About this file

This document contains an exhibit for evaluating contractor safety performance on federal construction projects. The evaluation form requests contractors to provide health and safety metrics including man-hours worked, OSHA recordable injuries, days away from work cases, and serious safety violations over the past three years. Contractors must also submit current OSHA forms 300 and 300A and identify their safety program administrator as well as current insurance experience modification rate and explanations for any rates over 1.0. The related federal contract opportunity is a solicitation from the Department of Veterans Affairs for exterior weatherproofing work at the Jackson VA Medical Center.
