth mental health services, dropout prevention, and family support strategies. Key deliverables include 360 mentoring hours, 48 group family workshops, and 540 hours of after-school recreation utilizing the Eisenhower Quantum Opportunities Curricula. The solicitation was released August 25, 2025, with a September 12, 2025 deadline for questions, September 19, 2025 response deadline, and October 17, 2025 proposal submission deadline at 3:00 p.m. Proposals will be evaluated on nine criteria, with highest weighted categories being Program Design and Methodology (20 points), Organizational Capacity and Staffing (15 points), and Evaluation and Logic Model (15 points).
The contract value is $500,000 annually with options to extend for two additional years at the same rate, subject to fund availability. The program implementation will occur across four locations: Cesar Chavez High School, Alhambra High School, ABC Community Center, and City of Phoenix Burton Barr Library, with a staffing model of 3 Full-Time Equivalents and 3 volunteers. Contractors must demonstrate 3-5 years of experience in youth-focused crime prevention programming, cultural competency in high-crime neighborhoods, grant management experience, and established partnerships with schools, law enforcement, and community organizations. Disadvantaged Business Enterprise (DBE) participation requirements mandate contractors track and report DBE engagement through the City's Certification and Compliance System and demonstrate good faith efforts including letters to potential suppliers and active recruitment through the Arizona UTRACS database.
