About this file

This is an Exhibit B budget detail and payment provisions document for an Optometry Services contract (PR269002) between the State of California and Porterville Developmental Center. The contract will provide optometry services to individuals at Porterville Developmental Center from July 1, 2026 through June 30, 2029. The contractor must be a duly licensed optometrist with expertise in the field and must have a minimum of five years of experience working in state hospitals, developmental centers, or prison settings. The contractor must also demonstrate cognizance of the special needs of developmentally disabled individuals and be capable of supplying all specified services in accordance with the sample agreement. All bidders must submit copies of their licenses with their bid documents, and staffing agencies must have qualified personnel in place at the time of bid submission.

Compensation shall be paid in arrears at a specified rate per full 4-hour clinic session for services performed to the State's satisfaction, with a total budget not to exceed a specified amount for the contract duration. The contractor may not bill any other third-party payer for additional payment beyond the contracted rate, as payment under this contract constitutes payment in full for services rendered. All invoices should be submitted to Brandi McElhaney, Contract Analyst, at Porterville Developmental Center with the contract number and itemization of services provided and time period covered. The contract is subject to budget contingency provisions whereby if the Budget Act fails to appropriate sufficient funds, the Agreement becomes void with no liability to the State. Additionally, all payments shall be made in accordance with the California Prompt Payment Act (Government Code Section 927 et seq.), and any budget modifications require written request with justification and revised budget documentation, subject to State approval.
