About this file
This is a Logic Model exhibit for a Youth Crime Prevention Program being solicited by the City of Phoenix Human Services Department in Arizona. The program targets juvenile crime reduction across four neighborhoods (Maryvale, South Phoenix, Alhambra, and Central City) and will serve youth aged 12-17 through comprehensive intervention services. Key deliverables include 360 mentoring hours (comprised of 60 individual and 60 group sessions), 48 family-centered group workshops or therapy sessions, and 540 hours of after-school recreation programming at two high schools (Alhambra and Cesar Chavez) from 3:00 pm to 6:00 pm. The program aims to enroll 120 youth and engage 60 families. The three-year contract will commence on or about January 1, 2026, with a proposal submission deadline of October 17, 2025 at 3:00 pm. The solicitation number is HSD-RFP-26-0074, released on August 25, 2025, with a September 12, 2025 deadline for questions and September 19, 2025 response deadline.
The total contract value is $500,000 annually with options to extend for two additional years at the same amount, subject to fund availability. Implementation will occur across four locations: Cesar Chavez High School Auditorium, Alhambra High School Library, ABC Community Center, and City of Phoenix Burton Barr Library. The program requires 3 Full-Time Equivalents and 3 volunteers, supported by community partnerships with Cesar Chavez High School, Alhambra High School, and West Valley YMCA. The curriculum will utilize the Eisenhower Quantum Opportunities Curricula, rated effective by the Department of Justice. Program outcomes are measured across three timeframes: short-term (3-6 months) improvements in school attendance and conflict resolution skills; intermediate (6-18 months) reductions in truancy and law enforcement contact; and long-term (18 months-3 years) decreases in youth crime and increased graduation rates. The procurement includes mandatory Disadvantaged Business Enterprise participation requirements and tracking through the City's Certification and Compliance System. Contractor evaluation criteria include Program Design and Methodology (20 points), Organizational Capacity and Staffing (15 points), and Evaluation and Logic Model (15 points), with preferred qualifications including 3-5 years of youth crime prevention experience and established partnerships with schools and law enforcement.
